Problem

In a supervised Codex thread, choosing Always allow this session on a command approval did not stop the prompts. The next command asked again, and so did every command after it.

T3 sends Codex acceptForSession, but Codex only remembers that exact command (or patch) for the session. A review agent that runs cat a.md, then sed -n 1,40p b.md, then cat c.py asks three times, whatever the user picks.

Seen in a delegated Codex review task (runtimeMode: approval-required). From that thread's runtime requests: the user approved once, then picked "Always allow this session" twice (20:43:42 and 20:43:43), and Codex asked again 2 seconds later for the next cat. The thread then sat waiting.

Change

The Codex adapter now remembers which request kinds (command, file-change) the user allowed for the session, per native Codex thread. Later requests of that kind are answered with acceptForSession without asking the user. This matches what the button says and how Claude threads already behave.

  • Covers the v2 item/commandExecution and item/fileChange approvals and the legacy execCommandApproval / applyPatchApproval ones.
  • Allowing commands does not allow file changes, and the other way round.
  • A turn in a different runtime mode drops the allowance, so switching modes (including switching away and back) asks again. That gives users a way back to being asked.
  • It is cleared when the thread unloads, like Codex's own session approvals.

Scope and approval

Theo asked for this fix in DMs after seeing the stuck thread ("it shouldn't happen, clone repo and fix it and file pr"). It is one small adapter-level bug fix with no contract, client or UI changes.

Not included: switching a thread to Full access while a turn is running still doesn't affect that turn, because Codex fixes the approval policy at turn/start. That is a separate problem.

Verification

  • New replay test stops asking for commands the user allowed for the session in CodexAdapterV2.test.ts:
    • first command → user picks Always allow → second command is answered acceptForSession with no new runtime request
    • a file change in the same turn still asks
    • a Full access turn, then a Supervised turn again: the command asks again (the allowance does not come back)
  • Without the adapter change the test fails right away: expected 'command' to equal 'file-change' (the second command asked again). Without the turn-start reset it fails on the third turn (CodeRabbit's mode round trip).
  • vp test run src/orchestration-v2/Adapters/CodexAdapterV2.test.ts: 168/168 pass.
  • apps/server typecheck passes. vp lint and vp fmt --check on the two files are clean (two pre-existing unused-variable warnings elsewhere in the file).
  • Not checked against a live Codex app-server. The behaviour comes from the recorded thread above and the protocol schema.

… command

Codex's acceptForSession only skips the exact command or patch it
answered, so a supervised Codex thread kept asking for every new command
after the user chose "Always allow this session". The Codex adapter now
remembers the allowed request kind per native thread and answers later
requests of that kind itself, until the runtime mode changes or the
thread unloads.

Approvability

Verdict: Not approved

Macroscope's review found this PR not approvable — The production adapter changes when Codex command and file-change requests are automatically approved, including both modern and legacy approval protocols, after a session-level choice. Because this affects execution and file-modification authorization, the focused scope and replay test coverage do not remove the need for human review.

📝 Walkthrough

CodexAdapterV2 now remembers session approvals by native thread, approval kind, and runtime mode. Matching requests bypass approval prompts. The adapter clears remembered approvals when the mode changes or the thread unloads.

Merge Risk: 🟡 Moderate · up to 75144

A delayed approval can restore a session allowance after a runtime-mode change, allowing a later turn to skip a prompt expected under the new mode-switch behavior. Resolve this bounded but material authorization risk before merging.

Security architecture risk: 🟡 Moderate · up to 75144

Permissions remain separated by thread, request kind, and runtime mode. However, a delayed approval can restore permission after a mode change was intended to revoke it, potentially allowing later commands or file changes without another prompt.

Retained concerns

  • Medium · security · inferred: Mode invalidation is not fenced against pending approval completion. If a mode-A approval resolves after a mode-B start deletes the allowance, rememberSessionApproval recreates mode-A authorization. A later mode-A start leaves that matching entry intact, allowing subsequent requests of the approved kind without a fresh prompt. The adapter-level transition is supported by source; production scheduling and stale-response rejection remain incompletely established.
Security review details

Security Blast Radius

  • observed — The reusable permission covers later command callbacks or later file-change callbacks for the keyed native thread and matching mode. It is not restricted to the original command, patch, or path. A command allowance does not also grant the file-change callback kind; practical host, filesystem, network, and credential exposure depends on native execution semantics not established here.

Security Findings and Attack Paths

  • inferred — The relevant abuse sequence requires a qualifying user decision, a mode transition before that decision finishes, and later agent-generated requests in the original mode. A stale allowance can then remove the renewed approval checkpoint for those requests. This is not an established unauthenticated or cross-tenant attack path, and production scheduling remains a coverage gap.

Trust Boundaries and Controls

  • observed — The adapter now makes later authorization decisions on the user's behalf. Recording requires acceptForSession or acceptAlways; reuse requires the cache's native-thread key, matching runtime mode, and matching request kind. Current handlers also require a resolved turn context. These checks constrain reuse but do not verify that a pending decision still belongs to the current authorization lifecycle.

Hardening Proposals

  • proposed — Fence allowance writes with a native-thread authorization generation that changes on mode invalidation and unload. Capture it when requesting approval and require it to remain current before recording reusable permission. Exercise delayed completion, interruption, unload/reload, and overlapping request orderings to validate that revocation cannot be reversed by an older decision.

Switching a Codex thread to another runtime mode and back reused the old
session allowance. Each turn now drops the allowance when its runtime
mode differs from the one it was given in.

Reject stale session approvals after a runtime-mode change.

The mode-A approval handler can remain suspended while a mode-B turn is queued and starts. Mode B clears sessionApprovals, but the old mode-A callback later writes its allowance back. A subsequent mode-A turn can then return acceptForSession without a new user choice.

Track a per-thread mode-generation. Capture it when each approval request starts, increment it when the runtime mode changes, and ignore approval results whose generation is stale. Clear the generation on unload. Add a replay test for an approval that resolves after the mode switch.

Cover the legacy approval methods in the replay test.

The new test covers only item/commandExecution/requestApproval and item/fileChange/requestApproval. The adapter also supports the independent legacy execCommandApproval and applyPatchApproval requests. These handlers use different payloads and legacy response values while recording and reusing the same session allowances. No existing test or replay fixture exercises either legacy method. A regression in legacy session-allowance reuse can therefore pass without detection.

2790-2987: 🎯 Functional Correctness | 🔵 Trivial | ⚡ Quick win

Cover file-change session reuse in the replay test.

The added test answers its only file-change request with accept. It never records a file-change session allowance or checks the reuse branch. Add a second same-thread, same-mode file-change request after an acceptForSession response, and assert that no runtime request is emitted.

After "Always allow this session", the Codex adapter answers later command (or file change) approvals in that thread for the rest of the session. A turn in another runtime mode, or the thread unloading, ends it. Takes over #19598.
