Problem

Forking a thread fails on a Claude instance with a custom homePath. The fork run fails before it starts (startedAt: null) with "Provider turn failed to start / Claude Agent SDK query failed", and the provider log shows session.fork retried 5 times with no session.forked. The underlying error is Session <id> not found in project directory for <cwd>.

ClaudeAgentSdkQueryRunner.forkSession calls the SDK's forkSession inside the server process. The SDK reads session files from process.env.CLAUDE_CONFIG_DIR, which is the server's environment, not the instance's. An instance with its own home stores sessions elsewhere, so the lookup never finds the source session. Instances on the default ~/.claude home are unaffected, which is why this only shows up with a custom homePath.

Change

When the instance's resolved config dir differs from the server's, the fork runs in the existing Claude history worker (claudeHistoryWorker, already built for the npm bundle and as the __claude-history subcommand of the single executable) under the instance's environment. The server's own environment is never modified. Instances that share the server's config dir keep the in-process call, so the common case does not spawn a process.

  • ClaudeAgentSdkSessionForkInput gains environment, passed from adapterOptions.environment in forkThread.
  • The config dirs are compared after resolving the default (~/.claude), because makeClaudeEnvironment always sets CLAUDE_CONFIG_DIR, even for the default home.
  • layerQueryRunner now also requires ChildProcessSpawner and Path; both are already provided by the production runtime.

Scope and approval

Fixes #18260, which a maintainer triaged and confirmed on main. This is a focused replacement for the fork part of #15040 (a larger bundle that currently has conflicts) and for #15598, which was closed in a PR cleanup after a "Would Approve" review. The packaging concern raised on #15598 is addressed below.

Verification

  • New regression test in ClaudeAgentSdkQueryRunner.test.ts writes a real session file under a temporary CLAUDE_CONFIG_DIR and forks it through the runner. It passes with the change. With the old always-in-process behavior forced back on, it fails with the same Session ... not found error as in the report.
  • A second test checks that an instance sharing the server's config dir still calls the SDK in-process.
  • Updated the exact-input assertion in ClaudeAdapterV2.test.ts for the new environment field.
  • pnpm run typecheck in apps/server exits 0, and vp fmt --check is clean. vp test run src/orchestration-v2 src/provider: 2752 passed, 1 failed. The failure is OrchestratorReplayFixtures ... claude_result_is_error, which fails the same way on unmodified main on my machine (authentication message text).
  • Packaging: vp pack emits dist/claude-history-worker.mjs next to the chunk that contains the adapter, which is where import.meta.url resolves. I ran the built worker (node dist/claude-history-worker.mjs forkSession <id> '{}') with CLAUDE_CONFIG_DIR pointing at a temporary dir containing a session, and it returned a new session id and wrote the forked file.
  • Not checked: the single-executable build (__claude-history path) and the desktop app (Electron, ELECTRON_RUN_AS_NODE). Both go through the same worker entry, but I did not run them.
  • Seen live on a nightly (0.0.46-nightly.20261012.3010) with a homePath: ~/.claude_pool instance: every fork of a pool-stored session failed; forks of sessions stored in ~/.claude succeeded on the first attempt. I have not run this patched build in that environment.

Model and harness: Claude Sonnet 5.5 (claude-sonnet-5-5), run through Claude Code in T3 Code.

🤖 Generated with Claude Code

…n home

The SDK's forkSession reads session files from process.env.CLAUDE_CONFIG_DIR,
which is the server's environment. An instance with a custom homePath stores
its sessions elsewhere, so every fork failed with "Session ... not found in
project directory" and the thread showed "Claude Agent SDK query failed".

Fork now runs in the existing Claude history worker, under the instance's
environment, when the instance's config dir differs from the server's. Instances
on the server's own config dir keep the in-process path.

Approvability

Verdict: Not approved

Macroscope's review found this PR not approvable — This is a targeted fix for Claude sessions stored under a custom home, with tests covering the worker path and preserving the in-process default path. The implementation nevertheless adds environment-sensitive subprocess execution and packaging-specific launch behavior that merits human validation, particularly for executable and Electron deployments.

📝 Walkthrough

Walkthrough

Claude session forks now compare the instance and host Claude config directories. Matching directories use the SDK directly. Different directories use a worker process with the instance environment. Tests cover both paths.

The query runner uses the SDK directly when the instance and host config directories match. Otherwise, it starts a worker with the instance environment and a 30-second timeout. Tests cover both paths and verify the fork options include the environment.

Forking a Claude session under a custom home can fail on a supported Node version. Make the source worker runnable there before merging.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 7c6a5

The change keeps fork execution tied to the selected instance without modifying the shared environment. No new security defect was established, but interrupted-fork recovery remains unverified.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The changed execution route is limited to forks whose resolved configuration directory differs from the host’s. The worker uses the host executable and receives the instance environment, including inherited values. Configuration separation is not an operating-system sandbox or a reduction in host credential authority.

Trust Boundaries and Controls

  • observed — The production caller still delegates native-versus-portable fork selection to the existing command policy using source/target instance equality and strong-native-reference flags. The adapter still rejects forks during active turns. Worker execution uses a fixed operation and explicit argument vector, not a constructed shell command.

Resilience and Maintainability Implications

  • inferred — The new process boundary adds a possible acknowledgement-loss window between SDK completion and receipt of the session identity. If the SDK persists before acknowledgement, interruption or timeout could leave an untracked session copy. SDK persistence and cleanup semantics were unavailable, so this remains a recovery uncertainty rather than an established security finding.

Hardening Proposals

  • proposed — Validate cancellation, concurrent repetition, and failure after persistence against the actual SDK and process runtime. If completed forks can lose acknowledgement and leave sensitive session copies, use SDK-supported idempotency or reconciliation rather than treating timeout as storage rollback.

official Node.js documentation TypeScript type stripping Node 22.16 22.18 --experimental-strip-types

💡 Result:

Official versioned docs:

- [Node.js v22.16.0 — TypeScript](https://nodejs.org/download/release/v22.16.0/docs/api/typescript.html): `--experimental-strip-types` enables running TypeScript files. It strips inline types without type-checking; syntax that requires JavaScript transformation needs `--experimental-transform-types`. ([nodejs.org](https://nodejs.org/download/release/v22.16.0/docs/api/typescript.html?utm_source=openai))
- [Node.js v22.16.0 — CLI](https://nodejs.org/download/release/v22.16.0/docs/api/cli.html): documents the flag as experimental, added in v22.6.0. ([nodejs.org](https://nodejs.org/download/release/v22.17.0/docs/api/cli.html?utm_source=openai))
- [Node.js v22.18.0 — TypeScript](https://nodejs.org/download/release/v22.18.0/docs/api/typescript.html) and [CLI](https://nodejs.org/download/release/v22.18.0/docs/api/cli.html): type stripping is enabled by default in v22.18.0; use `--no-experimental-strip-types` to disable it. ([nodejs.org](https://nodejs.org/uk/blog/release/v22.18.0?utm_source=openai))

**Bottom line:** use `node --experimental-strip-types file.ts` for v22.16.0. In v22.18.0, `node file.ts` enables stripping by default; the positive flag is no longer needed.

Make the source worker runnable on the minimum supported Node version.

The test runner invokes the source worker with Node, but the invocation does not pass --experimental-strip-types. Node 22.16 supports this package and requires that flag for .ts files. Removing CLAUDE_CONFIG_DIR does not change the child invocation; the test already overrides it with configDir.
