Problem

The server never collects SQLite planner statistics. layerSetup in apps/server/src/persistence/Sqlite.ts sets four PRAGMAs and runs migrations, and nothing in the repo runs ANALYZE or PRAGMA optimize, so sqlite_stat1 never exists and the query planner falls back to its default selectivity guesses, however large the database gets. #17955 reports multi-minute recovery reads on a 13.7 GB statev2.sqlite that went away after a manual ANALYZE.

Change

Run PRAGMA optimize = 0x10012 once after migrations, which is SQLite's recommended open-time pattern for long-lived connections:

  • 0x10000 checks every table, not just ones this connection has queried.
  • 0x2 runs ANALYZE on tables whose indexes have no sqlite_stat1 rows or whose row count changed about 10x since the last analysis. This also covers indexes added by migrations.
  • 0x10 applies a temporary analysis_limit.

The issue suggests 0x10002. That mask leaves out 0x10, so its ANALYZE is unbounded. On a 4.9 GB snapshot it took twice as long on first open (table below). Once statistics exist, later opens only check table sizes. The issue also suggests periodic and shutdown optimize. I left those out to keep this to the missing-statistics bug. The size check still refreshes statistics on the next restart after a table grows 10x.

Scope and approval

Fixes #17955 (not yet triaged). This qualifies as a very small, focused fix for an obvious bug: one PRAGMA in the shared persistence setup, plus one test. The code never creates planner statistics, which you can confirm by reading Sqlite.ts or by checking any long-lived install. The developer's own 5 GB statev2.sqlite has no sqlite_stat1 table. Nothing else changes.

Verification

Problem on main. I checked a read-only VACUUM INTO snapshot of a real, long-lived 4.9 GB statev2.sqlite (916k V1 events, 128k V2 turn items) and opened it through the main Sqlite.layerFromPath:

stat1 in live database: 0 sqlite_stat* tables
main layer: layer ready in 20 ms; sqlite_stat1 rows: missing
main layer: layer ready in 13 ms; sqlite_stat1 rows: missing (second open, still missing)

Fix, same snapshot (fresh copy each mask, warm page cache):

== 0x10012 first start / restart / restart
layer ready in 835 ms; sqlite_stat1 rows: 126
layer ready in 13 ms; sqlite_stat1 rows: 126
layer ready in 13 ms; sqlite_stat1 rows: 126
== 0x10002 first start / restart / restart
layer ready in 1795 ms; sqlite_stat1 rows: 126
layer ready in 13 ms; sqlite_stat1 rows: 126
layer ready in 13 ms; sqlite_stat1 rows: 126

Startup cost at the reporter's scale. I cloned every V2 projection row 10 times, which gave a 16.9 GB fixture with 1.4M turn items and 940k nodes. I then opened it through the fixed layer. Cold numbers come from evicting the file with posix_fadvise(DONTNEED) first.

first open of a never-analyzed DB time
warm page cache ~450 ms
cold page cache 31–33 s (one time)
any later open, cold 21 ms

The cold first open is dominated by the large turn_items and nodes indexes. Even under analysis_limit, SQLite walks each index once to count its rows, at 1.5–5.5 s per index cold. That cost is paid once per database, before the server accepts connections. The alternative is the unbounded stalls in the issue.

Focused test. apps/server/src/persistence/Sqlite.test.ts adds "collects planner statistics for an existing database on open". It seeds an indexed table, reopens the file through layerFromPath, and asserts that the index's sqlite_stat1 row exists.

main: Error: no such table: sqlite_stat1 (fails)
fix: Tests 4 passed (4)

I also ran vp lint and vp fmt --check on both files, plus vpr typecheck in apps/server. All clean.

What I could not reproduce. I ran every getRuntimeRecoveryProjection statement, extracted verbatim from ProjectionStore.ts, with EXPLAIN QUERY PLAN and timing for the 20 largest threads. On both fixtures, before and after statistics, the planner already chose the indexed plans the issue lists (runs_recovery_idx, nodes_live_idx, turn_items_recovery_idx). The 20 threads' reads took ~78 ms on the snapshot and ~270 ms on the 16.9 GB fixture, with no measurable difference after statistics. That matches the reporter's note that the same statements used indexes from a separate connection. So I could not reproduce the multi-minute plans themselves. This fix guarantees the statistics exist, which removes the planner's guesswork on large databases. I could not show a specific recovery read that regresses without them on this data. I also did not test on Windows or on the reporter's database.

Model and harness: Claude Opus 5.5 (claude-opus-5-5) via Claude Code in T3 Code; reviewed by GPT-6.1 Sol via Codex.

The server never ran ANALYZE or PRAGMA optimize, so sqlite_stat1 never
existed and the query planner had to guess on every database, however large.
Run PRAGMA optimize=0x10012 after migrations so tables with unanalyzed indexes
or 10x row growth get bounded, approximate statistics on open.

Approvability

Verdict: Not approved

Macroscope's review found this PR not approvable — This focused fix changes every server database open to synchronously run SQLite optimization and persist planner statistics. Large databases may incur substantial startup latency and the default runtime behavior changes for all deployments, so human review is warranted.

Merge Risk: 🔵 Low · up to 2a456

Adds planner statistics at database open. A very large database may see a one-time slower first startup, which could delay health checks. Measure this on a representative database; the risk is bounded and unlikely to block merge.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 2a456

The change is confined to the existing database and does not add external inputs, permissions, or public interfaces. Its main uncertainty is startup behavior if optimization encounters contention or failure; interrupted and concurrent recovery behavior has not been established.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The changed operation is confined to planner maintenance within each database opened by the existing persistence setup. Production server and authentication consumers already used this persistence layer; the PR adds no new route to its database authority.

Trust Boundaries and Controls

  • observed — The operation retains the existing database-path and client ownership boundary. Native extension loading remains disabled by default in the unchanged driver; optimization does not introduce a new capability or authority source.

Resilience and Maintainability Implications

  • inferred — An optimization execution error follows the existing setup-failure path rather than being silently ignored. This preserves failure visibility and connection cleanup, while adding a startup operation whose contention and recovery behavior remains unverified.

Explanation

The change satisfies the open-time requirement in #17955. layerSetup runs PRAGMA optimize = 0x10012 after migrations, and the new test verifies sqlite_stat1 creation for an existing database. The mask also performs a bounded analysis pass. However, #17955 requires a later optimize so statistics remain current as the database grows. The diff contains no periodic or shutdown optimize call. A long-lived server can therefore keep stale statistics until restart.

On CodeRabbit's linked-issue warning (no periodic or shutdown PRAGMA optimize): I left that out on purpose to keep this to the bug, which is that statistics never exist. Statistics still get refreshed. The 0x10000 bit makes every open re-check table sizes and re-analyze any table that grew or shrank ~10x, so a long-lived server picks up stale statistics on its next restart. A timer would add scheduling machinery and another synchronous ANALYZE on the live connection. I'd rather add it only if stale statistics between restarts turn out to matter in practice.

I ran this PR's exact pragma on a copy of a real 15.2 GB statev2.sqlite. The first run took 80.5 s with a cold file cache and 3 ms when repeated. It also gave the turn-start read of a large thread's turn items a new plan, which cut it from 17.5 ms to 0.19 ms. The description says it could not show a read that regresses without statistics. This is one, though a turn-start read rather than the recovery reads in the issue. I found no query that got slower.

Setup

Linux x86_64, Node 26.11.1 and its bundled SQLite 3.53.4. The database is my own long-lived install: 2,554,315 events, 333,233 turn items, 75,386 messages, 3,719,170 pages. I copied it with backup() from a read-only connection, switched the copy to WAL, and dropped its pages from the file cache with dd iflag=nocache before the first run. I did not run the server, only PRAGMA optimize = 0x10012 on a fresh connection.

First run on a real database

Step Result
PRAGMA optimize = 0x10012, cold file cache 80.5 s, 128 rows in sqlite_stat1
Same pragma again, statistics present 0.003 s
Second process trying BEGIN IMMEDIATE every 50 ms during the first run failed for all but 4 of 1,602 attempts in the window, from 42 ms after the start until 21 ms before the end
Reads (SELECT) during the same run 1,602 of 1,602 succeeded, longest 3 ms

This is one cold run, so no run-to-run spread. It is 2.5 times the 31–33 s cold figure in the description for the cloned 16.9 GB fixture. I do not know why. The description says this cost lands before the server accepts connections. I did not check that, and the write failures above only matter if anything else writes to the file while it runs.

What changed in other queries

I extracted the 272 distinct SQL statements in apps/server/src at b9ad3daf8d (no tests or migrations) and compared EXPLAIN QUERY PLAN before and after. 183 produced a plan, 50 gave no plan rows, and 39 could not be prepared because of ${...} fragments. 15 of the 183 plans changed. I timed the ones that could matter, with 5 fresh processes per arm, alternating, on the real thread ids:

Statement No statistics With statistics Reading
getTurnStartContext, turn items of one run, thread with 17,517 items (ProjectionStore.ts:3920) 17.5 ms [16.8–18.1] 0.19 ms [0.18–0.23] 98.9% less (about 90 times faster). The ranges are 75 times apart. The fast arm's spread was 0.0507 ms against my 0.05 ms floor, so by my own rule the cell is inconclusive on variation alone.
Same statement, the run with the most items (1,847 rows) 16.7 ms 13.1 ms no clear change
Highest event sequence of a thread (ProjectionStore.ts:4783, 4946) 0.003 ms 0.003 ms no clear change
Fork boundary (ProjectionStore.ts:3366) 0.005 ms 0.005 ms no clear change
Provider-thread lookup (ProjectionStore.ts:3065) 24.9 ms 24.1 ms no clear change

The first row is in the same function as the 10–13 ms node query I measured earlier in discussion #17939, which also drops to 0.0075 ms with statistics. Both are turn-start reads of 10–18 ms, not the multi-minute plans in the issue. I could not reproduce those either.

Limits

  • Plans were extracted statically, so dynamic SQL is not covered. Plans are compared as text.
  • One database and one first-run measurement. Windows is untested.
  • I did not run 0x10002 for comparison, so the description's 2x figure is neither confirmed nor contradicted.

A second data point from Windows, on a smaller database than the ones in #17955. We run a fork with long threads and adopted this change today.

Measured on a copy (SQLite backup API) of the fork's live statev2.sqlite, Windows 11, Node 24.13 with its bundled SQLite 3.50.4:

Database 1.75 GB, 262,253 rows in orchestration_events
sqlite_stat1 before did not exist
PRAGMA optimize = 0x10012, first run 147 ms, 110 rows in sqlite_stat1
same pragma, second run under 1 ms
PRAGMA optimize = 0x10002 after dropping the statistics again 333 ms, 110 rows

So the open-time cost is not noticeable at this size, and the bounded mask was about twice as fast as the unbounded one here too, which matches your table. The unbounded run came second on a warm file cache, so if anything that comparison flatters it.

On our branch apps/server/src/persistence/Sqlite.test.ts goes from 3 to 4 passing with your change, and tsc --noEmit in apps/server is clean.

Not measured: whether any query actually gets faster with the statistics. We have not seen the slow recovery reads from #17955 at 1.75 GB, so I have no before/after for that.

Merges `pingdotgg/t3code` up to `6002c5c68c` (42 commits, base
`5070a83a32`), using a merge commit. No open merge PR to stack on; this
branches from `main` at `882bb0a638`.

## Resolution

- **`package.json`** (conflict): dropped upstream's `tsc-rs` 0.6.0 bump
(pingdotgg#19187). The fork removed `tsc-rs`, and the `tsc-rs` tripwire holds
that. New path-policy entry: `root-package-json` (`decide`).
- **`pnpm-lock.yaml`** (conflict, `theirs`): took upstream's copy. Its
seven `tsc-rs@0.6.0` entries fail `minimumReleaseAge` before resolution
can prune them, so I stripped them and let `install.mjs` re-derive the
rest. The fork edges (`moatless-api`, `mermaid`) are back, and `verify
--only lockfile` passes.
- 14 other files that both sides touched merged without conflict.
`duplicate-adds` and `resolution-check` are both clean.
`FilePreviewPanel.tsx` (`decide`) took upstream's `threadRef` prop and
kept both fork deltas. The two unlisted files now have `converged`
entries: `terminal-drawer-open-in-editor` and `thread-shell-subtasks`.
In `threadShell.ts`, upstream's `isChildRunLive` and the fork's listing
filter merged together correctly.
- File counts: 136 files landed, against 136 in the upstream range. The
only differences are `package.json` (missing, because its one upstream
change was the dropped bump) and `docs/fork/inventory.json` (extra,
fork-only). The fork delta is 809 files.
- **Script fix:** Moatless replaced its dispatch match arms with a
`MethodTable` (moatless#1138). Because of that,
`unsupported-methods.mjs` found no arms and exited 2. It now reads
`.detached`/`.session`/`.stream` registrations and follows handlers
passed by name. Result: ADD 0, DROP 0, KEEP 2 (`vcs.switchRef`,
`git.preparePullRequestThread`), which matches `gaps.md`.

## Verification

`verify.mjs`: these checks pass: duplicate-adds, tripwires,
resolution-check, unsupported-methods (after the fix), lockfile, fmt,
lint, typecheck, build. `test` fails. Each of the failing files also
failed when run alone, and they are the same five files as in the
2026-10-13 merge, all caused by the sandbox: shared `AgentScope`, server
`ServerBrowserPage` (no Playwright browser), `AcpAdapterV2`,
`OrchestratorReplayFixtures`, and `OpenCodeServerLedger`.

## Usable as-is

- Full-screen visual replies with zoom (pingdotgg#17561). Links inside HTML
renders open the way chat links do (pingdotgg#18897). App shortcuts keep working
after you click into an HTML render (pingdotgg#18898).
- Touch-scrolling in terminal output (pingdotgg#11305). Fixed: answering a
question reset the editor caret (pingdotgg#11337). Links in agent questions are
now clickable (pingdotgg#19197).
- Fixed: "Subagent needs input" stayed after the subagent finished
(pingdotgg#18996). This is client-side and uses `activityRunStatus` with a
fallback to `status`.
- Slash skill search now works mid-message (pingdotgg#19107). Fixed: removing a
project opened a new thread for it (pingdotgg#15864). Settings now warn when an
environment silently drops a setting (pingdotgg#18334).
- Smaller fixes: context handoff details use the available width
(pingdotgg#19098), pace icon alignment (pingdotgg#18580), tool-call collection perf
(pingdotgg#18853), git file paths keep whitespace in the contract schema
(pingdotgg#12465), Android beta links (pingdotgg#18955).
- Pending context-handoff preview when you pick a model (pingdotgg#18369). It is
computed on the client from `server.providers`
(`requiresNewThreadForModelChange`, `continuation.groupKey`). If
Moatless does not report those fields, the preview shows a handoff on
every provider switch.

## Unsupported in Moatless / needs implementation

- Delegated tasks in separate worktrees (pingdotgg#19102): `isolation:
"worktree"` on the orchestrator MCP `delegate_task`, and
`workspaceStrategy` on the child-launch command. Moatless does not
create worktrees (`FEATURES.worktreeSelection`).
- These land behind flags that are already gated: scheduled-task dialog
fix (pingdotgg#18940, `scheduledTasks`), PR merge/auto-merge button sizing
(pingdotgg#18478, pingdotgg#19111, `pullRequestSurface`), remembering editor selections

## Backend behavior to consider reproducing in Moatless

I added these to the "Runtime fixes upstream made to its own server"
entry in `docs/fork/gaps.md`:
- Worktree isolation for delegated tasks (pingdotgg#19102).
- Snoozed threads still take queued automatic wakes (pingdotgg#19099).
- Indexed failed-thread error lookup (pingdotgg#18628).
- SQLite: no fsync on every commit, and planner stats collected on open
(pingdotgg#19205, pingdotgg#18471).
- Distinct CI checks with overlapping names (pingdotgg#19162).
- Bitbucket short-hash heads in review guides (pingdotgg#18900).
- Webhook template backtracking (pingdotgg#19160).
- OpenCode timeout formatting (pingdotgg#12469).
- Pi tool schemas and turn-start queueing (pingdotgg#18643, pingdotgg#18639).
- Usage hubs without a host, and Antigravity accounts deduplicated
(pingdotgg#11368, pingdotgg#19207).
- Haiku 5.5 as the default for background text generation (pingdotgg#18781).
