Backport to Jaybird 5: usage of FatalErrorHelper in FBPooledConnection

In Jaybird 5, FBPooledConnection still has its own logic for fatal error detection, which is much too wide. Backport Jaybird 6 changes to use FatalErrorHelper (see commit 0b77ce0)
