Empty outputSchema created for tools with unstructured output

**Describe the bug**

If a tool has unstructured output, rmcp still produces an empty `outputSchema` field in the tool list:

```
outputSchema: {}
```

This makes some tools crash with an error (similar to #445).

Optional fields should probably be omitted in the tool list response.
