Simplify Task Details with notes-first layout and tabbed reasoning

Task Details mixed everyday editing with lengthy assessment history and conversation controls. This change keeps notes and the ranking summary prominent while making deeper reasoning available on demand.

• Move task notes above Why this order, with keyboard-accessible Overview, Assessment, and History tabs in a bounded panel. Overview shows impact, visibility, effort, and work-style pills; Assessment retains importance, urgency, blockers, expandable rating rationales, and style corrections. Browsing history never replaces the latest judgments.
• Collapse private thread notes, code-agent controls, and conversation notifications into disclosures, while keeping active code work and errors discoverable.
• Remove the conversation reader, Load conversation, and cache-discard controls from Task Details without deleting saved conversations or notes. Keep Open source available and avoid conversation reads or fetches on task selection.

Update product/design documentation and regression coverage for keyboard navigation, narrow layouts, history isolation, read failures, and legacy assessments.
