Problem

Claude persists each session's task list on disk, but the Claude adapter only kept it in memory. After a server restart, or when a new CLI process resumes the session, a TaskUpdate on an older task was dropped and a TaskCreate replaced the whole list in T3 Code with just the new task.

Change

The first task tool result a process sees for a session now starts from Claude's stored list at <config dir>/tasks/<list id>/N.json:

  • The config dir is the instance's CLAUDE_CONFIG_DIR (resolved against the cwd, like the adapter's other config-dir lookups) or ~/.claude.
  • The list id is CLAUDE_CODE_TASK_LIST_ID, falling back to the session id, which matches how Claude picks it.
  • _internal tasks are skipped, as Claude's own TaskList skips them.
  • A missing or unreadable store falls back to today's in-memory behavior. The directory is read at most once per session per server process.

Alternative

Seed the list from T3 Code's own last persisted todo_list plan for the thread instead of Claude's files. That avoids depending on Claude's undocumented file format, but misses anything Claude changed while T3 Code wasn't watching. Reading Claude's store always matches what Claude's next TaskList returns, so this PR reads the store. Happy to switch if maintainers prefer the other trade-off.

Note

This touches the same task tool result branch as #18394 (subagent task updates). Whichever merges second needs a small rebase.

Scope and approval

A small fix to the Claude task list support merged in #14964.

Verification

  • vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts: 232 passed on top of current main, including a new test that starts from a task list stored before a restart.

🤖 Generated with Claude Code

The Claude adapter kept each session's task list only in memory, while
Claude persists it per session on disk. After a server restart or a new
CLI process resuming the session, a TaskUpdate on an older task was
dropped and a TaskCreate replaced the drawer with just the new task.

The first task tool result a process sees for a session now starts from
Claude's stored list (<config dir>/tasks/<list id>/<id>.json, list id
being CLAUDE_CODE_TASK_LIST_ID or the session id). A missing or
unreadable store falls back to the old in-memory behavior.

Approvability

Verdict: Approved at 8cba662

Macroscope's review found this PR approvable — This is a focused Claude adapter fix that restores task-list state from Claude’s existing on-disk session files after restart, while preserving current in-memory behavior and safely falling back when storage is unavailable. The accompanying tests cover recovery and empty-list handling without changing schemas, defaults, deployment, or security-sensitive code.

📝 Walkthrough

Walkthrough

The Claude adapter now loads persisted task files when a root task-tool result arrives before the process has an in-memory task list. It applies the result to the loaded tasks and projects the resulting list.

Changes

Claude task-list recovery

Layer / File(s) Summary
Load persisted task files
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts
The adapter resolves the Claude config directory and task-list ID, then reads numeric task JSON files. It skips invalid, unreadable, incomplete, and internal tasks.
Use recovered tasks in projections
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
When no in-memory list exists, the adapter applies the root task-tool result to persisted tasks. Tests cover recovery after restart, updates and creation, and deleting the last stored task.

Change: Bug fix

Merge Risk: 🔵 Low · up to 8cba6

Task recovery may miss stored tasks when Claude runs with a configured HOME different from the server’s. The change is mergeable with that bounded limitation explicitly accepted or corrected.

Security Architecture Review

Security architecture risk: 🔵 Low · up to fe6df

The change adds local task recovery without changing access permissions. Empty recovery does not establish a completed initialization state, and isolation for custom task-list identifiers is not fully established. No exploitable security issue was verified.

Retained concerns

  • Low · architecture · observed: The new recovery transition does not record an initialized empty list when applying the tool result returns undefined. Successful empty recovery and directory-read failure both become empty maps, and only a nonempty recovered map provides the fallback needed for caching. Subsequent recognized results can therefore repeat filesystem recovery instead of completing the intended once-per-session initialization. This is a bounded recovery and state-ownership concern, not a verified security vulnerability.
Security review details

Security Blast Radius

  • inferred — The inspected flow exposes recovered task subjects and statuses through the current application thread’s plan events, using the server’s filesystem service to read the configured store. Influencing imported data requires influence over that store or its configuration. Wider tenant exposure, filesystem ownership and downstream privileged interpretation were not established.

Trust Boundaries and Controls

  • observed — List-ID normalization removes traversal characters, filenames must match numeric JSON names, and imported fields undergo validation. Root-only gating excludes subagent task results. Normalization is nevertheless lossy: distinct custom identifiers can map to the same directory. Whether such identifiers are independently attacker-controlled, or normalized identically by Claude, remains unresolved; no cross-session exploit was verified.

Resilience and Maintainability Implications

  • observed — Recovery is read-only and suppresses filesystem errors, containing ordinary read failures within task projection. Cache mutation precedes event emission, and the inspected sequence has no transactional coupling between those steps; that ordering existed at the base and is not a newly introduced authority or rollback defect.

Hardening Proposals

  • proposed — Define a distinct recovery initialization state that preserves successful empty snapshots separately from unavailable stores, making the read-once and terminal-state ownership guarantees explicit.
  • proposed — Document and validate the custom list-ID namespace against Claude’s behavior, including collision handling and the authority required to select a shared task store.

After a restart, the first task tool can find Claude's stored list already
empty, for example when it deleted the last task. That empty list was
treated like a failed read, so no clearing update was emitted and later
results kept rereading the store. Only a missing or unreadable directory
now counts as no stored list.
