Mapping Template Planner: Template presets v2 #12

As a possible v2 from #4 , I want to be able to customize the selected mapping template, or auto-generate a template based on simple questions:

e.g.:
Template Type?: e.g. DynampDB query
PK/SK names?
index name?
scan index forward?: yes/no
etc
