fix(clients): hide disconnected environments when adding projects

Add Project listed every known environment, including connections switched off in Settings. With one connected environment and one disabled environment, this produced an unnecessary chooser containing an unusable destination.

Filter Add Project options to connected environments on web/desktop and mobile. The web flow now skips the environment chooser when only one is connected, and directs users to Connections when none are connected.

Validation: 51 focused tests passed; web and mobile typechecks passed; targeted lint passed with existing warnings. Verified the web command-palette flow against isolated development state with a saved, disabled environment. Native mobile UI was not exercised.
