Problem

Codex text generation (thread titles, branch names, commit messages, PR content) runs codex exec with the user's full ~/.codex/config.toml, so every MCP server the user configured starts for these one-shot calls. The model can then call those servers with the pasted prompt.

I hit this with the Augmenter MCP. I pasted a task into a new Claude thread whose text says "use the Augmenter MCP: link_thread with task_id …". The branch-name call (Codex) obeyed it and linked a phantom "Codex" thread to the task, with the summary Branch name: remove-default-character-shapes. Three seconds later T3 renamed the branch to exactly that name. The same thing can happen with any MCP server that has write tools.

Expected: metadata prompts need no tools, which is what Claude's text generation already does with --tools "" --strict-mcp-config.

Change

CodexTextGeneration lists the MCP servers Codex would start (codex --disable plugins --disable apps mcp list --json, with the same exec-safe launch args and CODEX_HOME). It then runs codex exec with:

  • --disable plugins --disable apps, since plugins and apps register MCP servers of their own;
  • --config mcp_servers={"name"={enabled=false},…} for each server still enabled.

Codex has no flag for "no MCP servers": -c mcp_servers={} merges into the user's table and turns nothing off, and dotted -c mcp_servers.<name>.enabled=false breaks for names containing .. So the servers are named in an inline table. If the list fails or times out (10s), generation goes ahead as it did before. --ignore-user-config would also drop the user's model provider, which T3's launch args and some users' configs rely on, so I didn't use it.

Scope and approval

A very small, focused fix of an obvious bug: Codex metadata generation should not start user tools, matching the Claude path (--strict-mcp-config). It touches only CodexTextGeneration.ts and its test.

Verification

  • vp test run src/textGeneration/CodexTextGeneration.test.ts: 27 pass. Two new tests: a title run disables plugins and apps and turns off each enabled server (including one with a . in its name), and a branch-name run still succeeds when codex mcp list fails. Both fail against main.
  • tsc --noEmit for apps/server and vp fmt --check pass.
  • With the real Codex CLI 0.160.1 on macOS, behind a wrapper logging argv, a generateThreadTitle call made one mcp list and then exec … --disable plugins --disable apps --config mcp_servers={"augmenter"={enabled=false},…}, and returned a title. The pasted prompt asked it to call the MCP; nothing reached the Augmenter server.
  • Checked by hand with codex exec: an Augmenter list_tasks call succeeds with the default config and reports no such tool with these flags.

Not checked: Windows, and the managed (openai_token_sharing) runtime; it builds the same command, so the same flags apply.

Model and harness: Claude Opus 5.5 in Claude Code (T3 Code).

Closes #18293

… MCP servers

Codex text generation (thread titles, branch names, commit messages and PR
content) ran `codex exec` with the user's whole config, so every MCP server
in ~/.codex/config.toml started and the model could call it with the pasted
prompt. Claude's text generation already runs with --strict-mcp-config.

List the servers Codex would start (`codex mcp list --json`), then run exec
with plugins and apps disabled and each listed server turned off by name.
`-c mcp_servers={}` merges rather than replaces, so the servers are named.
If Codex cannot list them, generation goes ahead as before.

Approvability

Verdict: Not approved

Macroscope's review found this PR not approvable — This production-path fix changes every Codex metadata-generation invocation by adding MCP discovery and overriding plugin/server configuration, with security-sensitive effects on which user tools can run. An unresolved high-severity finding identifies a way launch arguments can re-enable app/plugin tools, so human review is needed.

📝 Walkthrough

Codex text generation now lists enabled MCP servers before execution. It disables plugins and apps, and adds configuration overrides to disable listed servers. If listing fails, generation continues without MCP server overrides.

Changes: Codex text generation

Layer / File(s) Summary
MCP server discovery
apps/server/src/textGeneration/CodexTextGeneration.ts, apps/server/src/textGeneration/CodexTextGeneration.test.ts
The implementation lists MCP servers with a 10-second timeout and returns enabled server names when the output is valid. The fake CLI supports JSON output and configured exit codes.
Exec configuration
apps/server/src/textGeneration/CodexTextGeneration.ts, apps/server/src/textGeneration/CodexTextGeneration.test.ts
The exec command disables plugins and apps, and adds enabled=false overrides for enabled MCP servers. Tests cover listed servers, listing failure, and launch arguments that enable plugins and apps.

Priority: ➖ Normal

Estimated code review effort: 3 (Moderate) | ~20 minutes

Change: Bug fix · Severity of issue fixed: Medium

Suggested reviewers: juliusmarminge

Merge Risk: 🟡 Moderate · up to f475b

Title and branch-name prompts can still lead Codex to read project files through its built-in shell tool. A tool-free generation path should be established before merging.

Security architecture risk: 🔵 Low · up to f475b

The change is intended to reduce unintended extension access during background text generation. No new public access or privilege increase was found. Isolation remains conditional, however, and its effectiveness with the installed executable is not fully established.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — Residual exposure is bounded by the selected generation runtime and whatever authority its configured MCP servers possess. A write-capable server could affect resources outside the local checkout; actual credentials, downstream permissions, tenant scope, and affected assets are not mapped.

Security Findings and Attack Paths

  • inferred — User messages and repository-derived text reach generation through the existing operation interface. If discovery fails and a configured write-capable MCP server remains available, instructions in that text could lead to unintended tool actions. This conditional path predates the PR: the base applied no MCP-disable overrides. It is residual exposure, not an established introduced or worsened finding.

Trust Boundaries and Controls

  • observed — The new control appends plugin/app disable settings after user launch arguments and generates enabled=false entries for every enabled server returned by discovery. It changes process arguments rather than rewriting the user's persistent configuration.
  • inferred — Discovery and execution are separate processes without a visible immutable configuration snapshot or revalidation. A newly enabled server between them would not appear in the generated disable table. This limits the isolation guarantee but does not establish greater exposure than the base.

Resilience and Maintainability Implications

  • observed — Discovery state is local to each call. Discovery and exec use scoped effects, and generation ensures temporary-directory cleanup across completion and failure. Actual direct-child and MCP-descendant termination on timeout or cancellation is delegated to external process behavior and remains unverified.

Hardening Proposals

  • proposed — For a strict no-MCP guarantee, distinguish confirmed absence from failed discovery and decline generation when isolation cannot be established, while preserving required provider configuration.
  • proposed — Validate supported CLI versions against inert MCP fixtures, checking actual process startup, override parsing and precedence, configuration changes between discovery and exec, and timeout/cancellation cleanup rather than argument strings alone.

Explanation

Issue [#18293] requires Codex metadata generation to run without user MCP servers, tools, and plugins. The change disables plugins and apps and disables each enabled server when codex mcp list --json succeeds. The added tests cover enabled servers, dotted server names, and a listing failure. However, listEnabledMcpServers returns an empty list on nonzero exit, decode failure, timeout, or other failure. The subsequent codex exec then omits all MCP overrides, so the user's MCP servers remain enabled on that path. The exec arguments also contain no setting that disables Codex's built-in tools.

Resolution

Make metadata generation use a no-tools and no-MCP configuration even when server discovery fails or times out. Add automated coverage for the failure and timeout paths, including verification that the generated command cannot start configured user MCP servers.

191-193: 🧹 Nitpick | 🔵 Trivial | ⚡ Quick win

Surface MCP list failures in logs.

A nonzero exit code or a decode failure silently returns an empty list. In that case no MCP server is disabled. The user's MCP servers then start with the pasted prompt. This is the exact risk the PR addresses. The failure is invisible to operators.

Add a Effect.logWarning with bounded attributes, such as the exit code. Do not log stdout or stderr content. The guidelines require bounded log annotations.

As per coding guidelines: "Attributes and log annotations stay bounded: no raw payloads, command arguments or output".

…neration, and failed MCP listing is logged

`--disable plugins` loses to a `-c features.plugins=true` in the user's
launch args, so a plugin's MCP server could still start. Turn plugins and
apps off with `--config features.*=false` after the launch args, which wins.

Log a bounded warning (exit, decode or timeout; no output) when
`codex mcp list` fails and generation goes ahead with the servers on.

@coderabbitai Re your nitpick on CodexTextGeneration.ts 191-193 (surface MCP list failures): done in eb75bf2. When codex mcp list fails and generation goes ahead with the servers on, it now logs Effect.logWarning("codex mcp list failed; text generation keeps its MCP servers", { reason }). reason is exit (with exitCode), decode or timeout; no stdout, stderr or arguments are logged.

On the architecture note (fail closed when discovery fails): I kept fail-open on purpose. A broken or old codex that can't list its servers shouldn't stop branch names and titles, and failing open matches how text generation behaved before this PR. Maintainers may prefer otherwise; it's a one-line change.

@brycedbjork Thanks for the fix and the guidance. I verified that CodexTextGeneration.ts logs the exit, decode, and timeout cases without logging stdout, stderr, or launch arguments. That addresses the warning nitpick.

I accept the intentional fail-open behavior to preserve title and branch-name generation with older or broken Codex CLIs. I will not press for fail-closed behavior in this PR.

Learning: In apps/server/src/textGeneration/CodexTextGeneration.ts, MCP-server discovery intentionally fails open. If `codex mcp list` fails, title and branch-name generation continues without per-server disable overrides. This preserves compatibility with older or broken Codex CLIs and matches the behavior before PR #17058. Do not treat fail-closed behavior as a requirement without a maintainer decision.

aorwall added a commit to aorwall/t3code that referenced this pull request Oct 13, 2026
Merges upstream `pingdotgg/t3code` `5070a83a32` (48 commits, base
`e688034b5f`) into the fork. It does not stack on another PR.

## Merge
- 240 of 240 upstream-range files landed. Fork delta: 775 files.
- One conflict: `apps/web/src/routeTree.gen.ts`, regenerated for
upstream's new `/_chat/new` route. The other 18 files both sides touched
auto-merged: `duplicate-adds` and `resolution-check` are clean, and
every delta guard holds.
- `pnpm-lock.yaml` was re-derived by install, and `verify --only
lockfile` passes.
- Sweep: `promptLinks/HostedPromptLinkLanding.tsx` renders only in the
`hosted-static` auth state, which Moatless never reaches. Taken as
upstream's.
- Unsupported methods: ADD 0, DROP 0.

## Verification
`verify.mjs` passes duplicate-adds, tripwires, resolution-check,
unsupported-methods, lockfile, fmt, lint, typecheck and build. `test`
fails only on files that fail alone and failed the same way on the last
three merges, because of the sandbox: shared `AgentScope`
(`oom_score_adj` is 935 here), server `ServerBrowserPage` (no Playwright
browser), `AcpAdapterV2`, `OrchestratorReplayFixtures`, and
provider-opencode `OpenCodeServerLedger`.

## Usable as-is
- Prompt links (pingdotgg#18454): `/new?prompt=…`, `ThreadLinkPrompt` on thread
routes, and copy-prompt-link from messages and the command palette.
Choosing a project and placing the prompt work against Moatless.
- Terminal colors (pingdotgg#17168), context meter rounding (pingdotgg#17539), stable
button text (pingdotgg#18866), setup wizards follow themes (pingdotgg#18725), Git actions
follow the selected thread (pingdotgg#18862), dropped threads attach to pending
answers (pingdotgg#18789), the compact merge-button second click (pingdotgg#18878), a
numeric `q` on pull requests (pingdotgg#18666), the home-page bounce fix
(pingdotgg#18672), running terminals shown in the sidebar (pingdotgg#18740), and
computer-use titles (pingdotgg#18865).
- Mobile and desktop fixes (theme contrast, settings rows, the PR action
bar, image caching, the iOS 27.2 black screen, Hyprland, updater
diagnostics).
- "Update everything" sidebar pill (pingdotgg#18053): inert against Moatless,
which advertises no `serverSelfUpdate` or `versionAdvisory`.

## Unsupported in Moatless / needs implementation
- Prompt-link "clone this repository"
(`NewThreadLinkView.cloneRepository`): needs `projectClone.start` and
the `projectCloneTracking` capability. It refuses with a message today.
Added to gaps.md under *Project and repository management*.
- Renaming built-in models (pingdotgg#18840): saves through
`server.updateSettings` (custom model entry with a built-in slug). It
stays behind `FEATURES.providerConfiguration`, under the existing
settings gap.
- Provider secret settings (pingdotgg#18505, `secretSetting` and
`isSecretSetting` in contracts): stored on the host, so they fall under
the same settings gap.

## Backend behavior to consider reproducing in Moatless
Recorded in gaps.md under *Runtime fixes upstream made to its own
server*:
- A confirmed Claude subagent outcome settles that task and its child
work (pingdotgg#18778, the new `subagent` target on
`thread.background-work.settle`).
- Claude thinking before a tool call stays a collapsed Thought (pingdotgg#18887).
- Codex title and branch-name generation no longer starts the user's MCP
servers (pingdotgg#17058).
- The event store resyncs a thread holding an event type from a newer
build (pingdotgg#15937, `isKnownOrchestrationV2EventType`).
- A migration id clash stops startup with a clear error (pingdotgg#15925).
- Cached branch PR lookups start no git process (pingdotgg#17063).
- PR head commits are reported for Forgejo, GitLab and Bitbucket
(pingdotgg#18463, pingdotgg#18608, pingdotgg#18402), and PR bases resolve to branch names (pingdotgg#18754).
