Problem

The three-restart continuation cap counts the entire ancestry. A thread can work normally between upgrades and eventually stop resuming solely because it has restarted three times over its lifetime.

Change

Count only restart continuations requested within five minutes of the interrupted source run. Rapid loops still stop at the existing three-link cap; old successful work no longer permanently exhausts recovery.

Scope and approval

This is a focused correction to the established restart-loop guard. It does not add recovery mechanisms or replace shared-session startup ordering handled by #15820.

Verification

Run vp test run apps/server/src/orchestration-v2/RestartContinuation.test.ts -t "stops rapid restart loops". The existing cap regression now also ages one ancestor beyond the window. The rapid chain stays blocked in both versions, while the aged chain fails to resume on upstream and resumes after this change.

The requester stopped the remaining heavy checks. Completed focused results are recorded in the attached log; no repo-wide check was run.

The existing rapid-loop regression now covers missing completion timestamps at and beyond the request-time cutoff, alongside normal-work spacing.

Review verification at b7600d7f10. No heavy checks were run.

Reproduction and focused verification log

Prepared with GPT-6.1-Sol through the Codex harness.

📝 Walkthrough

Automatic restart continuation now counts only restarts within five minutes of the source run’s completion time, or its request time when completion is absent. Tests cover rapid restart limits and continuation after older ancestors fall outside the window.

The chain counter stops at ancestors requested before the five-minute window. Tests check request-age cutoffs and continuation when runs have completion timestamps set to the current time.

Merge Risk: ⚪ Minimal · up to b7600

The change preserves the rapid-restart cap while allowing continuation after older restarts age out. The request-time fallback is covered; no actionable merge risk is established.

Before / after behavior

  • observed — Modified behavior in apps/server/src/orchestration-v2/RestartContinuation.ts: The three-restart chain limit remains, and a five-minute loop window is added; the limit now applies to recent restarts rather than the chain’s lifetime.
  • observed — Modified behavior in apps/server/src/orchestration-v2/RestartContinuation.ts: The chain-counting loop now establishes a five-minute window from the source run’s completion time, or request time if completion is absent. It stops traversing when an ancestor was requested before that window, so restarts outside the window do not contribute to the chain limit.
  • observed — Modified behavior in apps/server/src/orchestration-v2/RestartContinuation.test.ts: The base run fixture now sets requestedAt to now and completedAt to null.
  • observed — Modified behavior in apps/server/src/orchestration-v2/RestartContinuation.test.ts: The cut-run fixture now sets completedAt to now.

1017-1024: 🎯 Functional Correctness | 🔵 Trivial | ⚡ Quick win

Exercise the request-time fallback in the rapid-loop test.

cutMidTurn sets completedAt: now, so the current rapid-loop assertions never use source.requestedAt as the window anchor. A regression that falls back to now instead of source.requestedAt when completedAt is null would pass.

Add a null-completion case at the cutoff and move the continuation case beyond the cutoff.

Update the rapid-loop test around `continuationTexts` to cover the request-time
cutoff when run completion times are null: assert that requests at the cutoff
produce no continuation, then move the continuation case beyond the cutoff and
assert that it produces one. Preserve the existing `afterNormalWork` behavior
for requests outside the cutoff.
