docs(upstream): correct source commit IDs in review index

The upstream review index contains 15 full commit IDs that do not resolve in Git. Their ten-character prefixes and adjacent upstream PR titles identify the intended source commits, but the incorrect suffixes break exact provenance checks.

This corrects those IDs in four existing review rows. Each replacement was checked against the frozen upstream range, the commit title and upstream PR number, and the linked merged Pylon adoption PR (#680, #683/#684, #686/#687, or #690). No disposition or review cursor changes.

Validation: all full commit IDs in the corrected index resolve with git cat-file; the file equals the previous version after only these 15 replacements; git diff --check passes.

Implemented with GPT-6 Sol in Pylon.
