fix(server): a provider switch mid-turn no longer leaves tool calls running - #17306

Conversation
…unning Switching a thread to another provider while a turn is running releases the old provider session, which fails the run's event stream before the provider can report the turn's end. The fallback that then fails the run closed open subagents but not the run's open tool calls, so a call such as the agent's own t3_thread_configure stayed "running" and the thread kept showing "Waiting on background tasks" until the server restarted. The run now tracks its open command and dynamic tool calls (and their nodes) and ends them as interrupted when its provider stream is lost.
ApprovabilityVerdict: Approved at Macroscope's review found this PR approvable — This is a focused server bug fix that closes orphaned tool calls only when a provider stream is lost, while preserving normal turn and background-work behavior. It introduces no schema, product-default, infrastructure, security, billing, or static-analysis changes and includes targeted regression coverage. You can add or adjust custom eligibility rules. Adapters report a tool call's node and its item as separate events, so a stream lost between them left an open node with no item. Interrupt every open tool node of the run, not only those paired with an open item.

A turn that fails to start is failed the same way as one whose provider stream is lost: no provider reports its end. Tool calls it reported before failing stayed running. Pass them to the final run write here too.

Problem
When a thread's provider session is released while a turn is running, the run's open tool calls stay
runninguntil the server restarts. This happens when an agent switches its own thread to another provider instance witht3_thread_configure, and with the detach thatt3_worktree_handoffperforms on purpose. The thread then shows "Waiting on background task" for work that no longer exists, and Stop doesn't clear it.Seen on Nightly 0.0.46 (build 2652): two
T3 Thread Configurecalls stayedrunningfor about 15 hours after their runs had failed with "The provider event stream closed unexpectedly". The shutdown reconcile cleared them when the app restarted.Change
Releasing the session fails the run's event stream before the provider reports the turn's end. The fallback in
RunExecutionServicethat then fails the run already closes the run's open subagents, but not its open tool calls. The run now tracks its open
command_executionanddynamic_toolitems and their tool nodes. Items are kept as the ingestor persists them, without unserved image bytes. When the provider stream is lost before the turn ends, those items end asinterrupted, notfailed: they were cut short, and a call such ast3_thread_configuremay already have done its work. This follows the Claude adapter's rule that only a turn ending on its own leaves its tools failed. The run itself still fails. This happens only in that fallback and in the matching one for a turn that fails to start; background work that outlives a turn that ended normally, and later runs that take over such work, are unchanged.Scope and approval
This is a small, focused fix for an obvious bug: one failure path, with no behaviour, contract or client changes.
It's related to #16000, whose triage traces the same detach and "provider event stream closed unexpectedly" path, and to #15300 and #15313 about self-configuration. It's independent of how those are resolved: the worktree-handoff detach is intended and still ends the calling turn this way.
Verification
Live, in a dev build with a real Claude session that calls
t3_thread_configureto switch itself to Codex. Onmainthe call staysrunningand the thread shows "Waiting on background task mcp__t3-code__t3_thread_configure". With this change it shows "Stopped setting thread model", nothing is left waiting, and the projection has no open tool calls. The run fails in both, as before ([Bug]: Self model configuration can stop its own run without continuing #15300).main)New
ProviderSwitch.integration.test.tscase: a provider with Claude's capabilities holds a turn with a runningdynamic_toolcall whileprovider.switchmoves the thread to Codex, through the real orchestrator, effect worker and session manager. Onmainthe item staysrunning("expected 'running' to equal 'interrupted'"). With this change it ends asinterrupted, andderivePendingBackgroundWorkis empty. The test adapter gains one option, heldRunningToolCall.New
RunExecutionService.test.tscase: after the stream is lost, open tool calls and their tool nodes end as interrupted, a node whose item never arrived is interrupted too, a call that already completed is left alone, and the stored item has no unserved image bytes. A tool call reported before the turn fails to start is interrupted the same way.Related suites:
