## Before you start

**This issue is public the moment you submit it.** Anyone on the internet can read it, including earlier versions if you edit it later. Share only what you are happy to see on the lab's public member page.

- **Never include** phone numbers, student or employee ID numbers, home addresses, your date of birth or anything else private.
- **Under 18? Do not use this form.** High school interns and anyone under 18 are never listed by name, so there is nothing to submit.
