fix(server): bypass owned caches on explicit provider refresh

Explicit provider refresh could reuse a fresh model manifest, cached discovery results, and update metadata, leaving the model picker stale after clicking Refresh.

The existing explicit refreshModels request now fetches the manifest before probing providers, bypassing its freshness and retry timers. It invalidates T3-owned Cursor model discovery, Claude capability probes, maintenance resolution, and package-version caches for the selected instances. Background checks keep their timers, failed manifest fetches preserve the last good copy, and provider update-check opt-out remains respected. Third-party caches, including Codex's models_cache.json, are untouched. The shared server RPC covers web, desktop, and mobile without a wire-format change.

• 49 tests passed in ModelManifest.test.ts and CursorProvider.test.ts.
• Four focused server RPC tests passed, covering refresh ordering, targeted invalidation, ordinary status checks, and reconnect behavior.
• Server typecheck and targeted lint passed. The macOS test run uses TMPDIR=/private/tmp to avoid existing temporary-path alias mismatches.

This changes server cache behavior without changing UI rendering. The evidence is focused tests that verify HTTP fetch counts, cache persistence, real Cursor ACP discovery requests, and cache state before the RPC's provider probes; screenshots are not applicable.

Reviewed CodeRabbit's outside-diff suggestion to remove the existing refreshModels pass: keeping it is necessary. Antigravity is the only driver exposing this hook. Its normal provider check updates availability/version/auth status while retaining the catalog. The explicit hook starts a disposable session and publishes the discovered catalog. Removing it would break explicit Antigravity model reload. The subsequent registry refresh returns that newly published state; other drivers do not enter this loop. This block predates the PR and the new cache invalidation does not duplicate Antigravity discovery.
