ApprovabilityVerdict: Not approved Macroscope's review found this PR not approvable — This narrowly scoped server fix changes how external favicon files are opened and streamed to prevent a path swap from serving unintended or sensitive data. Because it affects security-sensitive file serving, it warrants human review despite the focused implementation and regression test. You can add or adjust custom eligibility rules. Learn more. |

Addressed the serve-time path-swap race in 724c90790. Signed exact-file assets now stream from an already-open descriptor after no-follow and file-identity checks; the regression test replaces the path with a symlink and confirms the same URL returns 404. @coderabbitai review |

CodeRabbit’s issuance-time inode suggestion would change the intended path capability into a snapshot capability. The signed URL authorizes one canonical image path for its short lifetime, so atomically regenerated images at that path should continue to load. Inode binding would break that common write pattern and would not prevent in-place writes by a process that already controls the authorized file. The serve-time fix instead guarantees that the bytes come from the exact regular file opened at the signed canonical path, while rejecting symlink substitution. |

Effect service conventions review: one finding in apps/server/src/assets/VerifiedFileResponse.ts (catch-all promise pipeline erases typed failures). Everything else in scope (new AssetEnvironmentImage* tagged errors, AssetAccess claim handling, ws.ts routing, client-side renames) follows the conventions.

Addressed the Effect failure-boundary finding in 3101af203. Open, stat, realpath, stream creation, and close failures are now typed with file/stage/cause context and logged at the HTTP edge; only a successful identity check mismatch maps directly to 404. @coderabbitai review |

Effect service conventions review: one finding in apps/server/src/assets/VerifiedFileResponse.ts. The typed-error rework since the last run addresses the previously flagged bare-catch/erased-diagnostic issue.

Addressed the remaining actionable stream findings in 93f4b3255: POSIX opens are non-blocking so FIFO substitution cannot occupy the libuv pool, the stream is bounded to the opened size and no longer publishes a stale Content-Length, and the pure error-construction alias is gone. The inode recommendation is intentionally not applied because this is a short-lived canonical-path capability and must continue to support atomically regenerated images. @coderabbitai review |

The symlink-substitution regression test remains in place. The canonical-path capability model is consistent with support for atomically regenerated images. Verified feat(markdown): load images outside project roots at 93f4b325 as CDVolvik. Diff: environment-image asset type replaces workspace-file, mints canonical-path-bound signed URL, serves via verifiedFileResponse with O_NOFOLLOW + realpath/dev/ino checks, filename-bound resolution blocks sibling/tamper, thread-gated minting. Tests: markdownImages.test.ts 31 passed, ChatMarkdown.environment-images.test.tsx 24 passed, ChatMarkdown.test.tsx 30 passed (85 combined). AssetAccess new env-image cases 3/3 passed (exact capability, requires absolute image, rejects symlink to non-image). Full AssetAccess 14/16 passed; 2 Windows path sep failures reproduce on main, not introduced here. server.test.ts image signed URL requires Node >=22.16 (local 22.14) — CI reports pass. No push, branch restored. Verify — no blockers. |

Reopening: #9023 covers the main image-preview feature, but does not include this PR's descriptor-backed external favicon serving or its same-URL support for atomically regenerated images. My initial closure missed that remaining work; this PR is not fully superseded. |

Rebuilt on current main as just the favicon half (about 20 source lines). I dropped the same-URL-after-atomic-rename part: #9023 and environment-auth.md deliberately invalidate URLs on replacement, and the one-hour image cache plus the Retry control already cover regenerated images in practice. |

An external project favicon was checked by path during resolution, then reopened by path when the response was written, so a path swapped in between changed what was served. Open it with the existing openMediaFile guard during resolution and stream that descriptor, as host media does. Co-Authored-By: Claude Opus 5.5 (1M context) <noreply@anthropic.com>

When a project's icon is an image outside the project folder, the server checks that file and then opens it again by name to send it. If the file is replaced between those two steps, the browser gets whatever the path points to now. The icon URL is signed for the file the server checked, so it shouldn't be able to return a different file. In the focused test below, it returns the contents of an unrelated private file.
Fix:
resolveAssetopens the external icon once, during the check, using the existingopenMediaFileguard (no-follow, identity check). The response then streams from that open file, the same way #9023 serves host media. The file is scoped to the request and closes whether or not the response is consumed..icnsicons from #17149 are unchanged: they're already read into memory during the check and served as PNG.How it was established and checked
