Docs-only; no release-owned files, no plugin content.

Closes nothing (bookkeeping for #142/#143/#145/#148 already closed by their PRs).

• Added the 2026-10-21 closure record for all ten MedSci audit gaps.
• Recorded gaps #3, #5, #7, and #10 as closed.
• Kept gaps #1, #6, and #8 open with linked issues.
• Marked gap #9 as wontfix.
• Updated documentation only.
