Claude can enter plan mode on its own by calling EnterPlanMode, without the user picking plan mode in T3. Once it did, the thread stayed in plan mode: Claude kept refusing to edit, even across later prompts, and subagents it started inherited plan mode too. Only stopping the thread, which restarts the CLI process, got it out.

V1 avoided this by resetting the CLI's mode with setPermissionMode at the start of every turn. V2 sets the mode only when it opens the CLI process, then reuses that process for later turns, so nothing ever undid plan mode that Claude had entered. ExitPlanMode handling is unchanged: the plan is captured as a proposed plan and the tool is denied, as in V1.

Provider log from a real thread:

Fix

  • Track the mode the CLI reports on system init and status frames.
  • When a turn reuses the live process and the reported mode differs from the mode the process was opened with, call setPermissionMode to put it back before sending the prompt. A turn where the mode hasn't changed sends nothing extra.
  • Add setPermissionMode to the query session wrapper and the replay testkit.

Claude may still use plan mode for the rest of the turn in which it entered it, same as in V1.

Tests

  • The Claude plan test now emits a status frame with permissionMode: "plan" and asserts that the next prompt restores bypassPermissions. It fails without the fix.
  • ClaudeAdapterV2.test.ts and the orchestration-v2/testkit replay and integration suites pass (340 tests). The server typecheck is clean.

Done with Claude Opus 5.5 in Claude Code.

mode only when opening the CLI process and reused that process for later
turns, so after ExitPlanMode was captured (and denied, as in V1) the process
stayed in plan mode until it was replaced. V1 reset the mode on every turn.

Track the mode the CLI reports on init/status frames and, when a turn reuses
the live process and the mode has drifted, set it back to the mode the
process was opened with before sending the prompt.

Chat stays centered while the workspace card fits beside it with 32px to spare. When it does not fit, chat moves left only as far as needed, narrows only after it reaches the left padding, and the card becomes a popover below a 640px chat. The card is lighter: 280px wide, 32px rows, no section labels, no "Project folder" hint.
