Canon Cinema EOS MXF clips get a correct Finder thumbnail, but pressing Space only shows the static file info card instead of a playable preview. The Console log shows the format reader opening the file and finding the H.264 video, then failing on the first audio track.
Environment
QLVideo: 3.12 (Media Formats and Media Codecs extensions both enabled)
macOS: 27.0.1 (26A434)
Hardware: MacBookPro18,2, Apple M1 Max
Pro Video Formats 3.2.1 is also installed
File details (from the QLVideo Spotlight importer and the camera's XML sidecar)
Container: MXF OP1a (org.smpte.mxf), recorded on a Canon camera, file names like A007C194_260902IP_CANON.MXF
Video: H.264 High 4:2:2
Audio: Linear PCM, 48 kHz, separate mono tracks (sidecar reports EBUaudiochannels:mono_, acdc:c108)
Duration of test clip: about 60 minutes (70 GB)
I'd expect a playable preview, or at least video-only playback when the audio layout can't be determined. Instead Quick Look falls back to the generic info card.
I saw 3.09 made the reader more permissive about unspecified channel layouts, but these Canon mono PCM tracks still trip it. Treating an unlabeled single-channel PCM track as mono would probably cover it. I can record a short test clip on the same camera if a sample would help.
"unspecified channel layout" is just a warning. You're not seeing the "can't guess layout for N channels" error so the format extension (since 3.09) is assuming the audio stream is mono.
It's more likely that the problem is that the format extension has decided that the video stream is well-formed H.264 and so has handed it off to macOS VideoToolbox instead of to FFmpeg via the codecs extension, but VideoToolbox doesn't like it for some reason. (VideoToolbox is faster and more energy efficient than FFmpeg software decode, but is also pickier).
In any case please upload a short test clip. I don't think GitHub will allow you to upload a .mxf file directly so you will likely have to put it in a .zip file.
