Ports the three functions from trios/agent-server/apps/server/src/lib/agents/adapter-health.ts:
- AdapterHealthChecker_getHealth (with decision logic extracted to AdapterHealthChecker_getHealth_decision)
- runtimeSnapshotToHealth
- openclawFallback

Adds 9 test cases covering:
- openclawFallback for 'openclaw' and unknown adapters
- runtimeSnapshotToHealth for ready/unready snapshots, probedAt/lastErrorAt fallback logic
- AdapterHealthChecker_getHealth_decision for null runtime, host-process, and container runtimes
