Allow client-specific settings beside each MCP server configuration so authors do not need a separate mapping keyed by server name. Reuse reverse-domain extension namespaces across all transport variants. Validate container shapes through the existing server failure boundary while leaving namespace object contents and implemented extension behavior client-defined. Preserve portable environment semantics and the published 1.0 contract.

Summary
This PR adds an optional
extensionsfield to each MCP server configuration in Agent Plugins 1.1.0, using the same namespace model asplugin.json. Plugin authors can keep client-specific server settings alongside the portable configuration.Motivation
In openai/codex#38438, a plugin author needed to pass an environment variable such as
DB_PASSWORDto an MCP server. Codex supported this through anenv_varslist in its native configuration, but portablemcp.jsonhad no place for that client-specific setting. The Codex fix reads a second, legacy configuration and matches servers by name to obtain their environment-variable lists.Rationale
Keeping client-specific settings beside the MCP server they affect lets authors configure that server in one place, without maintaining a separate mapping that must stay synchronized when servers are renamed or copied. Reusing the existing extension model gives clients a place for native settings while keeping their meaning client-owned.
