docs: the #155 phase 2 design record the tree cites - #318

The spike folder lived out of tree in a scratch worktree directory, yet tools/, src/ and tools/package/ cite "DECISIONS N" from it. Copied as it stood (survey.err, empty, dropped), with an archive note on the README; trust_bundle.py's first cite now names docs/155-phase2/DECISIONS.md. No source or build change. Flatten to DECISIONS, S3, L3 and S4 plus the S3 survey data in data/; the README is now an index plus the S1 hardware storage findings, which are recorded nowhere else. Dropped as superseded: the S2 reclaim patch (merged as #260), the M3 client sketch (uci-m3 merged in #271) and the S4 probe script and raw JSON (the figures stay in S4-reu-banking.md). docs/155-phase2/. Until now it was an untracked folder outside the repo, but the tree cites it.
