Ship the amico verb surface: package launcher + AGENTS.md bookkeeping-verbs section

Stacked on #131 (base aaron/vault-mounts-verbs) — top of the b1→b3→#131 spine chain.

Layer 0 of the integration doctrine (spec-20260711-132200, personal vault): the session-study found the entire amico verb surface (catalog/vault/device/note) implemented but unreachable and undiscoverable — not staged into the .vsix, not on PATH, and never mentioned by AGENTS.md/scores/skills. Zero invocations across 6 fresh probe sessions; warm-starts were done by hand-globbing the pulse bank and results never recorded in-session.

Three changes:

1. esbuild staging ships launcher/amico + dist/amico.js beside amico-run — one PATH prepend now resolves both bins. Verified: vsix contains both; staged amico vault status resolves the real 3-mount stack.
2. AGENTS.md § Bookkeeping verbs: before authoring → catalog query (warm-start seeds) + vault query (prior art); after FINISHED → note write --from-run + catalog ingest --from-run (promotion refuses without verification.agree); vault status/resolve for mount questions. Plus the vault layout convention (amicode/{problems,pulses,memory}) — the invented-flat-path read-error class had 16 occurrences in session mining.
3. Router fix: deprecated amicode_ask → native question (removes the mixed guidance in the spliced prompt).

Tests: extension 435 passed | 3 skipped (unchanged), typecheck clean, pnpm run package green with both bins in the artifact. CI red on the pre-existing private-fork token — unrelated.

Hygiene sweep 2026-08-27: closing as stale/superseded — opened 2026-07-11, and opencode_config.ts has since been rewritten by the XDG refactor (#560/#562/#563) and onboarding work (#449/#455). The AGENTS.md bookkeeping-verbs section this PR's title describes already landed via the M1/M4 canonical-server migration. Branch survives for revival if anything here isn't covered.
