Add scheduled-only production transcript and agent smoke checks

Summary

• Add an isolated Cloudflare Worker at workers/production-smoke that calls the production transcript and agent APIs using a user-configured VIDEO2CTX_API_KEY secret.
• No public Worker fetch handler, workers.dev URL, preview URL, routes, or custom domain. Cron list is empty and checks are disabled until cadence is chosen and the operator enables them.
• Fixed single-video tests, bounded response sizes and timeouts, sequential execution, no admission retries, and persisted Durable Object deduplication/overlap protection.
• Resume unfinished known agent runs; stop for operator reconciliation when admission is uncertain. Sanitized Cloudflare logs only; no external alert destination configured.
• Add operator setup/recovery documentation and isolated CI. No platform deployment or public API contract changes.

Validation

• 20 Node/mock tests passed, including redaction, redirects, timeout/body limits, invalid responses, terminal failures, deduplication, resume and uncertain admission.
• Wrangler deploy dry-run passed.
• Actual local workerd/SQLite Durable Object test passed: scheduled handler, one mocked transcript/access/admission/poll sequence, duplicate suppression, and HTTP rejection. All outbound traffic was mocked.
• git diff --cached --check passed.
• No real API key read or configured; no production API tests or Cloudflare deployment performed. Full unrelated platform/library suite not run.

Before activation

Choose cadence (UTC, recommend at least 15 minutes), deploy only this package, configure the API key yourself, enable checks and approved cron, then verify first real run and disabled public routes. Agent calls consume account credits; timeouts do not cancel backend work. Reporting currently remains in Cloudflare logs; select an alert integration separately if desired.
