Closes #3672. Probe 2 skipped with 401 (DeepSeek), probe 3 with 402 (OpenRouter: requested 16000, affordable 2506). Spec adds RESERVE_SENDS_MAX_TOKENS / RESERVE_MAX_TOKENS (default 1024) / PAYMENT_REQUIRED_IS_DIAGNOSED, records both statuses, RESERVE_PROBES=3, RESERVE_ANSWERED_LIVE stays false. Pin 12758 -> 12759 (one test block). docs/now entry included.
