perf(server): stop re-running git for every project each minute

The server re-runs git for every project each minute. Two background sweeps (thread settlement and PR discovery) resolve the repository identity of every project once a minute. PR sync did too until #13704. The identity cache also lived for 1 minute, so each entry expired before the next sweep. Each git project cost 2 git spawns per minute. A folder that is not a git repository was not cached at all, so it spawned git on every call.

This came from a long-uptime slowdown report (T3 Code 0.0.40, 9 days uptime, 130 projects). That server made about 295 process spawns per minute with the window closed. The report attributes about 15k of its 16.8k spawns per hour to these identity lookups. It also recorded about 18k RepositoryIdentityResolver.resolve spans per hour.

Fix

In apps/server/src/project/RepositoryIdentityResolver.ts:

• A found root and identity now stay cached for 15 minutes (was 1 minute).
• A "not a repository" root now uses the 1-minute negative TTL (was no cache). Both caches now share one TTL rule.
• resolve is now untraced. Almost every call is a cache hit. The lookups that spawn git keep their own spans.

In apps/server/src/orchestration/ThreadPullRequestReactor.ts:

• PR discovery at the end of a turn now resolves the project identity with refresh: true. If the refresh returns null, it keeps the snapshot identity. Without this, an agent that adds or changes a remote during a turn (git remote add, gh repo create --push) could leave its PR unlinked for up to 15 minutes. Ported from #13722.

In apps/server/src/ws.ts:

• After a publish, the server resolves the identity with refresh: true, like clone already does. Publish can add origin to a repo that already has another remote, which changes the primary remote. Without the refresh, PR discovery could miss the new repo's PRs for up to 15 minutes.

Paths that need a fresh identity keep it: clone, publish, turn-end PR discovery, and PR discovery before it saves links all use refresh: true. git init needs no refresh, because the identity stays null until a remote exists.

Expected result: about 2 spawns per git project per 15 minutes instead of 2 per minute (about 17/min instead of 260/min at 130 projects), plus about 2 per finished turn. A non-git folder spawns at most once per minute.

Limits

• An identity can be up to 15 minutes old after someone changes a remote outside T3 Code and outside an agent turn (for example in a separate terminal).
• A failed root lookup (for example a git timeout) now counts as "not a repository" for up to 1 minute. Before, the next call retried it. The remote -v step already worked this way.
• Forgejo: if the fj/tea login check fails, or you log in to fj/tea after the identity is cached, the repo shows as an unknown host (no Forgejo PR discovery) for up to 15 minutes instead of 1.
• The publish refresh uses the publish cwd. If you publish from a worktree thread, the project root entry refreshes when its TTL ends or at the end of the next turn.

Verification

• vp test run apps/server/src/project/RepositoryIdentityResolver.test.ts. Two existing fake-git tests now use TestClock:
  • "refreshes the Git root only when requested" advances the clock 10 minutes before the second resolve and still expects no new git calls. It fails with the old 1-minute TTL.
  • "retries Git root discovery after the negative TTL" expects a failed root lookup to stay cached until 1 minute passes. It fails without the negative root cache.
• vp test run apps/server/src/orchestration/ThreadPullRequestReactor.test.ts (14 passed). New test "refreshes the project identity when a turn adds the remote" fails without the reactor change.
• vp lint and vp fmt on the changed files
• vp run --filter t3 typecheck
