fix(server): Codex works again after its app-server is killed - #26188
When the shared codex app-server process dies, T3 Code keeps its dead session. Every later Codex turn, new Codex thread, and delegated Codex task then fails at once with Codex App Server transport operation 'read-process-exit-status' failed. This continues until the T3 server restarts. "Restart agent session" does not help, because detaching a thread never releases a shared session.
This happened on a Linux host today. systemd-oomd killed the whole Codex agent scope (57 processes) during a short spike in memory pressure. Codex then stayed broken for more than an hour, while the host had plenty of free memory again.
Change
This takes over #22791 by @juliusmarminge and rebases it onto current main. His three commits are unchanged.
The Codex client exposes awaitTermination. The protocol's existing onTermination hook feeds it. It fires for a numeric exit, and also for a signal death, which arrives as read-process-exit-status.
When that happens, CodexAdapterV2 fails the session's event queue with ProviderAdapterEventStreamError. Events that are already in the queue are delivered first. The session manager's existing failure path then fails the running turns and releases the session. The next turn opens a new app-server.
The test now waits for the event reader fiber to end, instead of waiting one TestClock.adjust(0) step, as docs/internals/testing.md requires. Its comment now gives the correct reason for using a stream failure.
Scope and approval
Fixes#16502, a triaged bug. #22791 had green checks and two MERGE reviews, but it fell behind main.
Verification
Proof: on main's code, the new tests fail and a real Codex turn fails after an oomd-style scope kill. With this PR, the tests pass and Codex recovers on a new app-server.
New tests. The 4 tests in CodexAdapterV2.test.ts under "CodexAdapterV2 process exit" fail with main's adapter and client: the event stream never ends, so the test times out. With the fix, all 4 pass, in 3 of 3 runs.
Existing tests. All 7 server test files that use Codex replay transcripts pass (146/146). CodexAdapterV2, ProviderSessionManager and OrchestratorMcpToolkit.integration pass (153/153). effect-codex-app-server passes (28/28). vp run check:changed passes.
Runtime. Real Codex on a dev server on Linux, with agent scopes on. Both sides ran the same script. It runs one Codex turn, then runs systemctl --user kill --signal=SIGKILL on the dev server's Codex scope, which is what systemd-oomd does. It then sends a follow-up turn and starts a new Codex thread. This checks the systemd-oomd whole-scope kill, which fix(server): Codex recovers after its app-server process is killed #22791 did not check.

Review. A read-only Claude Opus 5.5 reviewer checked the rebased diff and found no blocking issues: MERGE, 8/10. It confirmed that normal shutdown and idle release do not turn into a false error, and that one death leads to one release and one new process for every thread. I fixed its two test notes (listed above). Two notes are left as they are:
No test covers that queued events are delivered before the failure.
One death shows the error on every thread attached to the shared session until that thread's next turn. This is existing session-manager behavior.
Council review did not run, because its GPT-6.1 Sol reviewer runs through Codex, and Codex on the review host was broken by this same bug.

…ask recording
The delegated_task_status recording ends with the recorder closing the
app-server while the parent turn is still open. The MCP integration test keeps
acting as that parent afterwards, so replaying the exit now (correctly) fails
the parent run and task_status reports parent_not_active. Drop the recorded
exit in that test's loader instead of changing what the app-server exit does.
Also assert that a dead process fails the event stream with
ProviderAdapterEventStreamError, since a clean end reads as manual_shutdown.

The process-exit test now waits for the reader fiber to end, as the
testing rules require, and its comment gives the real reason a stream
failure is used.

Macroscope's review found this PR approvable — Codex process termination now reaches existing session cleanup instead of leaving the event stream waiting indefinitely. The change is confined to failed transports and preserves buffered events before reporting failure.

The Codex app-server client now exposes transport termination. The Codex adapter forwards termination errors to its event stream. Tests cover numeric process exits, transport failures, and replay transcript filtering.
Changes: Codex app-server termination handling

Summary
Expose transport termination packages/effect-codex-app-server/src/client.ts
The client exposes awaitTermination. The effect fails with the error supplied when the transport terminates.
Forward termination to the event stream apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts, apps/server/src/mcp/OrchestratorMcpToolkit.integration.test.ts
The adapter forwards client termination failures to its event queue as provider event-stream errors. Tests cover numeric exits and transport failures with and without an active turn. The MCP integration test helper filters runtime_exit entries from decoded transcripts.

Explanation
Issue #16502 requires recovery after the shared Codex App Server dies by SIGKILL. The client now exposes awaitTermination and completes it from the protocol onTermination hook. CodexAdapterV2 fails the session event queue with ProviderAdapterEventStreamError, which lets the session manager retire the dead session. The new tests cover numeric and SIGKILL-like termination with active and inactive turns. The PR also reports that later turns and new threads start a new app-server.
Full details: Out of Scope Changes check
Explanation
The changes stay within issue #16502. The adapter and client changes implement process-death detection and session recovery. The new tests verify this behavior. The replay transcript adjustment removes a recorded process-exit event that would otherwise trigger the newly enforced failure path in an integration test. The schema-guard and event-reader changes support the test implementation and lint requirements.

Preserve active-turn synchronization while testing buffered events.
runtime.events reads directly from the queue, and Queue.fail delivers buffered items before the failure. The inactive cases currently consume turn.terminal before termination, so they do not test that behavior. Keep the turnStarted wait, interrupt the reader after it consumes the running event, enqueue the terminal event, and start a new reader after termination.
