fix(server): refresh held workspace skill lists on provider refresh - #13251

ApprovabilityVerdict: Not approved Macroscope's review found this PR not approvable — This PR adds cross-provider background workspace rescanning to existing refresh flows, including new registry orchestration and provider-specific filesystem/app-server work that changes user-visible skill lists. The asynchronous implementation also has an unresolved freshness race in the Cursor path, so the change warrants human review. Not approved because:

After provider syncs, the registry rebuilds stale workspace snapshots and publishes changed provider lists. Cursor scans update held workspace skills when the scan is newer than the stored snapshot. syncProviderstarts each workspace rebuild in a background fiber. If generationt1finishes after generationt2updatesprovidersRef, thet1result can pass the current guards and replace a held snapshot because the callback does not compare the current provider generation. If thet2scan fails, its ignored error leaves the older skills published.Suggested fix

Workspace skill snapshots were built the first time a folder was opened and kept for the life of the server, so skills installed later never reached the `$` picker for that folder. After each provider refresh, the registry now rebuilds the workspace snapshots it holds through a new optional `snapshotsForCwds` driver hook, once per refresh and in the background, publishing only when a folder's skills changed. Codex answers all folders from one app-server `skills/list` call; Claude and Cursor rescan their skill directories. Grok, OpenCode and Antigravity keep their existing behavior. …uild A background rebuild only replaces workspace entries older than its scan start, so a workspace refreshed while the probes run keeps its newer skills. Co-Authored-By: Claude Opus 5.5 (1M context) <noreply@anthropic.com>

The
Effect.catchhandler swallows everyprobeCursorSkillsfailure and logs it at debug level only. Failures such asEACCESorENOTDIRtherefore stay invisible in normal logs. The affected folder silently keeps its stale skill list. This follows the learning that onlyENOENTis a legitimate empty result and other errors must be surfaced. The omission itself is sound, because it preserves the held entry. Raise the level to
logWarningso real failures show up.Proposed change
- Effect.logDebug("Cursor workspace skills rebuild failed", { cwd, cause }).pipe( + Effect.logWarning("Cursor workspace skills rebuild failed", { cwd, cause }).pipe(Based on learnings: only ENOENT is a legitimate empty result; other directory-read errors must be surfaced as diagnostics.

Expose snapshotsForCwds for managed Codex.
When config.setupMode === "managed", CodexDriver.create returns before it attaches the batch callback. CodexManagedProvider exposes only snapshotForCwd, although it already resolves the runtime configuration and environment for workspace skill probing.
ProviderRegistry skips instances without snapshotsForCwds during refreshes. Managed Codex workspace snapshots can therefore remain stale after skills are installed.
Add snapshotsForCwds to the managed instance. Use the resolved runtime configuration and environment with probeCodexSkillsForCwds, as the standard Codex path does.

Thanks for working on this. We merged the orchestrator V2 rewrite in #2829, and we are closing this PR as part of that transition. The patch conflicts with the rewrite in apps/server/src/provider/Drivers/ClaudeDriver.ts, apps/server/src/provider/Drivers/CodexDriver.ts, apps/server/src/provider/Drivers/CursorDriver.ts and 1 other files. Even where the conflict is small enough to rebase, we are asking for fresh PRs against the new base so we can review and verify the behavior in V2. Sorry for the extra work this creates. If the change is still needed on V2, please rebuild it on current main, verify it there, and open a new PR linking back here. We're closing the current implementation without assuming the underlying request is resolved. |
The
$skill picker uses a per-folder skill list (workspaceSnapshots) that the server builds the first time a folder is opened and keeps until it restarts. A skill installed after that, global or project-level, never shows up in that folder's picker even though the agent can run it.Fix
After each provider refresh (the existing health check, 5 minutes by default, or a manual refresh), the registry rebuilds the folder lists it already holds. This happens in the background, at most once per refresh, and publishes only when a folder's skills actually changed. Opening a chat does no extra work.
