Claude Auto mode currently allows escalated commands without showing an approval request. Install the existing permission callback in Auto so requests from ask rules, hooks, or the classifier wait for a human decision. Full access and explicit approval-policy overrides keep their existing behavior.

Keep the SDK title and append its decision reason with terminal escapes removed. A session approval without SDK suggestions allows the current Bash/Monitor request without manufacturing a tool-wide command permission; scoped SDK suggestions and other-tool fallbacks remain unchanged.

Maintainer takeover of @TonybynMp4’s #15360, preserving the contributor commits and merging main. Rebuilt at the existing policy, approval-request, and session-permission boundaries. Removed the extra prompt helper and reduced test rewrites. The earlier web wrapping change is already on main; this PR changes only the server adapter and its tests.

Verification on headless Linux:

  • Original adapter suite: 291 cases → 290 cases; main’s 286 cases are preserved apart from the required Auto-policy expectation. Four new callback cases cover Auto accept/decline with the combined sanitized prompt, and current-command-only session approval for Bash/Monitor. Unchanged main production gives 285 passing / five failing; the rebuilt adapter gives 290 passing. Existing controls cover full access, explicit overrides, scoped SDK suggestions, and other-tool fallback.
  • All 391 tests across the adapter, runtime-request service, session manager, turn-start, and runtime-policy suites pass. Server typecheck and scoped lint/formatting pass; the unused adapter layer warning is pre-existing.
  • A standalone run through the real adapter with synthetic SDK transport observed pending command request → wait → runtime response → allow/deny. Session approvals returned updatedPermissions: [] for both Bash and Monitor. No shell command was executed by this adapter probe.
  • A separate real SDK 0.3.276 / Claude CLI 2.1.303 run in an isolated home and workspace confirmed the native escalation boundary:
permissionMode: auto
PreToolUse(Bash): ask
canUseTool: Bash, command="printf T3_SYNTHETIC_APPROVAL"
decisionReason: "Synthetic maintainer approval verification"
response: deny
result: success, "denied"
hook calls: 1; callback calls: 1; deadline reached: false

Current-head CI: all eight server shards, build/typecheck, lint, and aggregate Check pass on 4297440b83. Web and native-specific jobs skipped normally. Macroscope Approvability remains neutral and requires human acceptance; there are no unresolved review threads.

This is a behavior change for unattended Auto turns: scheduled, webhook, and delegated work can wait for a human when Claude escalates. Maintainer acceptance of that policy change remains required. Session approval without a scoped Bash/Monitor suggestion permits only that request, despite the existing session-approval choice; later commands may ask again. Other tools retain their existing fallback, including PowerShell, whose suggestion-less native behavior was not verified on Windows.

The adapter is shared by web, desktop, mobile, and local/remote connections; wire contracts and UI are unchanged. The native probe and synthetic adapter probe verify separate boundaries, not a full running-app approval round trip. UI rendering, persistence/WebSocket delivery with a real CLI, classifier- and ask-rule-triggered native requests, later native command-rule enforcement, macOS, Windows, iOS, Android, and packaged Electron were not verified.

Fixes #15353.

📝 Walkthrough

The Claude adapter now installs permission callbacks for runtime modes other than full-access when no explicit approval policy is set. It formats approval prompts with distinct decision reasons and avoids creating session-wide allow rules when Claude provides no permission suggestions.

Changes: Claude Permission Handling

Layer / File(s) Summary
Runtime permission callbacks
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter installs a permission callback for runtime modes other than full-access when no approval policy is set. Tests cover callback installation and escalation approval outcomes.
Approval prompt content
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
Approval prompts use the summarized tool request as the base. They append a distinct decision reason after stripping VT control characters and trimming it. Tests cover prompt content and allow or deny outcomes.
Session permission updates
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter returns no session permission updates for Bash or Monitor when Claude provides no suggestions. Tests cover accepting these tools for the session.

Merge Risk: ⚪ Minimal · up to 42974

Auto escalations use the existing approval delivery path, and no merge-blocking issue remains in the reviewed change.

Security architecture risk: 🔵 Low · up to 42974

The change strengthens human approval for escalated commands and prevents unintended session-wide command access. Remaining uncertainty concerns approval cleanup during interruption and recovery, not a demonstrated authorization bypass.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The affected authority is tool execution within the relevant provider sessions. The change strengthens approval for requests escalated by the SDK; it does not establish that every command requires human approval or introduce a new execution sandbox.

Trust Boundaries and Controls

  • observed — The callback denies requests without an active turn. For approval-requiring requests, human acceptance allows the supplied tool input, decline denies execution, and cancellation denies with interruption. Responses address an exact pending request ID rather than granting authority to an arbitrary tool invocation.

Resilience and Maintainability Implications

  • inferred — Concurrent responses target the same single-assignment decision, so they do not create multiple permission results for one callback. Explicit duplicate-response rejection and complete interruption/recovery behavior remain unverified; the underlying decision and cleanup mechanisms predate this PR.

Explanation

Issue #15353 requires Claude Auto escalations, including ask-rule commands, to show an approval request. claudeRuntimeQueryPolicyForRuntimePolicy now installs the permission callback for every mode except full-access when no explicit policy applies. The callback waits for the runtime decision, maps accept and decline to allow and deny, and appends a VT-control-character-sanitized decision reason. Tests cover Auto callback requests, decisions, and prompt content. Existing tests cover explicit policy behavior.

Full details: Out of Scope Changes check

Explanation

The changes stay within issue #15353. Permission callback installation implements Auto escalation prompts. Prompt sanitization supports the approval request. The Bash and Monitor session-permission guard prevents an approval without SDK suggestions from creating broad command access. The added tests directly verify these behaviors. No unrelated change is established.

Full details: Description check

Explanation

The description is comprehensive and covers the problem, implementation, scope, linked issue, approval context, verification results, limitations, and model/harness information. It does not use the template headings exactly and does not include a direct maintainer approval comment, but the required information is otherwise substantially present.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — The shared server adapter changes Claude Auto command execution from automatic escalation handling to human-gated approval and changes session permission scope for shell tools. Although the implementation is focused and well covered by tests, this is a security-sensitive command authorization change requiring human review.

Thanks for picking this up. The approach looks right to me. runtimeMode !== "full-access" also means any mode added later will ask by default, which is the safe direction. A few notes from reading it against current main and the pinned Agent SDK (^0.3.276):

1. The rebase conflict is small. The source file merges cleanly. In the test file, main has since added applyFlagSettings: () => Effect.void to the stubbed query session, so openClaudePermissionTurn needs that line too. I'm happy to push the rebase if that helps.

2. A hook's reason doesn't reach the user. #15353 is about a PreToolUse hook returning ask. The request prompt is built as title ?? description ?? decisionReason ?? …. The SDK nearly always sets title ("Claude wants to run …"), so the hook's reason in decisionReason gets dropped. The user sees an approval request but not why it was raised. The SDK describes that field as the reason the ask escalated, meant "for the consent line of the host's dialog", and notes it may contain ANSI escapes, so sanitize it before rendering. Could the prompt show it alongside the title?

3. Suggested test for the hook path. The new Auto tests send a plain git push through the callback. That proves the callback asks, but nothing covers the hook case from the issue. A test that passes decisionReason in the callback options and checks it appears in the raised request would cover #15353 directly, and point 2 too if it's addressed.

4. "Accept for session" in Auto (could be a follow-up). After this change, the requests a user sees in Auto are the ones Claude declined to auto-approve. The SDK passes suppressAlwaysAllowRule and defaultToNo for these, but the adapter doesn't read either. Also, when there are no suggestions, toSessionPermissionUpdates falls back to a whole-tool rule ({ toolName }). So accepting one escalated Bash command for the session allows every Bash command after it. That isn't new, but this PR is what starts putting these requests in front of Auto users. If it's out of scope here, it deserves its own issue.

5. Unattended runs (worth a line in the description). An approval only ends when someone answers or the SDK aborts the call. Scheduled, webhook and delegated threads in Auto will now pause at escalations instead of running through them. Supervised mode already works this way, so I think that's correct, but it changes how Auto behaves. If you want a follow-up option: without a canUseTool callback the SDK simply denies asks, so "deny when nobody is attached" would be easy to build.

Thanks, this was useful.

  1. Rebased onto current main, with applyFlagSettings added to the stubbed session.
  2. Done in beb7427. The prompt now shows decisionReason on its own line under the title, with ANSI escapes stripped. The web approval panel also wraps command prompts now, since it was hiding long reasons off to the right. I checked it on web with a PreToolUse hook returning ask with a reason.
  3. Added a test that passes title and an ANSI-coloured decisionReason through the callback in Auto and checks the raised prompt.
  4. Agreed it's real and predates this PR, but it's out of scope here. Could you open a separate issue for it?
  5. Added a note to the description. Unattended Auto threads now pause at escalations, as Supervised does.

The prompt is still one string, though, so the panel can't tell Claude's label, the command and the reason apart. The command itself is also often missing, because the SDK's title is usually Claude's own label ("Commit the test edit"). Fixing that properly means separate fields on the approval item and changes on web and mobile, which is out of scope here. Feel free to take it on in a follow-up PR.

The SDK nearly always sets a title, so the reason an ask escalated, such as
a PreToolUse hook's permissionDecisionReason, was dropped from the prompt.
It now follows the title on its own line, with ANSI escapes stripped.

When Claude offers no suggested rule, a session approval for Bash fell back
to a bare `Bash` rule, which allowed every later command for the session,

Taking over this one. I’ll merge current main into the original branch, reduce the tests to focused approval behavior, rebuild the adapter change, and run independent reviews plus headless verification. I’ll preserve the author’s commits and leave the PR unmerged.
