Propagate async MediaRecorder errors to the session, clear the lifted busy flag on unmount, release mic tracks when stop() rejects, abort the underlying transcribe upload on cancel, settle the recording handoff so cancel cannot wedge finishing, pin ownerKey to the pending-question target, and forward Safari mp4 bytes with matching MIME and filename.
Fetch follows redirects by default; a cross-origin redirect can strip Authorization while preserving the custom ChatGPT-Account-Id header. Request manual redirect handling so a 3xx from the transcribe endpoint surfaces as a plain transcription failure instead of re-sending credentials to a second origin.
Schema.TaggedErrorClass became Schema.TaggedError upstream; migrate VoiceProviderUnsupportedError. Re-export resolveTranscriptCommit from the voice-input barrel, which upstream stopped re-exporting but the web composer session still consumes.
…n, declared-length gate Include the pending request id in the voice draft ownerKey so a late transcript cannot land on a newer request that reuses the question index, id, and draft text. Construct ComposerVoiceSession in the mount effect instead of during render so StrictMode effect replay leaves a usable session. Reject oversize or malformed Content-Length declarations before credential reads and upstream transcription work.

What Changed
Add a Codex-backed live dictation prototype to the web composer, shared by web and desktop. Users can dictate a coding request, edit the draft or move the caret, and continue speaking at the new position. Stop returns control without waiting for optional AI polish; newer edits prevent a stale polish result from replacing the draft.
The selected environment and Codex provider instance handle the request. Codex CLI app-server owns account handling and realtime negotiation; microphone audio uses WebRTC. This replaces the original private transcription endpoint/auth.json approach. This is a prototype for scope discussion, following #12212. There is no maintainer approval yet. I understand the scope decision on #11278 and the preference for small contributions. This builds on Mina Sayed's work in that PR and preserves its commit history.
The full prototype remains larger than a small contribution, including shared controller integration and tests. If the direction is welcome, the proposed first contribution can be reduced to microphone start/stop, caret-aware insertion, and environment/account routing, with the extras deferred. The local Run App action and all evidence files are excluded from the code diff.
Review concerns addressed
Route requests through the selected environment and enabled Codex instance. Regression tests cover missing/disabled accounts, the selected CLI/home/environment, and cleanup pinned to the originating environment.
Let Codex CLI app-server own authentication and realtime negotiation; the voice backend does not read auth.json or call the original private transcription HTTP endpoint with Desktop identity headers. Realtime CLI support is still experimental.
Use the shared VoiceInputController for streaming lifecycle, cancellation, draft ownership, and insertion. The web session is now a browser microphone/transport adapter. Native file transcription retains its existing path and tests; both share the active-recording gate.
Hide dictation controls for unsupported providers instead of displaying a permanent disabled microphone or promising future support.
Add macOS microphone usage descriptions to development and release bundles, plus audio-input entitlement in custom signed bundles. Actual native permission behavior still needs platform verification.
Address the six initial CodeRabbit findings: preserve bad-request errors for disabled instances, retain running-turn interrupt during dictation, allow editable custom question answers, ignore stale waveform resume errors, tolerate brief WebRTC disconnects, and clarify optional hesitation cleanup.
Availability now waits for successful CLI/account preparation, with bounded checks and retry after failures. Partial dictation settings patches preserve unrelated preferences.
Disable inherited MCP servers, apps, and plugins for voice sessions and verify the thread MCP inventory before submitting speech or polish text. A synthetic configuration probe against Codex CLI 0.154.0 confirmed the inherited test server was disabled with an empty tool inventory.
Reject stale replacement edits after another client changes the entry, keep the microphone muted when stop races startup, reject whitespace-only replacement values, and preserve punctuation words inside identifiers.
