Polishes the README and docs/header.svg to match @otto-assistant/opencode-claude and @otto-assistant/opencode-cursor.

Changes

  • Animated header SVG (OTTO badge, mark, title, subtitle, accent bars, scan line) in the command-code green/amber palette
  • Full badge row: npm version, downloads, MIT, platforms, GitHub release
  • Nav: Install · Authenticate · Why this plugin · Architecture · Changelog
  • Install warning that command-code is not a built-in provider
  • Auth options aligned with sibling /connect / opencode auth login flows (Go plan, no Studio API key)
  • Release section (GitHub Actions workflow inputs) + FAQ
  • Keeps Laguna S 2.1 free as the default live target

Stacked on feat/opencode-commandcode-36eb so this PR stays README/docs-only.

Align badges, nav, Install/Auth/Why/Architecture/Release/FAQ with
opencode-claude and opencode-cursor, and replace the stub header.svg
with the animated OTTO badge layout in the command-code palette.
