Summary

sync the fork to upstream v0.0.21 from upstream/main@ada410bc
reapply the retained fork capsules on a fresh upstream base
switch the repo to the VPS-standard dev -> main -> production release flow
add integrated and hosted smoke coverage for the rebuilt fork runtime
fix the phase 4 web-push runtime regression caused by eager SqlClient resolution

Notes

t3-dev.claude.do is now running this dev release candidate from /srv/t3code/dev
