Policies leave the npm package and become packs — published as GitHub releases,
installed by digest, chosen by the person installing them rather than by us.

What a user does now

failproofai config setup: agents, daemon, cloud — NOT policies
failproofai policies what is on this machine
failproofai policies add pick from a list
failproofai policies add block-sudo a policy — no slash
failproofai policies add acme/guard a pack — slash means source
failproofai policies add core ours, from this package, offline
failproofai policies remove <what> a policy, or a whole pack
failproofai policies show acme/guard what a pack holds, before installing
failproofai publish ./my-policies.mjs build + release + upload

policy, pack and p all still work — they translate to policies, so
nothing anyone has typed before breaks.

Setup stops choosing policies for you

The wizard's policy step is gone, and so is the larger offender behind it: the
opening "Recommended" path skipped that question entirely and installed
RECOMMENDED_POLICIES, fifteen hardcoded builtin names, on behalf of somebody
who had not seen the list. failproofai ships no policies of its own now — they
arrive as packs — so pre-ticking our set is a product decision taken for a
user who cannot yet evaluate it.

Whatever the scope already had is carried through untouched: installHooks runs
with replace: true, and passing anything less would switch off policies the
user turned on. Running setup twice must never reduce protection.

policy-presets.ts is deleted. customPoliciesEnabled is left alone in both
modes rather than written from a checkbox that no longer exists — which also
closes the leak where finishing setup disabled every convention policy on disk.

Consequence, stated plainly: a fresh machine finishes setup with nothing
enforcing but block-failproofai-commands, the compiled always-on guard. That is
now the intended state.

Three commands become one

Two of them were a single letter apart and did unrelated things. policies add
takes either a policy name or a pack source, told apart by a slash — a policy
name matches /^[A-Za-z0-9._-]+$/, so a slash is already illegal in one and
unambiguous in the other. Same rule npm and docker use, and nobody has to
discover a flag before they can install somebody else's policies.

pack list was two commands wearing one name — bare it described this machine,
with an argument it described a pack somewhere else. Those are different
questions and they are different words now.

Installing shows you the list

policies add with no argument used to answer "Missing policy name" and tell you
to go read a list elsewhere and come back — the command telling the user to do
the work it exists for. The same objection applied to a bare pack add, which
took the publisher's defaults and only afterwards printed what it had decided: a
default is a suggestion, and a suggestion nobody saw is a decision taken on their
behalf.

One screen now, every policy grouped by pack and category, current state
pre-ticked, built on the multiSelect the wizard already uses rather than a
second picker.

Publishing is one command

Four before, two of which published nothing: installs read
releases/download/<tag>/<asset> and never touch the git tree, so git init and
gh repo create were for humans reading the source. A publisher could only learn
that by reading pack-store.ts.

It goes over the GitHub REST API, not gh release create — our own
block-gh-pipeline builtin matches that exact command, and shipping a publish
path our own guardrail blocks is not a thing to do.

Two silent failures are refused: a tag that does not describe the manifest
version, and a private repository (which publishes to nobody — pack add sends
no Authorization header at all, by design).

Help: 152 lines to 26

Every flag of every command was inlined on the index, so the thing you read to
find a command was the thing you read to use one, and the cost fell on the person
who knew least. One screen now, plus failproofai help <command> — which
dispatches to <command> --help, so there is exactly one copy of each and the
two spellings cannot drift.

Three things that were documented nowhere reachable now are: update --help and
migrate --help both exited 1 with "Unexpected argument" because SUBCOMMANDS
omitted them, and --hook — spawned on every tool call — appeared only in a
module docblock and one error string.

Colour

The design system defines two accent hues and says so explicitly. tui.ts
carried three: selection and "enabled" in #ff2e88, in no brand token, next to a
near-duplicate logoPink one byte from the real one. Collapsed to a single brand
pink, so the logomark and the prompts cannot drift.

A 256-colour tier is added between the two that existed — resolution jumped
straight from 24-bit to basic ANSI, so every terminal that does 256 but does not
advertise COLORTERM, which is most of them over SSH, fell all the way back.

Fixes found on the way

  • The release tag was never checked against the pack version. spec.tag only
built URLs, version only came from the manifest, and the two were never
compared — so a pack built --version 1.0.0 and released under v1.0.0
installed cleanly while recording a version that matched no URL. It bit
immediately: pack build told publishers to tag one way while this repo tags
the other, so house style broke your own pack. A leading v is accepted; any
other disagreement fails the install naming both values.
  • A tagless add misresolved on prereleases. Resolution reads the
releases/latest redirect, which GitHub does not issue for draft or prerelease
releases, so an install landed on an older stable tag or nothing at all.
  • A long policy name ate its own description. The name column caps at 24 and
the description budget is sized against that cap, but padEnd pads and does
not truncate. Measured on a real pty:
sanitize-connection-strings and sanitize-private-key-content both landed on
exactly column 80 — nothing wrapped, nothing looked wrong, the description was
simply cut by the terminal instead of by ellipsize and lost its ….
  • policies add answered a script with an exit code it could not act on. The
no-terminal refusal was checked after the no-packs branch, so a fresh machine
running it from a pipe got the empty-state screen at exit 0.
  • The dashboard offered a builtin source filter nothing can produce.
  • pack-manifest.ts documented a safety condition that had stopped being
    true.
It said its fail-open was defensible only while the builtins shipped
compiled in, and that the day they became a fetched pack this must be revisited
rather than inherited. That day arrived; the denying did move to
pack-failclosed.ts, so only the comment was lying.

Gates

tsc clean · lint 0 errors · 4312 tests passing · bun run build clean.

The 8 failures in dogfood-configs.test.ts on a contributor's machine are local
only — those configs are skip-worktree and their working copies get emptied. A
clean clone at HEAD runs them 63/63.

Not in this PR

docs/ still documents the old command spellings — 177 references to
failproofai pack … / failproofai policy … across 15 locales. They keep
working as aliases, but the site should be updated; that wants its own pass on
the English source, then the translation job. FailproofAI/policies' README
likewise still says builtins.

Summary

The pack system is broadly implemented and targeted type/tests passed. One medium-confidence data-safety issue remains: artifact digest equality can delete an unrelated installed pack.

Changes

  • Introduces GitHub-release policy packs with manifest validation, digest pinning, loading, fail-closed handling, selection, and publishing.
  • Unifies policy/pack CLI workflows and adds dashboard pack management.
  • Moves enforcement metadata into packs while retaining the compiled self-protection guard.
  • Adds incremental audit caching, home migration updates, and TUI presentation changes.

Validation

  • Passed docker run --rm -v /review/input/workspace:/workspace -v hermes-pr738-node-modules:/workspace/node_modules -w /workspace oven/bun:latest sh -lc 'bun install --frozen-lockfile --ignore-scripts && bunx tsc --noEmit && bun run test:run -- __tests__/hooks/pack-store.test.ts __tests__/hooks/pack-shared-artifact.test.ts __tests__/audit/incremental-scan.test.ts __tests__/audit/incremental-fallback.test.ts' — Dependency install, TypeScript check, and targeted pack/audit tests exited successfully in a nested container. Findings

No blocking findings.

1 advisory finding
  • Medium/High Do not treat identical artifact bytes as a pack rename — priorRecordFor accepts a record with the same SHA-256 even when its id differs (src/hooks/pack-store.ts:618). upsertInstalled then removes every differently named record with that digest (src/hooks/pack-store.ts:1157). Thus, after installing acme/one with a narrowed selection, adding other/two whose entry artifact is byte-identical replaces and removes acme/one; digest equality proves shared code, not that the user intended a rename. The loader already supports multiple pack records sharing one artifact. (src/hooks/pack-store.ts:618)

Update — the package no longer ships policies at all

policy-pack/ is out of files and out of build, installBundledPack is
deleted, and core is a spelling of FailproofAI/policies — resolved in
pack-store so the CLI and the dashboard cannot disagree about it, then fetched,
digest-verified and pinned like anybody else's pack.

A pack that ships inside the binary is a policy set chosen for the user and
written to their disk before they asked, and it gave our own policies a delivery
route no third-party pack could use — the opposite of what this lane exists to
make possible. Offline install now fails where it used to silently succeed,
which is the honest answer: there is nothing local left to install. An
already-installed pack keeps enforcing offline; only installing needs the
network.

Two things checked rather than assumed, because both would have been quiet
breakages:

  • The audit cache key does not move. Packs are folded in by
id|version|sha256, so a machine that had the vendored copy and now has the
fetched one only keys identically if the bytes match. They do — 9e63e6e2…
both ways — so no existing user takes a cold rescan on upgrade.
  • Audit scores identically with nothing vendored. registerFromVendoredPack
already returned false for an absent directory and fell back to the compiled
implementations, its own comment naming "a tarball packed without it" as an
    expected case.

The fp-reset migration deliberately does not fetch: resetHome is
synchronous and runs inside failproofai update, and an upgrade that blocks on
github.com — and fails when it is unreachable — is worse than one that finishes.
The carried names stay in config, which is what the no-pack fallback reads.

Three more fixes this turned up:

  • Only the deleted bundled branch ever suggested --policy, so every
third-party pack was told about --category and --all and never about
    taking a single policy.
  • policies remove promised "re-adding it works offline", which stopped being
    true.
  • The no-pack fallback warning told a stranded machine to run
failproofai update "to move them into the pack that ships with it" — both
halves false now. It names failproofai policies add core. Nothing had been
asserting on that string; a test does now.

Rebased on latest main. Two failure sets on this branch are not from it,
both verified rather than asserted:

  • dogfood-configs (8) fail only in a working tree where those skip-worktree
configs have been emptied locally; a clean clone at HEAD runs them 63/63.
  • python-version-pipeline (4) come from main: both _version.py files say
0.0.1b2 while the newest changelog section is 0.0.1b1. This branch touches
no file under sdk/ or fp-cloud-cli/. The auto-bump commits that caused it
carry [skip ci], which is why nothing caught it — it will now fail on every
branch cut from main until a 0.0.1b2 section exists.
  • Supply Chain is red for three chromadb advisories with no fixed version;
re-running the previously green scan on unchanged code fails identically.

Update — choosing at install time, one linear setup, and a TUI pass

Installing a pack now asks instead of announcing. policies add <source>
took the publisher's defaultEnabled flags and printed the result afterwards,
which turns a recommendation into a decision made on the user's behalf — by
which point the policies are on their machine. A human who names no flags gets
the pack's list first, defaults pre-ticked, grouped by category. It reads the
MANIFEST only, so deciding about a stranger's pack still never downloads a
stranger's code. Flags and non-TTY skip it entirely.

Setup is one linear flow. The opening "Recommended or Customize?" is gone —
a question about the wizard rather than the machine, unanswerable until you know
the alternatives, which you learn by picking one. Recommended then took global
scope, the detected CLIs and fifteen unseen policies. Three questions remain, in
the order the machine needs them: the daemon (first, the only one needing a
password), which harnesses, and whether to connect. Scope is global always; the
harness step is now always asked rather than inferred.

The listing stopped contradicting itself. It said "not installed" directly
above an installed pack, because it was reporting whether HOOKS are wired — a
different question from whether policies exist, and one that hid the state that
actually matters: thirty-eight policies present and nothing calling failproofai
to run them. It reads N on · NOT ENFORCING now.

A TUI pass against the house guide, which found two things looking could not:

  • Every keystroke redrew the region as TWO writes — clear, then lines. A
terminal may paint between them, so the cleared state is a real frame:
invisible locally, a blank flash per keypress over SSH or in tmux. One write
now, wrapped in synchronized output (DECSET 2026).
  • The NOT-ENFORCING warning was hand-wrapped into four lines while warning()
wraps each element it is given, so the author's breaks became paragraph breaks
and wrapped again inside themselves — leaving "them." alone on a line at 60
    columns.

⚠ is also gone: the design system forbids emoji, and it takes emoji
presentation on most terminals, which makes it two columns wide and silently
broke the hang-indent of the block it sat in. Section rules move to the brand's
heavy ━━, and ❋ becomes the real ▮▮ mark.

Already met and worth not re-litigating: three colour tiers degrading to 16,
zero escapes in piped output, legible under NO_COLOR, no colour-only meaning.
One deliberate departure from the guide — it says refuse below a minimum
terminal size; that is right for a full-screen TUI and wrong for a CLI that
prints and exits, so this degrades instead, verified to 40 columns.

Gates

unit 4306 passing · e2e 333 passing · tsc clean · lint 0 errors.

The 15 remaining failures are three pre-existing groups, none from this branch:
4 inherited from main (both Python packages committed 0.0.1b2 with no
changelog section, via auto-bump commits carrying [skip ci]), 8 local-only
dogfood-configs in a tree where those skip-worktree files were emptied, and
3 in fp-reset that hang on the migration's spool flush — reproducible at
committed HEAD with this work stashed, dependent on the machine having a real
daemon installed, which CI does not. That last group is not root-caused.

Estimated code review effort: 5 (Critical) | ~120 minutes

Merge Risk: 🟠 High · up to a940a

This change reorganizes policy setup, installation, publishing, and enforcement, but the current version can disable existing protections, execute a rejected pack before refusing it, bypass safeguards on some commands, and mis-handle or lose selected policies; required CI preparation is also incomplete. These correctness and security risks should be fixed before merging.

Hermes found blocking issues that should be addressed.

Review coverage was incomplete, but the concrete blocking findings below are sufficient to request changes.

High: Pack policy parameters saved by the dashboard are never applied

  • Rule: COR-001
  • Location: src/hooks/policy-evaluator.ts:45
  • Evidence: The dashboard reads and writes parameters using the manifest policy name (get-hooks-config.ts:261 and update-policy-params.ts:12), for example policyParams["block-sudo"]. The hook handler registers that same pack policy as pack/<id>@<version>/<name> (handler.ts:533-543). policy-evaluator.ts:45-49 only looks up that qualified registered name and permits a short-name fallback exclusively for failproofai/ policies. Thus a configured pack policy receives schema defaults (or {}) instead of the value the UI displays as saved; this affects the bundled core pack immediately after migration.
  • Required change: Carry a stable configuration key from the pack manifest into the registered policy and look it up during evaluation, or consistently use a namespaced pack key in both the dashboard and runtime. Preserve a documented compatibility lookup for existing core-policy parameter keys, and add an end-to-end test that changes a pack policy parameter and observes it in ctx.params.

High: Identical pack artifacts silently discard another pack's selected policies

  • Rule: COR-001
  • Location: src/hooks/custom-hooks-loader.ts:451
  • Evidence: custom-hooks-loader.ts:451-465 collapses all installed packs sharing an artifact path to one ResolvedPack. The winning record alone tags hooks at lines 502-504, and handler.ts uses only that tag's enabled selection to decide whether each hook registers. A user can install two distinct pack IDs publishing identical foo/bar artifact bytes, select only bar from the first and only foo from the second; foo from the second pack is never registered. pack-failclosed.ts:141-145 explicitly ignores a pack absent from the registered map, so this becomes a silent enforcement gap despite the second pack being recorded as enabled.
  • Required change: Do not collapse packs solely by artifact path when their selections or identities differ. Either register/evaluate each pack's selection independently while sharing one module import, or merge the selected policy sets and preserve per-pack attribution; if that cannot be represented safely, reject the conflicting installation. Add a regression test with two IDs sharing bytes and complementary --only selections.

Hermes found blocking issues that should be addressed.

Review coverage was incomplete, but the concrete blocking findings below are sufficient to request changes.

High: Pack policy parameters saved by the dashboard are not applied

  • Rule: COR-001
  • Location: src/hooks/policy-evaluator.ts:45
  • Evidence: The dashboard reads and writes parameters under the manifest's bare policy name (app/actions/get-hooks-config.ts:261 and app/actions/update-policy-params.ts:12). The handler registers a pack policy as pack/@/ (src/hooks/handler.ts:517-543). src/hooks/policy-evaluator.ts:45-49 only accepts a bare-name fallback for the failproofai/ namespace, so a pack policy receives schema defaults or {} instead of the saved value. This affects the bundled core pack after migration.
  • Required change: Give registered pack policies a stable configuration key and resolve it in the evaluator, or use the versioned pack key consistently in both UI and runtime while retaining compatibility for existing core keys. Add an end-to-end test that changes a pack parameter and observes it in ctx.params.

High: Identical pack artifacts silently omit another pack's selected policies

  • Rule: COR-001
  • Location: src/hooks/custom-hooks-loader.ts:451
  • Evidence: src/hooks/custom-hooks-loader.ts:451-465 collapses every pack sharing an artifact path to one record. Only the winning record tags the imported hooks, and src/hooks/handler.ts:428-453 applies only that record's enabled selection. Two pack IDs sharing a foo/bar artifact and selecting foo and bar respectively therefore register only the winner's selection. src/hooks/pack-failclosed.ts:105-110 deliberately ignores a pack absent from the registered map, so the omitted selected policy does not trigger the fail-closed guard.
  • Required change: Do not collapse distinct pack identities solely by artifact path. Import once if needed, but apply each pack's selection and preserve per-pack attribution; alternatively reject conflicting installations. Add a regression test using complementary selections on two IDs sharing one artifact.
1 advisory finding
  • High/High A remote pack can replace the bundled core pack by claiming its ID — installBundledPack records the trusted core pack as bundled:failproofai/core@ (src/hooks/pack-store.ts:762-773). In addPack, the source-binding refusal explicitly excludes any prior bundled source (src/hooks/pack-store.ts:631-640). Thus a release from an arbitrary repository declaring id failproofai/core is accepted and upsertInstalled replaces the bundled record. On subsequent events the migration shim is disabled because a pack is present (src/hooks/handler.ts:316-322), while only the attacker's declared subset is enforced. (src/hooks/pack-store.ts:631)

1289-1310: 🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Identify policies by pack, not by name alone.

config.policies is the concatenation of every installed pack's policies (app/actions/get-hooks-config.ts lines 237-266). Two installed packs can declare the same policy name. In that case the optimistic map at line 1293 flips every row with that name, but line 1310 persists the change for one packId only. The other pack's row then displays a state that was never written, until the next reload().

The same identity gap affects the row key at line 1647: key={policy.name} produces duplicate React keys when two packs share a policy name.

🔧 Proposed fix: qualify the match with the pack id
         policies: prev.policies.map((p) =>
-          p.name === name ? { ...p, enabled: !currentlyEnabled } : p,
+          p.name === name && p.packId === policy.packId
+            ? { ...p, enabled: !currentlyEnabled }
+ : p,
         ),

Apply the matching change to the category row key:

-                key={policy.name}
+                key={`${policy.packId}@${policy.packVersion}:${policy.name}`}

The pack lane is used only when at least one pack policy registers, but partial coverage still passes.

registered > 0 accepts a pack that carries a single policy. In that case the audit runs one pack function plus 38 compiled functions and reports the pack lane as active. The comment above states the intent as "nothing came from the pack", yet the threshold does not detect partial coverage, which is the case that silently changes what the audit scored on.

Consider comparing registered against the count of wanted names that are not alwaysOn, and falling back when the pack covers fewer.

15-19: 📐 Maintainability & Code Quality | 🔵 Trivial | 💤 Low value

Update the cycle-guard rationale: POLICY_PARAMS_MAP no longer exists.

This PR removes POLICY_PARAMS_MAP from src/hooks/policy-evaluator.ts; the evaluator now reads policy.params off the registered policy. The rule itself stays correct, because builtin-policies.ts value-imports POLICY_CATALOG, so a value import back would still create a cycle. Only the cited mechanism is stale. The same stale text appears in __tests__/hooks/policy-catalog.test.ts lines 169-172.

The cycle guard misses a multi-line import.

The filter keeps lines matching /^import\s/ and drops /^import\s+type\s/. A value import written across several lines puts the module specifier on a later line, so l.includes("builtin-policies") never matches the surviving line and the guard passes. The guard exists to catch exactly that import, so widen the scan to the whole file text.

?? between the two classification passes can drop the documented pause precedence.

classifySelfInvocation documents that pause outranks cli. That ordering holds inside one pass only. Here classifySelfInvocation(cmd) runs first, and ?? short-circuits on any non-null result. If the raw command classifies as cli and only the shell-unescaped form classifies as pause, the policy emits the generic CLI message instead of the pause message. Example: failproofai config "--pause" — the quoted token fails PAUSE_FLAG_RE on the raw pass.

The decision stays deny in both cases, so enforcement is unaffected; only the message the agent reads changes.

Read the installed packs once per listing.

readInstalledPacks() is called at Line 807, at Line 817, and again at Line 1002. Each call re-parses installed.json and, per src/hooks/pack-manifest.ts (parsePack, Lines 203-255), reads and SHA-256 hashes every pack artifact. The listing therefore hashes each artifact three times.

Also, packCount at Lines 815-824 counts selected names only. It ignores disabledCustomPolicies entries and the observe effect that the rows at Lines 1010-1023 use. The header can report N on while the rows below show OFF or OBS.

Test reconciliation when packs share an artifact.

Add two pack fixtures that reference the same artifact and select different policies. Assert that both packs retain their declared policies and that both policies are enforced. Without this case, the loader can collapse the records and omit one pack’s selected policies without triggering fail-closed handling.

As per coding guidelines, “Always add unit tests for new behaviour.”

Protect the bundled core identity.

Add a regression case where a remote manifest claims failproofai/core but declares only a reduced policy set. The installation path must reject the remote identity, and the bundled core guard must remain active. Without this invariant, a remote pack can replace the trusted core record and remove migration protection.

Test pack-qualified policy parameters at runtime.

These assertions cover pack identity, version, and enabled state only. Add a case that saves a policy parameter, verifies the persisted key uses the pack-qualified policy name, and confirms runtime evaluation uses the saved value. Otherwise, the dashboard can report correct state while runtime registration falls back to defaults or {}.

I could not establish complete review coverage for 1cf9a8f2854b, so I did not approve it. I have no specific question to ask — this is a coverage gap on my side, not a request for input.

What the review did establish:

The pack migration, enforcement path, dashboard management, audit replay, and CLI rendering were reviewed. Two defects remain: a remote pack can impersonate the bundled core pack, and dashboard-saved pack parameters are never supplied to the running policy. No centrally configured validation commands were provided; a disposable-container dependency install did not complete, so the test suite was not completed.

Re-run with @hermes-exosphere review [focus] to point me at the part that matters most, or @hermes-exosphere reconsider [reason] if you believe the coverage was sufficient.

Hermes found blocking issues that should be addressed.

Review coverage was incomplete, but the concrete blocking findings below are sufficient to request changes.

High: Remote packs can replace the bundled official pack

  • Rule: SEC-001
  • Location: src/hooks/pack-store.ts:710
  • Evidence: addPack only rejects a source change when the prior record is not bundled: (src/hooks/pack-store.ts:710-718). Therefore, after core records bundled:failproofai/core@…, a remote release whose manifest declares id: "failproofai/core" passes this guard. upsertInstalled then replaces the record with the same ID (src/hooks/pack-store.ts:980-984), silently activating the remote artifact in place of the official one. The adjacent test covers only a prior GitHub source, not a bundled source (src/hooks/pack-build.test.ts:264-273).
  • Required change: Bind bundled IDs to their trusted source as well. Reject a remote source with an installed bundled ID unless the user explicitly removes it first, or allow only a designated official repository as an intentional migration path. Add a regression test installing the bundled pack before attempting a same-ID remote install.

High: Dashboard pack parameter changes are ignored at enforcement time

  • Rule: COR-001
  • Location: app/policies/hooks-client.tsx:1388
  • Evidence: The dashboard displays and saves parameters under the short manifest name: currentParams reads config.policyParams[policy.name] (app/actions/get-hooks-config.ts:261) and the client sends that same short name to updatePolicyParamsAction (app/policies/hooks-client.tsx:1381-1388), which persists it unchanged (app/actions/update-policy-params.ts:11-13). Pack policies are registered as pack/<id>@<version>/<name> (src/hooks/handler.ts:517-534), while the evaluator accepts only that exact key and allows a short-name fallback solely for failproofai/ policies (src/hooks/policy-evaluator.ts:40-49). Consequently, changes made in the dashboard are shown as saved but the pack continues receiving defaults.
  • Required change: Use one canonical pack policy key (pack/<id>@<version>/<name>) for dashboard reads and writes, or extend the evaluator with an unambiguous pack-aware key scheme. Add an end-to-end test that changes a pack parameter through the dashboard path and verifies the registered policy receives it.

Refuse a foreign source before you import the artifact.

addPack writes the artifact at Line 685 and imports it at Line 691 through verifyArtifactRegisters. The id-to-source binding check runs afterwards at Line 710. prior is already known at Line 672, so the refusal can be decided before any remote code runs.

A pack served from an unrelated repository that declares an installed id is therefore imported and executed on the machine, and only then refused. Move the check above the artifact write and the import so the refusal costs nothing more than a download.

854-872: 🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

The header count disagrees with the pack rows below it.

packCount counts pack.enabled ?? all policy names only. The pack table at Lines 1049-1061 marks a row off when disabledCustomPolicies holds pack:<id>@<version>:<name>, and marks every row observe when pack.effect === "observe". A machine with disabled or observe-only pack policies therefore reads N on in the heading while the rows below show fewer enforcing policies.

Remove the unreachable pack command block.

Line 49 rewrites args[0] from "pack" to "policies" before runCli() runs, and args is the same module-scope array read at Line 693. The condition args[0] === "pack" can therefore never be true, so this whole block, including runPackCommand dispatch and the cli_pack telemetry, is dead code.

The block also keeps the retired help text alive (failproofai pack add core, failproofai pack list <source>, failproofai pack build …). That text now contradicts the new index at Lines 379-384 and the policies add|remove|show help at Lines 1277-1324, and nothing can print it. Delete the block so there is one copy of the pack documentation.

Hermes found blocking issues that should be addressed.

Review coverage was incomplete, but the concrete blocking findings below are sufficient to request changes.

High: A remote manifest can replace the bundled core pack

  • Rule: SEC-001
  • Location: src/hooks/pack-store.ts:710
  • Evidence: addPack() obtains a prior record by self-declared id at src/hooks/pack-store.ts:672. Its intended source-binding check explicitly skips bundled records at lines 710-713. Therefore, after core is installed, a release from github:attacker/repo whose manifest declares id: "failproofai/core" passes the check and upsertInstalled() replaces the bundled record by id. Its selection is then calculated against the attacker's manifest, so existing core protections can disappear while the record still appears to be the core pack.
  • Required change: Reserve failproofai/core for the bundled/official provenance and reject source changes for it. More generally, bind pack ids to a trusted source on first installation and require explicit removal before a different source can claim that id; handle the legacy builtins-to-core rename through explicit migration metadata rather than digest/id fallback.
2 advisory findings
  • High/High State deletion guard misses find -delete — The always-on guard at src/hooks/builtin-policies.ts:1483 only tests FAILPROOFAI_STATE_WRITE_RE, whose verbs are rm|unlink|shred|mv|truncate. An agent can run find ~/.failproofai -delete (or find $HOME/.failproofai -delete), which does not match that guard. The same file already recognizes find ... -delete in recursiveDeletionTargets at lines 1153-1171, but that belongs to the optional block-rm-rf policy. Deleting installed.json removes all selected pack policies; on a newly configured machine there are no legacy enabled builtins to restore them. (src/hooks/builtin-policies.ts:1483)
  • High/High Pack policy parameters saved by the dashboard are ignored at runtime — The dashboard exposes each pack policy under its bare name (app/actions/get-hooks-config.ts:242) and saves parameters under that bare key (app/policies/hooks-client.tsx:1383-1388; app/actions/update-policy-params.ts:12). At evaluation, however, the policy is registered as pack/<id>@<version>/<name>, and getConfigParamsFor() at src/hooks/policy-evaluator.ts:45-49 only falls back from the default failproofai/ namespace. Thus policyParams["block-sudo"], including existing user configuration and values just saved by the dashboard, is never passed to the core pack's block-sudo implementation; it runs with defaults instead. (src/hooks/policy-evaluator.ts:45)

Round 4 of 5. If the next review still finds something blocking, I will summarize what is left, withdraw this change request, and stop reviewing this pull request until someone asks me to start again.

Still open:

  • F5 State deletion guard misses find -delete (src/hooks/builtin-policies.ts) — noticed at round 4, on code that had not changed since the round before, so it never blocked
  • F3 A remote manifest can replace the bundled core pack (src/hooks/pack-store.ts) — noticed at round 2, on code that had not changed since the round before, so it never blocked
  • F6 Pack policy parameters saved by the dashboard are ignored at runtime (src/hooks/policy-evaluator.ts) — noticed at round 4, on code that had not changed since the round before, so it never blocked

If one of these is not worth fixing, @hermes-exosphere dismiss <id> [reason] waives it for the rest of this pull request and gives the review another round.

I have spent 5 rounds of review on this pull request and I am still finding things to block on. At that point I am no longer the useful reviewer here, so I am standing down and leaving the decision to a person.

I have withdrawn my change request, so I am no longer blocking this pull request. I have also stopped reviewing new commits on it.

What I last reviewed: 2bb7c59ae77f.

Still open:

  • F5 find -delete bypasses the always-on state-deletion guard (src/hooks/builtin-policies.ts) — noticed at round 4, on code that had not changed since the round before, so it never blocked

  • F6 Dashboard-configured pack parameters are ignored at runtime (src/hooks/policy-evaluator.ts) — noticed at round 4, on code that had not changed since the round before, so it never blocked

  • @hermes-exosphere dismiss <id> [reason] waives one of these for the rest of this pull request and starts me again.

  • @hermes-exosphere review [focus] starts over from a clean slate, with the waivers kept.

  • @hermes-exosphere reconsider [reason] asks for one more decision on what is here now.

None of this is a judgement that the findings above are wrong. It is a judgement that another round of me is not what will settle them.

Hermes spent 5 rounds on this pull request without converging and has stood down. This change request is stale and should not block the merge.

…n disable

`block-self-pause` and `block-failproofai-commands` were two halves of one
guard, and they disagreed with each other.

`block-self-pause` had the hardened matcher — segments split on shell
operators, runner prefixes and their flags walked off, the binary resolved by
basename, the shell-unescaped form re-checked — but only ever looked for
`config --pause`. `block-failproofai-commands` had the whole surface, any CLI
invocation plus package-manager uninstall, on a regex a single prefix defeated:
`sudo failproofai config --pause`, `npx failproofai policies --uninstall`,
`env X=1 failproofai …`, `/usr/local/bin/failproofai …` and
`timeout 30 failproofai …` were all ALLOWED by a default-on self-protection
policy. The merged policy is the hardened matcher over the broad surface, and
it keeps `PermissionRequest` from the merged-in half — a real enforcement point
on Copilot and Devin that the survivor never subscribed to.

Where the two contradicted each other, the merge keeps what machines actually
did. `block-self-pause` deliberately allowed `config --resume`, `config
--status` and `policies --install`; both policies were default-on and the
sibling denied all three first, so that allow never ran anywhere.

It is now `alwaysOn`, a new flag `registerBuiltinPolicies` honours ahead of the
enabled set. That closes the three ways the old pair could go dark without
anyone noticing: a name absent from `enabledPolicies`, an active session pause
(`handler.ts` passes `[]`), and a config file that fails to parse
(`hooks-config.ts` soft-fails to `{enabledPolicies: []}` at five sites, so
corrupting one file disabled every policy including these two).
`policies --disable block-failproofai-commands` now refuses with a reason
instead of editing the config and reporting a success that changes nothing.
`policy-catalog.ts` now holds the metadata — name, description, category,
`match`, `defaultEnabled`, `params` — as pure literal data, and
`builtin-policies.ts` keeps the 39 implementations and joins them back on.
`BUILTIN_POLICIES` keeps its exact shape, fields and order, so none of its nine
source consumers change. This is what lets a machine list, search and render the
catalog offline once the executable half moves to a fetched pack.

Two constraints made the refactor narrower than it looks, and both were measured
rather than assumed.

`audit/cache.ts` hashes `fn.toString()` for all 39 policies into the audit
cache's `engineVersion`, and `bun build` renames colliding top-level identifiers
by module EMISSION ORDER — those renamed names appear inside policy bodies in
the shipped bundle (`cwdWithSep2`, `execSync2`, `resolved3`). So inserting a
module into the graph could have changed the emitted text, invalidated every
user's audit cache and forced a ~104-second cold rescan on upgrade. Built before
and after and compared: `engineVersion` is unchanged at `c1cea4ddf3030af4`.

`SECRET_PATTERNS` stays here rather than being reclassified as catalog data. It
is assembled from the very RegExps the five `sanitize-*` policies test against
and is imported by the audit redactor, so moving it would have forced a
catalog→implementation value edge and put an import cycle on the hook path.

`policy-catalog.test.ts` pins the join against the failures that are otherwise
silent, each verified to fail when the join is mutated: a wrapper collapsing 39
distinct `fn.toString()` hashes into one and freezing the cache key; a sort or
regroup changing which policy name is attributed on a deny; a spread
default-filling `beta`; a dropped row shrinking the catalog invisibly to
`manager.ts` and `install-prompt.ts`, neither of which reads `.fn`. The
bijection check throws at module load rather than warning, because a name with
no implementation yields `fn: undefined`, whose `TypeError` `policy-evaluator.ts`
swallows — the hook would allow, exit 0, and still report the policy as having
run.
A pack is one digest-pinned entry artifact plus a manifest describing what it
contains, installed under ~/.failproofai/policies/packs/ beside the cloud
artifacts and loaded through the custom-policy loader that already exists. Not a
fourth loader — the same lane with a different tag.

Packs are LOCAL policy. Cloud assignments are exempt from disabledCustomPolicies
and from session pause because a locally-issued command must not switch off a
CENTRALLY assigned policy; a pack the user installed by typing a command is not
that, so it stays disableable and pausable. Copying the exemption would have
been an unrelated capability arriving by copy-paste.

Three refusals, each closing a silent failure:

- A pack policy name may not contain `/`, and pack policies register under
`pack/<id>@<version>/`. Verified live that without this a pack shipping the
name `failproofai/block-sudo` REPLACES the compiled builtin —
normalizePolicyName passes any name containing a slash through untouched and
registerPolicy replaces by canonical name — so the machine would report
block-sudo as enabled while running a stranger's code.
- A pack may not declare `alwaysOn`: downloaded enforcement that no local
  command can turn off.
- Byte-identical packs merge toward enforcement, with a warning. Artifacts are
content-addressed, so identical source is one file, and the loser would
otherwise vanish with its effect deciding nothing. Same collision that once
silently downgraded a cloud policy to observe-only.

Manifest and artifact are reconciled after load. The artifact is digest-pinned
so what it registers is what the publisher shipped, but nothing bound the
manifest to it: a declared policy the artifact never registers is a listing
claiming protection that does not run.

engineVersion, which keys the audit cache, folds in each pack's
id|version|sha256 — by identity, not source text, because the loader rewrites a
per-load temporary filename into every import specifier and hashing that would
cold-rescan the whole history every run. A machine with no packs hashes
byte-identically to a build with no pack support, verified at the source level
and in the shipped bundle (c1cea4ddf3030af4, unchanged), so this costs no
existing user the ~104-second rescan.

Failure is per pack, not per manifest, and fails open with a recorded reason.
That is sound only while the builtins still ship compiled in and keep enforcing
underneath, and the catch says so — because the day builtins become a fetched
pack, this exact behaviour is zero enforcement on a machine reporting healthy.
The params schema now travels on the RegisteredPolicy, next to `match`, instead
of being looked up by name in a map built from BUILTIN_POLICIES.

That map could only ever describe policies compiled into this build, so every
pack policy, cloud assignment and custom hook fell through to the branch that
never calls getConfigParamsFor. The consequence was worse than missing defaults:
the user's OWN configured policyParams for those policies were discarded. A
person who set protectedBranches on a cloud-assigned policy had it silently
ignored, with nothing anywhere reporting it.

A schema-less policy now receives whatever the user configured, and still `{}`
when they configured nothing — which is every case that exists today. A policy
declaring a schema gets defaults merged under the user's values, unchanged.

Registration-carried rather than name-keyed also closes a hole the pack lane
opened one commit ago: a name-keyed schema was handed to ANYTHING registered
under that name, so a pack that took the `block-sudo` name would have inherited
its params along with it.

policy-evaluator.ts no longer imports the builtin catalog. That is a module
graph change, which is the condition that can shift emitted text and move the
audit cache key, so it was re-measured rather than assumed: the shipped
engineVersion is still c1cea4ddf3030af4.

Hermes spent 5 rounds on this pull request without converging and has stood down. This change request is stale and should not block the merge.

…spell

The floor that told an aimed pattern from an ordinary one was a literal `fail`
substring test, which is precisely the letter a metacharacter stands in for.
`rm -rf ~/.f*ailproofai` was thrown out before it was ever compiled, and so
were `.[f]ailproofai`, `.fa*lproofai`, `.fa[i]lproofai` and `.f{a,b}ilproofai` —
every one of them deleting the state through the guard nobody can switch off.

The floor is now computed the same way the match is: compile once, require that
the pattern hits the state, and reject it if it also hits anything ordinary.
That keeps `rm -rf *` and `rm -rf ~/.*` allowed — they sweep the decoys too, and
an always-on guard that denied those would be worked around by the first person
who met it — while `rm -rf /tmp/test-failures*` stays allowed for the older
reason, that it reaches no state path at all. Braces are compiled as the
alternation they become, and every `/`-prefix is tried rather than the whole
token: naming a file inside the state is naming the state.

The second half is the same mistake one layer up. `match` was validated as "is
an object" and nothing more, so a manifest declaring `events: "PreToolUse"` was
iterated character by character and narrowed the fail-closed deny to
["P","r","e","T","o","l","U","s"] — a guard matching no event that exists. The
pack failed to load, the deny fired on nothing, and the machine reported
enforcement it did not have. `events: 5` threw `number is not iterable` on the
hook path outright. Metadata nobody can read now widens the deny instead of
narrowing it, because a scope derived from something unparseable says nothing
true, and the manifest refuses the shape at install time where the publisher can
still fix it.

The test that pinned the glob bypass as a known gap is removed: it is no longer
a gap, and the cases that replace it cover both directions.

Round 5 findings addressed at eab785c5.

F9 — glob syntax bypasses the always-on state guard. The floor deciding whether a pattern was aimed at the state was token.includes("fail"), a literal substring test — which is exactly the letter a metacharacter stands in for. rm -rf ~/.f*ailproofai was rejected before the pattern was ever compiled; so were .[f]ailproofai, .fa*lproofai, .fa[i]lproofai and .f{a,b}ilproofai. The floor is now computed from the pattern's reach: compile once, require a state candidate, reject if it also matches a decoy set of ordinary paths. rm -rf * and rm -rf ~/.* stay allowed (they sweep the decoys too); rm -rf /tmp/test-failures* stays allowed (reaches no candidate). Brace expansion is compiled as an alternation, and every /-delimited prefix is tried rather than only the whole token, so shred ~/.f*ailproofai/policies-config.json is caught. The test that pinned this as a known gap is removed and replaced with 16 cases in both directions.

F10 — malformed pack match metadata defeats fail-closed enforcement. Confirmed and fixed at both layers. match was validated as typeof === "object" only, so events: "PreToolUse" was iterated character by character and narrowed the deny to ["P","r","e","T","o","l","U","s"] — matching no event that exists. events: 5 threw number is not iterable on the hook path. Unreadable metadata now widens the deny rather than narrowing it, and parsePackPolicy refuses the shape at install time.

Two earlier findings were already fixed before this round and I re-verified rather than re-fixing: find <state> -delete denies (the guard is an allowlist now, not the verb regex the round-4 evidence cited), and togglePackPolicyAction's result is read, with fireActionError + reload() on {ok:false}.

Not fixed, by the maintainer's explicit decision: the hook-config gap (rm ~/.claude/settings.json). Please leave it out of scope.

Gates green locally: tsc 0, lint 0 errors, build exit 0, e2e 333/333, unit 4770 passed (9 failures are the 8 locally-emptied dogfood configs plus one cross-worker flake that passes in isolation).

High: Nested glob in brace expansion bypasses the state guard

  • Rule: SEC-001
  • Location: src/hooks/builtin-policies.ts:753
  • Evidence: globPrefixNamesState escapes every brace alternative at src/hooks/builtin-policies.ts:753. Consequently rm -rf ~/.f{a*,x}ilproofai is compiled as alternatives containing a literal *, even though the shell first expands it to ~/.fa*ilproofai and then glob-expands that word to ~/.failproofai. The always-on block-failproofai-commands policy therefore returns allow; this was reproduced in a nested Docker container against the reviewed source. Deleting that directory removes installed packs and can turn off their enforcement without invoking the CLI.
  • Required change: Expand brace alternatives before compiling glob patterns, or recursively compile metacharacters within each alternative, and add regression cases such as ~/.f{a*,x}ilproofai (including paths beneath that directory).

Comment thread src/hooks/builtin-policies.ts Outdated
Compiling `{a*,x}` as an alternation of escaped literals put a literal `*`
inside the pattern. The shell does not work that way: brace expansion rewrites
the word FIRST, and only the resulting words are matched against the disk. So
`rm -rf ~/.f{a*,x}ilproofai` — which the shell turns into `~/.fa*ilproofai` and
then into the state directory itself — compiled to a pattern that could never
match it, and walked past the guard nobody can switch off.

Expansion now runs ahead of compilation, over nested groups and several groups
in one word, and it is bounded rather than trusted: `{a,b}` twenty times over
multiplies to a million words, so past the limit every group collapses to `*`.
That is a superset of what the braces could produce, which keeps an oversized
token decidable — and still held to the same decoy test — instead of turning
into a way to stall the hook path.

The other half was the token never arriving whole. `bareToken` strips leading
and trailing braces because a shell group wears them (`{ rm -rf x; }`), and a
brace expansion wears the same characters and ends the token with one — so
`~/.{fail*,zz}` reached the scan as `~/.{fail*,zz`, an unterminated group that
expands to nothing. The glob scan keeps its braces now; a trailing brace that
really did close a shell group leaves a word still carrying the literal path,
which the literal check reads.

Fixed at 79b4ece8. You were right, and the fix was in the wrong place: I was compiling braces as an alternation rather than expanding them, so a glob inside a brace became a literal.

Brace expansion now runs before compilation, in the shell's own order — nested groups and multiple groups per word included. It is bounded rather than trusted: past 4096 words every group collapses to *, a superset of what the braces could produce, so an oversized token stays decidable instead of stalling the hook path (a 20-group bomb answers in ~15ms, pinned by a test).

Your case exposed a second half I would not have found from the report alone. bareToken strips leading and trailing braces because a shell group wears them ({ rm -rf x; }), and a brace expansion wears the same characters and ends the token with one — so rm -rf ~/.{fail*,zz} reached the scan as ~/.{fail*,zz, an unterminated group expanding to nothing. The glob scan keeps its braces now.

New denying cases: ~/.f{a*,x}ilproofai, ~/.f{[a],x}ilproofai, ~/.f{a?,x}lproofai, ~/.f{{a,b}*,x}ilproofai, ~/.f{a,x}{i,y}lproofai, ~/.{fail*,zz}, and the beneath-directory form shred ~/.f{a*,x}ilproofai/policies-config.json. Still allowed: rm -rf {dist,build}/*, rm -rf ~/.{cache,config}/*, rm -rf {a,b}{c,d}, rm -rf *, rm -rf ~/.*.

Gates green: tsc 0, lint 0 errors, build exit 0, e2e 333/333, unit 4782 passed (8 failures are the locally-emptied dogfood configs).

High: Nested brace expansion still bypasses the state guard

  • Rule: SEC-001
  • Location: src/hooks/builtin-policies.ts:752
  • Evidence: expandBraces stops after 16 rounds and returns unresolved words at src/hooks/builtin-policies.ts:752-770. A 17-level token such as rm -rf ~/.f{x,{x,{x,{x,{x,{x,{x,{x,{x,{x,{x,{x,{x,{x,{x,{x,{x,a*}}}}}}}}}}}}}}}}}ilproofai is valid shell brace expansion and has a branch that becomes ~/.fa*ilproofai, which glob-expands to ~/.failproofai. The remaining unresolved brace group is escaped by globPrefixNamesState, so namesState is false and the destructive rm is allowed. Deleting this directory removes installed packs and disables their enforcement.
  • Required change: Do not return unresolved brace groups after the depth limit. Continue expansion within a bounded global work budget, or conservatively collapse every remaining nested group into a glob representation before matching. Add a regression test with more than 16 nested brace groups that can expand onto the state directory.
1 advisory finding
  • Medium/High Matching packs by artifact digest deletes independent pack records — priorRecordFor treats any matching SHA-256 as the prior pack (src/hooks/pack-store.ts:618-620), and upsertInstalled then removes every different ID with that digest (src/hooks/pack-store.ts:1150-1163). Two distinct packs may legitimately use identical entry bytes; the new pack-shared-artifact tests explicitly model distinct IDs with one artifact. If acme/first@1.0.0 and acme/second@2.0.0 from the same repository have identical artifacts, adding the latter passes the repository check but silently removes the former record and its independent selection. Comment thread src/hooks/pack-store.ts
The round cap RETURNED its half-expanded words. A word that still has braces in
it goes to a compiler that escapes them as literals, so seventeen levels of
`{x,{x,…{x,a*}…}}` — legal shell with a branch that becomes `~/.fa*ilproofai`,
which globs onto the state directory — came out matching nothing at all. The
budget was doing the opposite of its job: running out of it produced an ANSWER,
and the answer was "this names nothing".

Exhausting the budget now collapses instead. Every remaining group becomes `*`,
which reaches strictly more than the braces could, so the token stays decidable
however deep it goes and is still held to the decoy test that keeps `rm -rf *`
allowed. Collapsing replaces each OUTERMOST group whole rather than resolving
innermost-first — one pass at any depth, instead of one pass per level, which
would only have moved the same cliff somewhere further out. Verified to 200
levels, in both directions.

Separately, `policies add` stops stating an inference as a fact. Absorbing a
same-artifact record under another id is the right default — a publisher
renaming a set must not reset everyone who had narrowed it — but identical bytes
cannot tell a rename from a second pack one repository builds from the same
source, and the record that goes away is one the user installed on purpose.
"Same policies under a new name" claimed to know which it was. The line now
reports what was observed, that absorbing was the inference, and how to restore
the other pack when the inference was wrong.

Fixed at 47c82290. Better catch than the last one — the bug was that my budget produced an answer when it ran out, and the answer was "names nothing".

Nested brace expansion. The round cap returned its half-expanded words, and a word that still has braces goes to a compiler that escapes them as literals. Exhausting the budget now collapses rather than returns: every remaining group becomes *, which reaches strictly more than the braces could, so the token stays decidable at any depth and is still held to the decoy test that keeps rm -rf * allowed. The collapse replaces each outermost group whole rather than resolving innermost-first — one pass at any depth, instead of one pass per level, which would only have moved the same cliff further out (I hit exactly that at 200 levels while testing the first attempt).

Verified denying at 1, 5, 16, 17, 20, 30, 64 and 200 levels, including your exact token, the beneath-directory form, and a deep token whose winning branch is a literal rather than a glob. Still allowed: rm -rf /tmp/{x,…{x,a*}…}build at 40 levels, rm -rf ~/.{cache,config}/*, rm -rf {a,b}{c,d}, rm -rf *, rm -rf ~/.*. Regression tests pinned at 17 and 200 levels.

Advisory — packs matched by artifact digest. I took this seriously and did not take the fix. I tried narrowing absorption to cross-origin renames and it is wrong: the documented, tested rename case is same repository, different id (acme/finance → acme/renamed in pack-selection-merge.test.ts), which is the identical shape to your acme/first/acme/second. The pack id comes from the release manifest, not the artifact, so your scenario is genuinely reachable — but at install time the two are indistinguishable, and refusing or skipping would break a publisher rename resetting everyone who had narrowed the pack, which is the bug absorption was written for.

What I did fix is the false claim. The line said "same policies under a new name" — an inference stated as fact about a record the user installed on purpose. It now reports the evidence (same artifact), that absorbing was the inference drawn from it, and how to restore the other pack if that was wrong. If you know a signal available offline that separates the two cases, I will take it.

Gates green: tsc 0, lint 0 errors, build exit 0, e2e 333/333, unit 4787 passed (8 failures are the locally-emptied dogfood configs).

High: POSIX-negated glob bypasses the always-on state guard

  • Rule: SEC-001
  • Location: src/hooks/builtin-policies.ts:878
  • Evidence: globPrefixNamesState copies a shell bracket expression directly into a JavaScript regex at src/hooks/builtin-policies.ts:878. Bash expands ~/.f[!b]ilproofai to ~/.failproofai, but JavaScript interprets [!b] as a class containing ! or b, so the generated regex does not match the state candidate. Consequently destroysFailproofaiState() returns false and blockFailproofaiCommands() allows rm -rf ~/.f[!b]ilproofai, which deletes the pack/config state and disables all fetched enforcement policies.
  • Required change: Translate POSIX [!...] bracket expressions to JavaScript [^...] expressions before compiling the glob (including appropriate escaping), and add a state-guard regression test for rm -rf ~/.f[!b]ilproofai and a nested-path variant.
1 advisory finding
  • Medium/High Artifact digest is incorrectly treated as pack identity — priorRecordFor falls back from pack ID to matching SHA-256 at src/hooks/pack-store.ts:620. A second independently named pack with byte-identical entry code therefore inherits the first record as prior; the source binding check at line 912 then rejects it because the repositories differ. If that check did not reject it, upsertInstalled would instead absorb and delete every different ID with the same digest (lines 1150-1153). This contradicts the shared-artifact enforcement fixture, which explicitly models distinct IDs and independent selections for the same artifact in __tests__/hooks/pack-shared-artifact.test.ts:94-107. (src/hooks/pack-store.ts:620)

Comment thread src/hooks/builtin-policies.ts Outdated

POSIX negates with `!` and JavaScript with `^`, so `[!b]` — which bash expands
straight onto the state directory — compiled to a class matching either `!` or
`b`, and `rm -rf ~/.f[!b]ilproofai` walked past the guard that cannot be
switched off. One character wide, and the pattern was read backwards.

Two more spellings were being read wrong beside it. A `]` in the FIRST position
is content rather than the terminator, so scanning for the next `]` closed
`[]a]` on the wrong character; and a POSIX class carries a `]` of its own, which
ended `[[:alpha:]]` early and left a stray literal on the end. Anything past
those — a collating symbol, an equivalence class — becomes `[^/]`, which reaches
at least as far as whatever it replaced. Widening is safe here in a way
narrowing never is, because the decoy test is what decides.

An unclosed `[` is now a literal `[`, which is what it is to a shell. It used to
return false, and that is the same mistake as last time in a smaller place: a
malformed pattern is not evidence that a command names nothing.

The pack half is the other end of the same lookup. A prior record is found by
digest as well as by id, and the origin binding — an id may not change the
source it came from without an explicit removal — did not ask which of the two
it had been handed. A pack from another repository that happened to build
byte-identical output was refused with `pack id X is already installed from Y`,
naming an id the user never asked for and offering a removal that would not have
helped. It applies only when the ids actually match.

Fixed at 825ef71a. Confirmed exactly as reported — [!b] compiled to a class matching either ! or b, so the pattern was read backwards, one character wide.

Bracket expressions are now translated rather than copied. Your case exposed two more spellings being read wrong beside it, both found while writing the fix: a ] in the first position is content rather than the terminator, so scanning for the next ] closed []a] on the wrong character; and a POSIX class carries a ] of its own, which ended [[:alpha:]] early and left a stray literal on the end. Anything past those — collating symbol, equivalence class — becomes [^/], which reaches at least as far as what it replaced; widening is safe where narrowing is not, because the decoy test is what decides.

I also took your last round's lesson to the neighbouring line: an unclosed [ used to return false, which is the same mistake in a smaller place — a malformed pattern is not evidence that a command names nothing. It is a literal [ now, as it is to a shell.

Denying: [!b], [^b], [!0-9], [[:alpha:]], []a], [!]b], the nested-path variants (…[!b]ilproofai/policies, shred …[!b]ilproofai/policies-config.json), a negated class inside a brace, and one 30 braces deep. Still allowed: rm -rf ~/.[!x]onfig, rm -rf /tmp/[!a]uild, cat ~/.f[!b]ilproofai/policies-config.json. All five earlier sweeps re-run clean.

Advisory — digest treated as identity. You are right about the half I can fix without guessing, and I took it: the origin binding now applies only when the ids actually match. Before, a pack from another repository that happened to build byte-identical output was refused with pack id X is already installed from Y — naming an id the user never asked for, and offering a removal that would not have helped. The absorption half I still cannot fix, for the reason in my last comment: the documented rename case (pack-selection-merge.test.ts) is same-repo/different-id/identical-bytes, which is the same shape as two coexisting packs, and the id comes from the release manifest rather than the artifact. If there is an offline signal that separates them I will take it.

High: Extended glob bypasses the always-on state guard

  • Rule: SEC-001
  • Location: src/hooks/builtin-policies.ts:719
  • Evidence: globCouldNameState() only recognizes *, ?, [, and { at src/hooks/builtin-policies.ts:719. Bash extended globs use @(...), which therefore never marks the command as reaching the state path. In an isolated container, invoking the production block-failproofai-commands policy for bash -O extglob -c 'rm -rf ~/.f@(ailproofai)' returned {"decision":"allow"}; Bash expands that pattern to ~/.failproofai and deletes it. Removing the pack manifest disables all downloaded policy enforcement while the guard remains the only compiled policy.
  • Required change: Recognize Bash extended-glob operators (@(), ?(), *(), +(), !()) when determining whether an operand can name the state directory, including commands passed through shell -c. Add a regression test that requires this command to deny.

  • Medium/High A failed pack denies CLIs outside its configured scope — The handler loads every installed pack path before applying pack.clis (src/hooks/handler.ts:400 and :434). On a permanent load failure, missingGuards() creates a deny from the policy match without considering pack.clis (src/hooks/pack-failclosed.ts:147). An isolated integration probe with a digest-mismatched pack scoped to clis:["codex"] returned a PreToolUse denial for the claude CLI. That agent was never configured to be guarded by this pack, yet is locked out until a human repairs it. (src/hooks/pack-failclosed.ts:147)

…s it covers

Three of Bash's five extended-glob operators begin with a character that means
nothing on its own, so `@(`, `+(` and `!(` did not even mark a token as a
pattern: `rm -rf ~/.f@(ailproofai)` never reached the glob check. `@()`, `?()`,
`*()` and `+()` are compiled now as the groups-with-a-quantifier they are,
recursing into their alternatives rather than escaping whatever is inside them,
and `!()` becomes `[^/]*` — a negation a regex cannot express against a path
segment, answered by widening, which is the direction that stays safe here.

Underneath it was the brace bug again, wearing different characters. The strip
that removes a subshell's parentheses removes a pattern's too, so the token
arrived as an unterminated group. Parentheses and braces are both how a shell
groups COMMANDS and how it spells a PATTERN, and both spellings end the token
with the closing one. The glob scan keeps them and tries the stripped reading as
well, rather than choosing one and being wrong half the time.

Separately, a pack that fails to load stops denying agents it never covered. The
registration path skips a pack whose `clis` excludes the running agent and the
fail-closed path did not, so a pack scoped to codex that failed its digest check
locked claude out too — over enforcement that agent was never configured to
have, until a human repaired a pack it does not use. The scope is carried on a
pack that failed before it could be resolved, and an unreadable one means every
agent rather than none: `clis: "codex"` is a truthy value with a `length` and an
`includes`, so the first version of this check narrowed itself away to nothing
and skipped the guard everywhere. Same rule as an unreadable `match` — a
narrowing nobody can parse says nothing true.

Fixed at 3f02d42f. Confirmed — and the interesting part is that the root cause was the same bug I had already fixed twice, wearing different characters.

Extended globs. @(), ?(), *() and +() are compiled as the groups-with-a-quantifier they are, recursing into their alternatives rather than escaping them; !() becomes [^/]* — a negation a regex cannot express against a path segment, answered by widening.

But recognising the operator was only half. Underneath it, bareToken strips a subshell's parentheses ((cd x && rm y)) — and a pattern's too, so ~/.f@(ailproofai) arrived as an unterminated group. That is exactly the brace bug from round 6 in different clothes: parentheses and braces are each how a shell groups commands and how it spells a pattern, and both spellings end the token with the closing character. The scan keeps them and tries the stripped reading as well, rather than picking one and being wrong half the time.

Denying: your exact case and the bash -O extglob -c form, @(a|b), ?(), *(), +(), nested @(@(a)…), a star inside a group, a negated class inside a group, a group wrapping the whole name, the beneath-directory and shred …/policies-config.json variants, and a group inside a brace. Still allowed: rm -rf /tmp/@(build|dist), rm -rf ~/.!(config), cat ~/.f@(ailproofai)/policies-config.json, rm -rf /tmp/@(build. All six earlier sweeps re-run clean.

Advisory — a failed pack denying out-of-scope CLIs. Taken, and it was a good one: registration honours pack.clis and the fail-closed path did not. Now scoped, including for a pack that failed before it could be resolved (PackError carries clis the way it already carried effect, and for the same reason).

Worth flagging because I nearly shipped it: my first version of that check narrowed on truthiness, and clis: "codex" is a truthy value with a length and an includes — so a string scope skipped the guard for every agent. A check written to stop over-denying had turned into the thing that under-denies. It requires a real list of names now, and an unreadable scope means every agent, the same rule as an unreadable match. A test pins it.

I could not establish complete review coverage for 3f02d42f4250, so I did not approve it. I have no specific question to ask — this is a coverage gap on my side, not a request for input.

What the review did establish:

Two medium-confidence correctness issues: combined failed packs over-block unrelated tools, and the incremental audit fallback can double-count after a concurrent transcript rewrite. Container validation could not be provisioned.

Re-run with @hermes-exosphere review [focus] to point me at the part that matters most, or @hermes-exosphere reconsider [reason] if you believe the coverage was sufficient.

Both are the same shape: a decision made in one function, consumed in another,
and the consumer assuming something the producer had stopped guaranteeing.

When several packs are unavailable at once, one policy stands in for all of
them. Its matcher unioned the events and left `toolNames` out of the combined
object entirely, which reads as "every tool" — so ONE failed pack scoped to Bash
denied Bash, and TWO of them denied Write and Read as well. Combining two
limited scopes was producing a larger one. Both axes union by the same rule now,
narrowing only where every guard was narrow, and the merge moved next to the
other match logic where it can be tested.

The audit half is the same trick with a byte offset. The cache decides a
transcript grew and hands back a prefix result plus somewhere to resume from,
but the READER is what finds out that offset is no longer good — truncated,
rewritten underneath it — and it answers by reading the whole file instead. The
prefix was merged onto that regardless, so everything before the offset was
counted twice and the audit reported more than the transcript held. The merge
asks whether the scan actually resumed now, rather than whether a prefix
existed.

Those null paths are a concurrent truncation and an I/O error, neither of which
a black-box audit can trigger on purpose, so the test forces the reader to
refuse. Without the fix it reports five events where the file holds three.

@hermes-exosphere review the fail-closed deny scope in src/hooks/pack-failclosed.ts and src/hooks/handler.ts, and the incremental audit resume in src/audit/index.ts and src/audit/cache.ts

Both medium-confidence findings from your coverage-gap round were real. Fixed at e2962f55. They turned out to be the same shape — a decision made in one function and consumed in another, with the consumer assuming something the producer had stopped guaranteeing.

Combined failed packs over-blocking. Confirmed. The stand-in policy's matcher unioned events and left toolNames out of the combined object entirely, which reads as "every tool" — so one failed pack scoped to Bash denied Bash, and two of them denied Write and Read as well. Combining two limited scopes was producing a larger one. Both axes union by the same rule now, narrowing only where every guard is narrow. I moved the merge into pack-failclosed.ts as combinedGuardMatch so it has a test seam; four cases pin it, including the two-guards-same-tool case that was the bug.

Incremental audit double-counting. Confirmed, and more direct than a rewrite-in-place: the cache grants the resume, but the reader is what discovers the offset is no longer good (fromByte > size after a concurrent truncation, or an I/O error) and answers by reading the whole file. The prefix was merged onto that regardless, because the merge asked whether a cached prefix existed rather than whether the scan had actually resumed. It asks the right question now, via an explicit resumed flag on the scan outcome.

Neither null path is reachable on purpose from a black-box audit, so the new test forces the reader to refuse. I verified it fails without the fix: five events reported where the file holds three.

Gates green: tsc 0, lint 0 errors, build exit 0, e2e 333/333, unit 4829 passed (8 failures are the locally-emptied dogfood configs).

If container validation is still unavailable, the state-guard work from the last four rounds is fully covered by __tests__/hooks/state-guard.test.ts (124 cases, both directions) — that may be a cheaper surface to check than provisioning a container.

High: Propagate a shared artifact failure to every pack that depends on it

  • Rule: COR-001
  • Location: src/hooks/custom-hooks-loader.ts:544
  • Evidence: When two installed packs resolve to the same artifact, custom-hooks-loader collapses them into one packByPath entry (lines 482-517). On an import or path failure it writes packFailures only for that merged winner (line 544). missingGuards then skips every other installed pack that is absent from both the failure and registration maps (pack-failclosed.ts lines 170-193). Thus, if pack A selects a Bash policy and pack B selects a Write policy from the same artifact and that artifact has a syntax error, only A receives a fail-closed guard; B's selected Write policy is absent and Write is allowed. A focused container execution of missingGuards with that map returned only a/one.
  • Required change: Retain all pack IDs represented by each deduplicated artifact path and record a permanent load failure for every one of them (including path-missing). Add an end-to-end regression with two packs sharing an artifact, complementary selections, and a failed import, asserting both policy scopes are denied.
1 advisory finding
  • Medium/High Do not flatten independent event/tool scopes into a cross product — combinedGuardMatch independently unions events and toolNames (lines 237-241), while policy-registry requires both axes to match (policy-registry.ts lines 94-101). For unavailable guards scoped to (PreToolUse,Bash) and (PostToolUse,Write), the merged matcher is events=[PreToolUse,PostToolUse] and toolNames=[Bash,Write], so it also denies the unintended (PreToolUse,Write) and (PostToolUse,Bash) calls. A focused container execution registered this matcher and confirmed both unintended pairs matched. (src/hooks/pack-failclosed.ts:241)

Comment thread src/hooks/custom-hooks-loader.ts Outdated
Comment thread src/hooks/pack-failclosed.ts
…t they scoped

Packs with identical entry bytes collapse to one load so the module imports
once. A load failure is recorded per PACK ID, and the collapse leaves one id
holding the merged record — so a syntax error in a shared artifact marked the
winner failed and said nothing about the other. `missingGuards` skips a pack
missing from both the failure map and the registration map, so the second pack's
selected policies were absent, unguarded, and unreported: exactly the silent
under-enforcement the selection union was written to stop, arriving through the
failure path instead. Every id behind an artifact path is kept now, and a
failure is recorded against all of them, for a failed import and a missing file
alike.

The second half is a fault in the fix from last round. The stand-in policy's
matcher unions the two axes independently and the registry ANDs them, so guards
scoped to (PreToolUse, Bash) and (PostToolUse, Write) also caught (PreToolUse,
Write) — a cross product of scopes neither pack declared. Narrowing the matcher
is not available: a policy that is never dispatched cannot decide anything. So
the matcher stays a superset and the pairing is settled during evaluation, where
the real event and tool are known.

Both are covered end to end by tests that fail without them: the shared-artifact
one allows the second pack's tool outright, and the cross-product one denies a
pair no pack asked for.

@hermes-exosphere review

Both fixed at cbf3b010, and the advisory was a fault in the fix I made last round — good catch.

Shared artifact failure not propagating. Confirmed exactly as described. Every pack id behind an artifact path is retained now and a permanent failure is recorded against all of them, for a failed import and for path-missing alike. The end-to-end regression you asked for is in pack-shared-artifact.test.ts: two packs, one broken artifact, complementary selections scoped to Bash and Write, asserting both scopes deny. Verified it fails without the fix — the Write scope came back exitCode 0 with no deny at all.

Cross product of the two axes. Also confirmed, and it is my regression from the combinedGuardMatch change one round ago: I fixed toolNames being dropped and introduced this in the same edit.

The matcher cannot be tightened, though — a policy that is never dispatched cannot decide anything, so it has to stay a superset. The pairing is settled during evaluation instead, via guardsCover(guards, event, tool), where the real event and tool are known. Unit cases pin the invented pairs, and an end-to-end case with guards differing on both axes ((PreToolUse, Bash) and (PostToolUse, Write)) asserts (PreToolUse, Write) is allowed while (PreToolUse, Bash) still denies. That one also fails without the fix.

Worth noting I initially wrote that e2e with both guards on the same event, and it passed with and without the fix — the registry already excluded the third tool, so it proved nothing. The cross product only appears when the guards differ on both axes.

Gates green: tsc 0, lint 0 errors, build exit 0, e2e 333/333, unit 4839 passed (8 failures are the locally-emptied dogfood configs).

High: Fail closed for partially registered shared artifacts

  • Rule: SEC-001
  • Location: src/hooks/handler.ts:457
  • Evidence: Artifacts sharing a path are collapsed to one merged pack in custom-hooks-loader.ts (lines 482-530), so every loaded hook carries only the winner's pack ID. handler.ts records it only under that ID (lines 457-460). missingGuards then skips the other installed record when it has neither a load failure nor a registered entry (pack-failclosed.ts lines 189-193). If a shared artifact loads but omits a policy selected only by the non-winning record, that policy is neither registered nor represented by the synthetic deny. An isolated invocation of missingGuards with this state returned an empty guard list.
  • Required change: Propagate the actual registered hook names to every original pack ID behind the shared artifact path, then add an end-to-end regression where a successfully imported shared artifact omits a policy selected only by the non-winning pack and verify its declared scope is denied.

Comment thread src/hooks/handler.ts
The mirror of the last fix, on the path where nothing goes wrong. An artifact
that imports FINE can still register less than its manifest declared — a
publisher's catalog and their source drifting apart — and registration is
recorded per pack id while a collapsed load hands back hooks carrying only the
id the collapse kept. So a policy selected by the non-winning record and absent
from the artifact left that pack in neither the failure map nor the registered
map, and `missingGuards` skips a pack in neither. Nothing registered the policy
and nothing denied on its behalf: the machine reported itself enforcing a scope
that was running nothing.

The loader now returns the ids sharing each artifact, and registrations are
recorded against all of them, so the pack that selected the missing policy is
measured against what actually loaded rather than against its neighbour.

Read through an optional chain, because a caller that stubs the loader has no
alias map and the honest reading of its absence is "this id only" — the same
answer the map gives for an artifact only one pack resolved to.

Fixed at 6ccdd31e. Confirmed — and it is the exact mirror of the previous round's finding, on the path where nothing goes wrong: I propagated the FAILURE map to every pack behind a shared artifact and left the REGISTRATION map keyed to the collapse winner alone.

loadAllCustomHooks now returns the ids sharing each artifact path, and registrations are recorded against all of them, so the pack that selected a policy the artifact never registered is measured against what actually loaded rather than against its neighbour.

The end-to-end regression you asked for is in pack-shared-artifact.test.ts: a shared artifact that imports cleanly, registers block-refunds only, and a manifest where the non-winning pack selects require-deploy-note scoped to Write. Its declared scope denies; the winner's Bash scope, which did register, still allows — so the fix does not over-correct into a blanket. Verified the first assertion fails without it (exitCode 0, no deny).

One incidental: the alias map is read through an optional chain. A test that stubs the loader has no such map, and the honest reading of its absence is "this id only" — the same answer the map gives for an artifact only one pack resolved to.

Gates green: tsc 0, lint 0 errors, build exit 0, e2e 333/333, unit 4841 passed (8 failures are the locally-emptied dogfood configs).

…899 + OTel) (#880)

* feat(otel): add reversible opt-in agent setup and wizard step

* feat(otel): add durable loopback relay and per-agent transcript lanes

* docs(otel): link changelog to draft PR 880

* fix(ci): update vulnerable existing build and SDK test dependencies

* docs(otel): clarify metrics acknowledgement in paired server

* feat(tui): palette for the 2026-10 screen language

Step 1 of the TUI redesign handoff: add the redesign's colour roles to
HUES and expose them through paint() and brandAnsi(), so no caller ever
hard-codes an escape.

- ink3 (#767f8b, labels, heading meta, key hints) and track (#3e434c,
the empty part of a progress bar) are the designed hex at 24-bit and
the SGR 2 dim attribute below it.
- err (#f07178) is the red for failed lines: 203 at the 256 tier, 31 at
  the basic tier.
- ink2 is the identity: descriptions keep the terminal's own foreground.

Deviations from the handoff, as agreed in the review (D12):
- The handoff suggested fixed 256 greys (248/243/238) and a basic-tier
bright black (90). A fixed grey fights every theme that is not ours,
and 90 is the background colour in Solarized Dark, so ink3 and track
stay the dim attribute below 24-bit, as dim already does.
- The designed ink2 #a7adb6 is tuned for dark terminals and measures
about 2.2:1 on a light one. Nothing detects the background, so
  descriptions are never painted.
- err uses 203 instead of the suggested 204: 204 is nearest by distance
but differs from the brand pink (168) only in its red channel and
reads as pink at that tier.

Audit cache keys are unchanged (engineVersion 10c367f637740d67,
detectorVersion 895c2fedece85e93): the roles are properties on existing
objects, so the bundle gains no top-level name.

* feat(tui): building blocks for the 2026-10 screen language

Step 2 of the TUI redesign handoff: header, head, rows, kv, cmd, the
●/○ state glyphs, ok/caution/fail, keys, bar, logo and helpPage, which
match reference/screens.js FP.primitives. They are pure builders over
RenderOpts, colour on or off, so every screen built from them can be
asserted at any width without a pty.

Deviations from the handoff, as agreed in the review:
- Nothing copyable is ever cut (D9). Commands, URLs, paths, ids and kv
values are left whole. Only prose (row descriptions) is shortened, with
`…`, and only when a caller sets `fit` for a terminal; piped output is
  never cut.
- Columns are derived from the block, with a guaranteed two-space gap,
instead of the reference's fixed widths. The longest real policy name
(require-no-conflicts-before-stop) is exactly the 32 columns the design
fixes, so a fixed column runs it into its description.
- The logomark returns nothing below its minimum width. The old ▮▮
one-liner is retired with that mark.
- Without colour, a progress bar's empty part is blank rather than a run
of ━, which would show no progress at all.
- Everything lives inside one exported factory, screenKit(). A new
top-level name that matches a local in a builtin policy or an audit
detector (cmd, row, head, …) makes the bundler rename that local and
rescans every user's audit history. The bundle gains one name, and
  engineVersion 10c367f637740d67 / detectorVersion 895c2fedece85e93 are
  unchanged.

* feat(config): remember which agents failproofai traces

Step 6a of the TUI redesign handoff. config.json gains an optional
`agents: { selected, seen }`: the remembered answer to the new setup's
"which agents should failproofai trace?". `selected` will drive hooks,
collection and backfill. `seen` is what was detected at the last setup,
so a newly installed agent can be offered as new.

- Absent means today's behaviour, every agent. writeConfig emits nothing
in that case, so an untouched config file keeps its exact bytes, and a
garbage value reads as absent. A broken file can never quietly switch
  tracing off.
- updateConfig carries the selection forward unless a patch replaces it
(`null` clears it). Every mutation path goes through it, the wizard's
own connect step among them, and an unrelated write that dropped the
key would silently turn every unticked agent back on.
- The ids are kept as written, minus non-strings and repeats. The daemon
owns the list of real agents and warns about an unknown one.
- isAgentTraced(id) is the one predicate per-agent surfaces ask.

Audit cache keys unchanged (engineVersion 10c367f637740d67,
detectorVersion 895c2fedece85e93).

* feat(policies): one shared answer to "is this machine enforcing?"

Groundwork for the redesigned policies and launch screens, which both
warn when policies are not enforcing. One predicate,
notEnforcingReason(), answers for both so they can never disagree. It
returns "no-hooks" (no agent calls failproofai), "no-policies" (wired,
nothing on, counting policies switched off after install) or
"observe-only", and null when something enforces.

It reads files only, so it is cheap enough for the launch path:
installed packs, cloud-managed policies, configured custom policy files,
convention files on disk (counted, never imported) and the legacy
migration shim.

Deliberately not causes, because each needs its own words:
- A refused pack and a down daemon on a daemon-configured machine fail
closed and DENY calls. "Not enforcing" would say the opposite of what
  is happening.
- A session pause belongs to one session, not to the machine.

Audit cache keys unchanged.

* feat(tui): pickers in the 2026-10 screen language

Step 5 of the TUI redesign handoff. selectOne and multiSelect are drawn
like the design's policiesAdd and agentPicker screens:
- an UPPERCASE heading with grey meta, by default a live "n of m
selected" that updates on every toggle
- a pink `›` cursor gutter and a bold cursor row; ■ ticked and □ not
- bold section names, `↑/↓ n more` when the list scrolls
- the key hints at the foot
Answered prompts collapse to the heading and `✓ <answer>`, or to the
caller's own lines. The spine glyphs are gone from both.

- The window is 12 rows, not 8. Twelve reproduces every frame in the
design; eight cannot show the nine agents of its setup screen. The
WINDOW identifier itself is kept, because the audit cache keys on
  bundled names.
- Plain `a` toggles all, as the design's hints say. ctrl+a stays as an
  alias.
- New opt-in options for the config flow's agents step: a meta
function, per-row tags recomputed on every toggle, the ✕ message for
minSelected, and a caller-defined collapsed form. selectOne gains
  `meta`, `collapsed` and `spaceSelects`.
- The `policies -i` / `--install` / `--uninstall` agent menu was
  install-prompt.ts's own raw-mode loop, with a two-write (non-atomic)
repaint and a colour gate that ignored pipes. It now uses selectOne.
It keeps its "not installed" section of forward installs, Space still
picks and Esc still exits 130. Its aggregate row reads "All N agents",
  as the design spells it.
- The two raw yellow warnings in the same file are now ▲ lines through
the palette, so they respect NO_COLOR and pipes, and they drop a
  stale six-agent list.
- install-prompt.ts's searchable policy picker had no caller since
`--install` stopped choosing policies. It is deleted with its tests
(approved), and the manager tests keep their real assertion: setup
writes the policy selection back unchanged.
- The pack pickers in pack-cli.ts take the heading meta instead of
hand-written hint strings. The pack picker is preceded by the design's
  `failproof ai v… · Add <pack>` header, the per-pack agent picker
shows display names, and Jev checks are still stated before the list.

Audit cache keys unchanged (engineVersion 10c367f637740d67,
detectorVersion 895c2fedece85e93). Full unit suite green.

* fix(otel): preserve agent settings and require real Copilot installation

* feat(otel): add optional durable TypeScript and Python span exporters

* fix(otel): initialize delivery before collector startup milestone

* feat(help): every help page in the 2026-10 screen language

Step 3a of the TUI redesign handoff. Every `<command> --help` is now the
design's helpPage: the wordmark header, then USAGE, OPTIONS and
EXAMPLES, and nothing else (decision D11). Explanations and the privacy
footer are gone (D6), and docs/ is not touched.

Each page was rebuilt against its command's real argument parsing, not
its old text. Every real flag is kept with its real default, and the
reference's misses are corrected:
- config: adds --session, --connect and --machine-id. --machine-label
says it renames and never runs setup, which is what it does today.
- policies: adds the --install/--uninstall lanes, -i/-u, --only, the
12 --cli ids and `all` for --scope when removing.
- jev: one usage row per subcommand. The design's `status | test |
models | remove` runs as a shell pipeline if pasted.
- publish: adds --min-cli-version and --entry.
- flush --wait says what it actually checks: the queue is empty.
- update --no-daemon: "migrate everything", not "settings only".
- uninstall --yes: also removes the daemon, and is needed without a TTY.
- backfill --since documents 1y and YYYY-MM-DD, which it accepts.
- Examples never put a key on argv: config shows `read -rs` into
FAILPROOFAI_CLOUD_TOKEN, the form the CLI's own warning recommends.

The index follows the design: GET STARTED (three numbered steps that end
in a command), then SET UP, ENFORCE, OBSERVE and LESS OFTEN, one row per
command, then EXAMPLES. It fits 80 columns with light rewording (taller
is allowed, D10). `config --status` takes the --token example's slot,
and the last line names `failproofai -v` and the docs.

- `policies add|remove|show --help` prints the one policies page, which
carries every flag of every lane, so `help policies add` lands right.
- Prose is shortened with `…` only on a terminal; commands are never
cut (D9), and piped output never is.
- The hook page is titled `hook`, the topic you type.
- The usage `harness` and `jev` print after a mistake now uses the same
  rows as their help pages.
- Fix: `policies remove` without a terminal suggested
`remove <owner>/<repo> --policy …`. Pack-lane remove takes an id and
no flags, so following that hint uninstalled the whole pack.

Tests: help-index.test.ts has a new contract for the taller index:
- at most 36 lines and 80 columns, `·` the only wide character, no
  escapes in a pipe
- six headings in order, and three numbered steps ending in a command
- the indexed commands must EQUAL bin's SUBCOMMANDS
- `help X` byte-identical to `X --help` for every one of them
- no module-scope assertions, so a layout change fails tests, not
  collection
The bare-publish guard is re-anchored on markers the new page really
prints, so it cannot pass vacuously. The audit help test drops the
local-only promise D6 removed and checks the option column instead of
wrapped lines.

Audit cache keys unchanged (engineVersion 10c367f637740d67,
detectorVersion 895c2fedece85e93). Full unit suite green.

* test(otel): isolate optional-library and initial-render assumptions

* feat(daemon): agent selection gates collection; scoped backfill requests

Step 6b of the config v2 plan: the Rust half of "which agents does
failproofai trace". The CLI writes the selection (6a) and the backfill
requests; the daemon obeys both.

Contract
- config.json may carry a top-level `agents: {selected: [...], seen: [...]}`.
  fpai-collect's config::load parses `selected` into a new
CollectorConfig::agents (AgentSelection), so the existing 5-second
PartialEq compare cycles the collector when it changes, with no restart.
`seen` is not read, so a newly detected agent cycles nothing. No `agents`
or no `selected` means every harness, as before. A value that cannot be
read counts as absent, the way readAgents in fp-config.ts reads it, and
gets a journal line. No deny_unknown_fields, so older daemons keep
  ignoring the key.
- One table, agents::SOURCES, maps every source (= top-level cursor dir)
to its agent: claude <- claude, claude-subagent; openclaw <- openclaw,
openclaw-sqlite; the other ten 1:1; `hooks` belongs to no agent. The
collector gate and the backfill rewind both resolve through it. Nested
stores inherit their top-level dir's agent; unknown top-level dirs are
skipped by scoped rewinds. HARNESS_KEYS is untouched.
- An unselected harness starts no task: not its default roots, not its
extra paths, not any Hermes profile. Hook activity and SDK spool delivery
always run. Journal: "agents.selected names <x>, which has no collector
source; ..." once per build, outside the sessions block; "<h> is not
traced; its N extra capture paths are idle"; and one line naming what a
  selection collects.
- state/backfill-request.json is {sinceMs, requestedAtMs, agents?, kind?}.
No kind = "user"; no agents = every selected agent (every agent when
there is no selection). "user" forgets the cursors of files modified at
or after sinceMs in the named agents' top-level AND extra-path stores,
plus `hooks`; Hermes profile stores are left alone. "added" forgets every
cursor of the named agents, Hermes profiles included, never touches
`hooks`, and sets their first-sight window to the default 7 days. Named
agents are taken as named, not intersected with the selection: config
writes the request before the selection that ticks them.

Deviations from today
- A user backfill now rewinds nested extra-path stores (<source>/<label>/,
Hermes extras included). The old rewind read top-level stores only.
- The process-global, sticky BACKFILL_SINCE_DAYS is replaced by a
per-agent window (backfill::Windows), captured when tasks are built and
kept until a later request for that agent. A scoped backfill no longer
widens other agents' windows, and an agent added after a backfill starts
on 7 days. A sinceMs in the future gives a 1-day window instead of 0.
- "added" requests older than 10 minutes, or without requestedAtMs, are
dropped with a journal line; "user" requests are honoured at any age.
An unknown kind or a non-list `agents` is discarded, not guessed at.
- A pending request is applied before the collector first starts and
before it restarts after collection is re-enabled, as well as on each
tick (still ahead of the config compare).
- filetail's "file source started" line now carries since_days.

Tests
- fpai-collect: AgentSelection parsing (absent, seen-only, garbage, the
CLI's normalisation); a selection change alters CollectorConfig and a
seen-only change does not; CursorStore::forget_all and
  forget_modified_since.
- failproofaid unit: the exact task list with no agents key (regression);
an unselected agent's tasks, extra paths and Hermes profiles absent; the
table covers every registered task and equals HARNESS_KEYS; journal
text; request parsing and scope; per-agent windows (a scoped backfill
does not widen another agent's; an agent added after a 30-day backfill
gets 7); user rewind (claude reaches claude-subagent, openclaw reaches
openclaw-sqlite, nested extra-path stores rewound, Hermes profiles not,
hooks rewound); added rewind (everything incl. profiles, hooks
untouched); a request with no agents and no kind covers the selected
agents; a stale added request is dropped, a stale user one honoured.
- agent_selection_e2e, against the real binary with an isolated HOME: no
agents key starts all 18 tasks; an unselected agent and its extra path
never start; an unknown name is warned about on every build with
sessions off; a mid-run selection change cycles to the right sources and
a seen-only edit does not; a pending request is applied before the first
start; a stale added request is dropped; a fresh one forgets everything
but hooks; a re-added agent never resumes stale cursors even when the
request is read before the selection lands.

* feat(tui): the cloud-key prompt, and a helper to collapse sudo's prompt

Step 6c of the TUI redesign handoff: the CONNECT TO CLOUD field that
config v2 draws, as its own prompt (promptCloudKey) rather than options on
promptText. promptText keeps its one-row rule, which its other callers
depend on.

What it does:
- One masked "API key ›" field, one • per character, under an UPPERCASE
heading with the destination host as grey meta.
- Tab resolves OPEN_SOURCE, a sentinel like BACK.
- A saved key is drawn at once and checked in the background (D5).
  - works: ✓ line
  - refused: ▲ line, and the step behaves as if no key were saved
  - unreachable: ▲ line, and Enter still keeps it, unverified
Enter waits for a check still in flight.
- A typed key is checked by the caller's function, with a spinner after
150 ms (80 ms frames). A refusal prints one ✕ line, keeps the key in the
field and switches the hints to "enter try again".
- Under 8 characters: an inline ✕, no server call. An empty Enter with no
saved key: the caller's ✕ line.

Spec fix 11:
- A tab or newline inside a paste no longer switches to open source or
submits halfway. Bracketed paste is turned on, and Node and Bun both
report its start and end as keypresses.
- Keys are ignored while a typed key is being checked; Esc still cancels.
- Cursor and paste mode are restored on every exit, including a process
exit while the prompt is open.

Copy change, flagged for review: the saved-key hint reads "type to replace
it" instead of the reference's "type a new key to replace it". The
reference is drawn at 104 columns; at 80 the longer line is cut through
the Tab hint, the only place Tab is mentioned.

collapseAfter(stdout, rows, fn) reserves rows, saves the cursor, runs fn
and erases back, so the wizard's DAEMON step can collapse sudo's own
password prompt to one line. The rows are reserved first so the prompt
never scrolls the saved position away.

Tests: 22. They cover every key path, the paste guard, the four saved-key
states, Enter waiting for the saved check, the spinner delay, terminal
restore, the key never reaching the output, colour on and off, and
collapseAfter with and without a terminal.

Audit cache keys unchanged (engineVersion 10c367f637740d67,
detectorVersion 895c2fedece85e93).

* feat(config): the v2 setup flow — key, agents, apply

Step 6d of the TUI redesign handoff. `failproofai config` is now
sudo → API key (Tab for open source) → which agents to trace → apply,
drawn in the 2026-10 screen language. The logomark and header open the
run, each step collapses to its heading and a ✓ line, and DONE says what
was written and whether anything enforces. The "Ready to apply?" step is
gone: Enter on the agents step is the last decision, and Esc anywhere
before it changes nothing (D3).

DAEMON
- Same logic: probe, version skew, broken worker, stale unit.
- sudo's own password prompt is erased after it answers, so the step
collapses to "✓ Root granted to install failproofaid".
- The broken case keeps "every tool call is denied" on screen.
- The install still happens at apply, before anything else is written.

CONNECT TO CLOUD
- One field (promptCloudKey) with the host as heading meta.
- A saved key from either credential is shown at once and checked in the
background (D5). Reading only the policy credential, as setup did,
showed a collector-only machine as unconnected.
- Enter on an empty field keeps the saved connection exactly as it is:
no connectToCloud, so collector settings are not reset. It writes
mode: cloud if that is missing.
- Tab never disconnects (D4). On a connected machine the hint is
"tab skip" and the line "Not connecting. The connection to <host> is
unchanged."; --disconnect stays the way to disconnect.
- Keys are checked with introspect first (organisation and permissions,
no side effect), with the old empty ingest probe on servers too old
for introspect. A key with one of events:add / policies:pull is
accepted with a ▲ note; one with neither is refused.
- A refused --token re-prompts on a terminal and still fails a headless
run (spec fix 9). A --url or FAILPROOFAI_CLOUD_URL override is named
  in the heading.

AGENTS (D1: all twelve listed)
- A first run ticks what is detected. A re-run opens on the saved
selection, with agents detected since then ticked and tagged "new on
this machine". An upgraded machine opens on what is hooked at user
scope today (spec fix 8: a gateway counts when any profile is hooked).
- Rows are tagged live: will be added / will be removed / not installed.
- seen accumulates across runs, so a PATH glitch cannot make an agent
someone unticked arrive ticked as "new" later.
- No history wording (D7). A removal promises only what the code does.

Apply order (spec fix 3):
1. daemon
2. hooks for the chosen agents, and user-scope removal for the rest
3. the "added" backfill request
4. agents in config.json
5. the connection

Unticking takes failproofai's entries out of that agent's user settings
only. It never calls removeHooks, which also resets policy config in
every scope (spec fix 1). A test pins every policies-config.json
byte-identical.

backfill-request.ts is the one writer of backfill-request.json (spec
fix 6). Requests merge, keeping the stronger: a pending user request
stays user, the widest window wins, and no list means every agent. The
file is written atomically. An "added" request carries no sinceMs, so a
daemon older than `kind` drops it instead of rewinding every agent.
Config writes one only for agents added back, and only when something
will collect afterwards. `failproofai backfill` writes through it too.

The log-only check (spec fix 2): a hook event from an untraced agent is
evaluated as usual and logged once per session, through a bounded
record on disk. Allowing such a call would open a hole in a hook
somebody installed on purpose.

DONE:
- the generic "Policies are not enforcing yet." (D18) plus "Turn on
ours", or "N policies are enforcing on M agents". The count comes
from enforcingPolicyCount, which counts the way notEnforcingReason
  decides (spec fix 10).
- the custom policy files that will not load, moved here from the
  removed review step
- Jev's status lines, unchanged

Telemetry adds mode, agents_selected, agents_detected and
agents_changed. Counts only, never names.

Copy that differs from the reference, both for 80 columns:
- the agents meta reads "from last setup" / "as protected today"
- the DAEMON and CONNECT lines say what was checked, not what was
  written (D3)

Also: notEnforcingReason no longer calls a machine whose only pack was
refused "not enforcing". That pack fails closed and denies.

Removed with the review step: reviewLines, buildCompletionSummary,
policyNamesLine and their tests. Two review-screen tests are gone from
custom-policy-discovery.

Tests:
- configure-wizard: 85, rewritten for the new flow (decisions above,
both Esc-before-apply cases, the byte-identical policy configs)
- backfill-request: 14 (read as the daemon reads, merge rules, atomic
  write, the backfill command)
- untraced-note: 5
- not-enforcing: 11 (refused pack, the count)

Full unit suite and e2e green. Audit cache keys unchanged
(engineVersion 10c367f637740d67, detectorVersion 895c2fedece85e93).

* feat(policies): the policies screens in the new language

Step 3b of the TUI redesign: `failproofai policies`, the result of
`policies add <pack>` and `policies show <pack>`, built from screenKit()
and drawn as designed (decision D14).

- The listing names each pack once as a `pack` row, counts `enabled N of
M`, and lists policies ● on or ○ off under UPPERCASE category headings
with "n of m" meta. Off policies fold into one "○ k more off" line per
category, and the screen closes on "See all M with failproofai policies
  --all".
- New bare `failproofai policies --all` lists every policy, nothing
folded. Wired into the listing parser only; its help row lands with the
  help rewrite.
- At most one attention line, most severe first: a pack that will not
load and fails closed (✕, it denies what it covers; fix `failproofai
policies remove <id>`), then notEnforcingReason()'s one generic
"Policies are not enforcing yet." (D18; fix `failproofai config`), then
a custom policy file that is not running, hooks in more than one scope,
and an unknown policyParams key. The policy_params_validation_warning
event still fires for every unknown key, whichever line is shown.
- Custom, convention and cloud-managed policies get sections in the same
style. The scopes subtitle, the `Config:` footer and the explanatory
  paragraphs are gone (D6).
- The listing is a pure renderPoliciesScreen() over a gathered value, so
it is tested with colour on and off at any width. listHooks keeps every
other behaviour, including mirroring convention policies into the user
config. Descriptions are shortened only on a terminal; names, ids,
  paths and commands never.
- `policies add` opens with "✓ Turned on N policies from <id>@<version>:
<reason>.", then per-category counts and one line per outcome fact
(rename, pin, Jev checks, min-CLI note, warnings), and closes on "Turn
on the other K with failproofai policies add <source> --all". It no
longer says "Turned on" when nothing was: a carried re-add says "Kept",
a merge says how many are on, and a pack with nothing on, or with only
Jev checks, says it was installed. The five reasons stay distinct.
- `policies show` opens with kv rows: publisher (from the source owner,
never the self-declared id), commit when there is one, "N in M
categories ■ on by default", and effect / requires / jev checks rows
when they apply; then ■/□ rows per category and "Install with …". No
  release date (D16).
- screenKit() gains failed (✕), selected (■), unselected (□) and meta (a
grey qualifier) as properties; tui.ts gains no top-level name.

Judgment calls where the design is silent:
- A pack row carries grey tags rather than new states: `observe`; its
Jev checks; `from <source>` when the source is not the repository its
id names (keeps the old anti-spoofing display); and `failed to load:
<reason>` for a refused pack, so the reason and its fix stay visible.
- Category headings are prefixed with the pack id only when two packs
have regex policies, so the shipped FailproofAI/policies +
  FailproofAI/jev-policies pairing reads as designed.
- Every off policy folds, as briefed (the reference also draws a few off
rows by name), and the "See all" line shows only when something folded.
- An empty machine shows the generic line, "Turn on ours: …", and one
more line in the same shape, "Or anyone's: failproofai policies add
<owner>/<repo>", to keep first-run-hint's "ours is not the only
  channel" intent.
- A cloud-managed observe policy keeps ● and is tagged `observe`, like an
observe pack. A convention file that fails to load is a ✕ row but no
attention line, matching the old warning set.
- The name column is derived from the names shown: at least the design's
32, two spaces past the longest, shared by every pack section.
- `add`'s closing command uses the source as typed, never the id, which
can name a repository the pack did not come from. "Pinned to" names
  the full source, owner/repo@tag.
- The Jev checks section on `show` keeps its rows and notes; only its
heading moved to the new style.

Tests that pinned the old screens are updated deliberately, keeping each
intent, with negative chip pins rewritten as positive checks in the new
vocabulary; manager-cloud-listing now runs against a temp home. New tests
cover --all, folding, multi-pack prefixes, the observe note, precedence,
the show screen, colour on and off, and the new kit pieces.

Audit cache keys unchanged (engineVersion 10c367f637740d67,
detectorVersion 895c2fedece85e93).

* feat(config): status and pause in the new language

Step 3c of the TUI redesign: `failproofai config --status` and the
`--pause` / `--resume` confirmations, built from screenKit().

config --status (new src/hooks/config-status.ts)
- THIS MACHINE / CLOUD / ENFORCEMENT. Every kv row of all three sections
goes through one kv() call, so they share one value column derived from
the widest label (INV-24). Values are never cut.
- gatherStatusFacts() is the only part that reads the machine;
renderStatusScreen() is pure, so every row and the order between
problems are pinned without a daemon, a terminal or a clock.
- New data, approved in D16. The dashboard row probes the bind host from
lib/dashboard-host.ts on port 8020 with a 150 ms connect. Delivery reads
collector-health.json (nothing on this side read it before), the refused
batches in state/failed and the spool backlog. Today's blocked and warned
counts come from the hook-activity store's own windowed reader
  (getHookActivityEntriesSince); no second parser.
- At most one attention line, by severity: a daemon-configured machine
whose daemon is not answering (every tool call denied), a pack that will
not load (the calls it covers denied), the cloud refusing the key, not
enforcing (notEnforcingReason), unhealthy Hermes profiles, the daemon
down on a connected machine (macOS "unknown" gets its check command),
other refused batches, then daemon/CLI version skew.
- Today's facts stay on screen as rows, so a worse problem cannot hide
them: the skew on the daemon row, refused batches on the delivery row,
one hermes row per unhealthy profile, macOS's unreadable service state
on the daemon row, one row per paused session with its id and time left,
and "Cloud-managed policies keep enforcing." whenever something is
  paused.

--pause / --resume (session-pause-cli.ts)
- "✓ Paused enforcement for this session until 14:32 (30 minutes).", with
the duration actually granted (the 8h ceiling counts from
firstPausedAt), an "Extended the pause ..." variant, then the cloud line
and "Resume early: failproofai config --resume". Resume answers in the
same shape; errors are one ✕ sentence and one fix line.
- Clock, locale and time zone are injectable, so the tests are
  deterministic.

Also: kv() paints no empty colour span for an empty (continuation)
label, and flush-cli's spoolBacklog() adds the oldest queued batch's age
(pendingBatches counts through it).

Judgment calls
- The account row is "<org> on <host>". The host was today's "cloud
connected to <url>" row; the design shows the org alone. With no org on
record it reads "Connected to <host>". ", for reporting only" and
"Not sending, connected for policy only" keep today's half-connection
  facts.
- The machine row keeps describeMachine's label plus 8-character id,
because labels collide; the design shows the label alone.
- Dropped as fine print (D6): the layout version, the masked token row,
healthy Hermes rows (with the copy-mode note), and the daemon
paragraphs ("install it as a service ...").
- Two attention candidates the brief did not list. "Policies are not
enforcing yet." is D18's shared predicate. A pack that will not load is
listed above it, because such a pack fails closed and DENIES; calling
that machine "not enforcing" would say the opposite of what happens.
- Delivery problems other than refusals stay on the row: no report yet, a
record older than 2 minutes (the daemon rewrites it every 30 s), no
delivery block (the daemon has no key), and a stuck backlog (the oldest
queued batch and the last delivery both older than 15 minutes).
- Times are to the minute in the machine's locale (seconds dropped), and
durations are in words. A renewal the ceiling cut short adds "Capped at
8 hours from when this pause began."
- An explicit --session is named back, and its resume command carries it.
The status screen closes with --resume --session and the id, or <id>
  when several sessions are paused.
- runPauseCommand's "status" action is gone. Its two tests moved to
config-status.test.ts with the same intent. connectionStatusReport,
connectionStatusLines and versionStatusLines are no longer called by
--status. They stay for now (tested, and another step may reuse them)
and can go in the clean-up step.

Audit cache keys unchanged: engineVersion 10c367f637740d67,
detectorVersion 895c2fedece85e93.

* feat(launch): the dashboard launch screen in the new language

Step 4 of the TUI redesign handoff. Bare `failproofai` prints the
redesign's launch screen once the local dashboard is listening: the
logomark (terminal only), the header, a DASHBOARD block (url, policies,
agents, cloud), at most one attention line, LINKS and the key hint.

- scripts/launch-screen.ts: a pure builder over screenKit(), facts in and
lines out, plus the ✕ block for a start that fails.
- scripts/launch-facts.ts: reads the facts from files only. policies come
from the same inputs as notEnforcingReason(). agents come from
agents.selected, else the agents hooked at user scope, by display name,
with detectInstalled() kept off the path. cloud follows config
--status's precedence (environment, credentials, org, refused uploads
over the credential file's claim), without its systemctl read.
- scripts/launch-ready.ts: readiness (decision D16). Next's "Ready in"
  line; a TCP probe of the bind address every 250ms after 5s without it;
a 30s deadline that prints the screen with "○ starting"; and an exit
before ready reported as one ✕ line with the real cause. For a taken
port it drops Next's stack trace and says the port is in use. Server
output is held until the start is decided, so the screen opens it.
- scripts/skew-log-filter.ts: makeNextStartupFilter() drops Next's four
startup lines until the ready line, beside the skew filter, whose
  pass-through test is untouched.
- scripts/launch.ts: launch(mode, { screen, logo }). Audit's hand-off
passes screen: false and keeps its own lines (LAUNCH-1); the bin passes
logo: false after the first-run wizard ran (LAUNCH-8). FORCE_COLOR
reaches the child only when stdout is a terminal (LAUNCH-M2). The
exposed-bind warning is the screen's attention line and outranks "not
enforcing"; with no screen it goes to stderr before the server starts.
Dev mode keeps inherited stdio and prints its screen up front, marked
  starting.
- The audit tests that pinned launch("start") now expect the screen
option, the value this change sets.

Judgment calls:
- docs is https://docs.befailproof.ai. docs/index.mdx is the deployed
docs' home page; the /introduction page #461 pointed at moved to
docs-old/ in #699 and has no redirect in docs.json.
- The shipped CLI cannot change its port (the bin rejects --port and
launch sets PORT itself), so the port-in-use line says how to free it.
Only the contributor scripts, which take --port, are told to pass
  another one.
- policies reads "N on from <id>@<version>", "N on from K packs" or
"K sources", or "none on"; custom policy files are noted, never
imported. agents names four and "and K more" past five. cloud gives
half connections and refused uploads their own wording, so the row
never says connected while nothing arrives.
- The security warning keeps all three consequences in one sentence, so
its fix sits on the next line rather than inline.
- "The wizard drew the logomark" is read as maybeFirstRunConfigure()
returning true. That also covers its two early refusals (sudo, an
unsupported platform), which draw no art.
- renderLaunchBanner has no callers now. It is left for the step-8
cleanup so this commit leaves tui.ts alone; renderBrandLogo and intro()
  are unchanged.
- The node:path and node:net imports are aliased, so the scope-hoisted
CLI bundle renumbers and renames nothing outside the launch modules.

Audit cache keys unchanged (engineVersion 10c367f637740d67,
detectorVersion 895c2fedece85e93).

* feat(audit): per-agent progress and the audit screens in the new language

`failproofai audit` now draws the 2026-10 progress screen: a bar per
agent with sessions to scan and running counts of what was found,
finished in place when the scan ends (decision D16). The sign-in,
--schedule, --no-schedule and --status screens move to the same
language.

runAudit
- RunAuditOptions.onProgress: ONE `discovered` event with every agent's
transcript total (0 included) and the number of policies a replay can
fire, then one `transcript` event per transcript - cache hit, scan,
failed scan or skipped task alike - with its agent and hit counts.
Optional: the dashboard and the scheduled run do not pass it, and the
result is identical either way. A callback that throws is caught, so it
can never count as a scan error.

The audit screen (src/audit/cli.ts)
- buildAuditScreen() is pure: header("Audit"), SCANNING HISTORY with
"<n> agents, <m> policies", one row per agent (bar(30) plus "301 of 412
sessions" or "✓ 1,284 sessions"), FOUND SO FAR with "would block" /
"would warn" / "audit only", then the key hint. Finished, the same
screen reads SCANNED HISTORY / FOUND, every bar done, and the summary
replaces the key hint. While transcripts are still being listed the
heading meta is the dashboard's first stage label.
- Live only on a TTY with colour, through the kit's new live() region:
at most one frame per 100 ms (tokens.json), each ONE write in
synchronized output. The old spinner wrote its cursor-up and its lines
as separate chunks and advanced on a timer. Piped or under NO_COLOR, the
finished screen prints once, with no escapes and no frames. Prose is
fitted to a terminal, never to a pipe (D9); the bar gives up width
  before the counts do.
- FORCE_COLOR is no longer read, matching tui.ts (OPS-24).
- The hand-off is one ok line naming the audit URL, then the key hint:
"✓ The audit is ready: http://localhost:8020/audit" and "ctrl+c stop
the dashboard". Audit prints no logomark itself; the launch("start")
call is unchanged (its banner is the other branch's change).
- AUDIT_STAGES stays and still mirrors the dashboard's RunProgress; its
comment now says the CLI draws real bars (D16) and uses the first label
for the discovery phase. The drift test passes unchanged; only its
  header comment is updated.
- audit --help is untouched.

Kit (src/hooks/tui.ts), no new top-level names
- screenKit().live(out): a redraw-in-place region. Cursor hidden from the
  first frame and shown again on done, on exit and on SIGINT/SIGTERM;
a signal is re-raised once the cursor is back, so Ctrl+C still stops
the command exactly as before. The last frame is never truncated.
- repaint() takes an optional frame { lead, tail, cut } so a cursor mode
rides in the same write as the frame.
- promptText({ field: true }): the redesign's field, " email ›
value", with a refusal as a ✕ line under it, drawn in one write and
cleared as soon as the value changes.

Sign-in and scheduling
- cli-login: header("Audit"), SIGN IN, fields, one ✓/✕ line per outcome;
no intro() logomark and no spine (LAUNCH-10). Validator messages are
  sentences now.
- schedule-cli --status: header("Audit"), SCHEDULE kv rows on one column
(● / ○ states), then one ▲ per distinct problem with its fix (daemon
first, then digests). --schedule and --no-schedule state results as
sentences; the daemon warning is a ▲ line on stderr.

Judgment calls
- KEPT, despite D6, the one line `audit --schedule` prints about what
each report sends: "Each report sends finding counts, redacted example
commands and this machine's name." as one ink3 line under the result.
`reportsConsentedAt` records that a person READ that disclosure before
anything leaves the machine, so it is part of the consent, not fine
print. A test now pins it. The product owner will be told explicitly.
- "would block" / "would warn" go by what the policy does, not by
severityForBuiltin, the score's heuristic, which files protect-env-vars
and prefer-package-manager under warn although both deny: warn-* would
warn; sanitize-* would warn too, because it acts on PostToolUse after
the call ran (#669); every other builtin would block. The audit-only
detectors get a third row, "audit only", instead of being dropped or
misfiled (OPS-11). All three rows always show, so the block never jumps.
- "<m> policies" counts the builtins a replay can fire (34 today): the
Stop-only require-*-before-stop gates are registered but never asked.
- The summary says "already covered by your policies", not "already
blocked": an enabled warn-* policy warns rather than blocks.
- The empty-history ending no longer points at `policies --install`: the
audit reads each agent's own history, hooks or not. It says "Run it
again after using your agent: failproofai audit".
- Counts carry thousands separators (1,284 sessions), like the summary.
- The onboarding audit (runPostSetupAudit) shares the screen; its own
lines are restyled: no ✦ banner, "▲ None of this is enforced yet. ·
failproofai policies add FailproofAI/policies", "✕ The audit could not
  finish. · failproofai audit".
- --no-schedule drops "your session is untouched" (D6); the session is
  still kept, as before.
- The sign-in fields have no key-hint line under them: promptText draws
one row, and the placeholder hint stays on that row.

Tests: audit-progress.test.ts (the callback on real fixture transcripts
for claude and factory, cache hits included), audit-screen.test.ts (the
builder at fixed widths, plain and coloured, mid-scan and finished; the
piped, NO_COLOR and TTY wiring, with every frame one atomic write),
live() and the field prompt in the kit tests, and the sign-in and status
screens. Assertions on the old buildSummary copy, the old status labels
and a lowercase "running" are updated deliberately; nothing is deleted.

Audit cache keys unchanged: engineVersion=10c367f637740d67
detectorVersion=895c2fedece85e93.

* feat(backfill, config): traced agents only, --agents, --oss

Step 6e of the TUI redesign handoff.

backfill
- Re-sends only the agents failproofai traces (agents.selected, or every
  agent when there is none).
- `--agents a,b` narrows it to a subset:
  - a name that is not an agent is refused ("Not an agent")
  - an untraced agent is refused with the reference's backfillUntraced
    copy
  - so is any daemon/CLI version mismatch, because an older daemon reads
    every request as "rewind everything"
Nothing is written in any of those cases.
- Without --agents the request names nobody, so the daemon applies the
selection current when it acts, not a stale copy.
- Session counts come from the audit adapters, the same readers the
audit uses. They know all twelve agents and count sessions; the old
file survey knew seven and walked the wrong directory for three
  (OPS-1).
- Database-backed agents (Goose, OpenCode, Devin) say "all N sessions,
whatever the window": the collector reads them by row and re-sends all
of it. Hermes says it is not re-sent, because the daemon does not
rewind its profile stores until they have a window (D7).
- The screens follow the reference: backfillDry (WOULD RE-SEND,
traced only, "Not traced, skipped: …"), backfill (one ✓ line), and
  backfillUntraced.
- The result says when the data arrives ("over the next few minutes",
watch with config --status) instead of implying flush --wait finishes
it (OPS-3). It keeps the streams caveat and the stopped-daemon line
  (OPS-2).

config
- `--oss`: skip the connect step. A connection already present is left
as it is (D4). It overrides FAILPROOFAI_CLOUD_TOKEN; only an explicit
--token contradicts it, which is an error.
- `--agents a,b|all`: any of the twelve, validated before the wizard
runs, and the agents question is skipped.
- Both are documented in config --help.
- The help index's config row now says what setup asks: "Connect to
cloud or use open source, and pick agents to trace". This is the
design's row, trimmed to fit 80 columns.

config --status counts agents found on this machine but not traced
("8 traced, 1 not traced, 3 not installed"), as config-flow.md asks.

harness list / add-path
- Drawn with the kit: a heading per agent with its count, rows of path
and agent ids, one ✓ or ✕ line per result.
- An agent that is not traced gets a ▲ line, because the daemon starts no
task for it, extra paths included.

Fixes (pre-existing, found while restyling):
- `backfill 6m` (no --since) silently ran the default 30 days. Every
stray word is now refused, with "did you mean --since 6m" when it
  looks like a window.
- `config --token K --machine-label L` ran a rename and ignored the key
(spec fix 9). With --token, --oss or --agents it is now the name to
  set up under.
- `config` ran full setup with an unknown option silently ignored. An
unknown option or a stray word is an error now, and `config status`
  suggests `config --status`.
- backfill's not-connected error taught `config --token <key>`, a key
on argv. It points at `failproofai config`.
- `harness add-path` claimed `harness list` shows what is captured. It
reads the same file; the daemon's log is where rejections show up.

Tests:
- new: backfill-cli (11)
- updated to async: backfill-request
- harness-extra-paths: the ━━ rule pin becomes the UPPERCASE heading,
plus untraced tests with their own temp home
- config-status: not-traced count
- cli-args e2e: --oss with --token, unknown agent, unknown option, the
  bare-word suggestion, help rows, `backfill 6m`

Full unit suite (8158) and e2e (341) green. ~/.failproofai/config.json
was checksummed unchanged across the run. Audit cache keys unchanged
(engineVersion 10c367f637740d67, detectorVersion 895c2fedece85e93).

* fix(daemon): a config edit right after a backfill was never applied

After applying a backfill request, the collector manager rebuilt the
collector from the config read at that moment, then read the config
again to record what was running. An edit that landed between those two
reads was recorded as already running, so the next compare saw no change
and the edit was never applied.

This was latent on main. Setup v2 makes it routine: when `failproofai
config` re-adds an agent it writes the `added` backfill request first
and the selection right after, which is exactly that window. The
re-added agent stayed untraced until some unrelated edit cycled the
collector.

The fix records the config read BEFORE the build as what is running,
the same rule the config-change branch below it already documents ("What
was BUILT FROM, not a fresh read"). An edit landing between that read
and the build now costs one extra cycle instead of being lost.

agent_selection_e2e's
an_agent_re_added_on_a_running_daemon_never_resumes_its_stale_cursors
reproduces it. It failed 2 of 3 runs on an idle machine before the fix
and passed 6 of 6 after. cargo fmt, clippy -D warnings, and the
failproofaid crate's 190 tests all pass.

* fix(config): --disconnect no longer sends you to restart the daemon

The disconnect result said a running failproofaid "keeps the key it
started with, so restart it to stop the current process sending". That
was true when the collector manager started once per daemon lifetime.
It now polls the collector config every interval, and that config
includes the ingest credential (CollectorConfig::ingest; is_enabled() is
ingest.is_some()), so the collector stops within one interval once the
key is gone. The restart advice sent people to do work that was not
needed, and it needs root.

The line now says the daemon stops sending within a few seconds. The
test that pinned the restart wording tested exactly this value, so it
is updated to pin the corrected one.

* fix(otel): lazy imports, isolated setup and precise local configuration

* fix(policies): show that the pre-pack shim still enforces builtins

On a machine with `enabledPolicies` and no regex pack installed,
handler.ts registers those builtins itself: the migration shim. The
policies screen showed none of it, so it drew only the convention files
while thirty builtins enforced. config --status counted them ("30 on
from legacy builtins"), so the two screens disagreed. Found in a QA pass
of the merged screens.

The screen now has a BUILT IN heading, "N on, from before packs",
followed by the command that moves them into a pack. It uses the same
hasInstalledRegexPacks() test the hook path does. It is a count, not a
list of names, because #738 pins that this screen names no builtin
(cli-args e2e: the list never contains "block-sudo"). A machine like
that is no longer drawn as empty either.

Also: config --help's --machine-label row said "never runs setup". Since
6e, `--token K --machine-label L` sets the machine up under that name,
so the row now says so.

* feat(cli): flush, migrate, update and uninstall results in the new look

printReport is the shared presenter for the action commands that return
bare facts: flush, migrate, update and uninstall. It now draws the
2026-10 shape instead of the retired `title()` + indented-wrap one:
- the kit header names the command, with any meta after it
  ("Migrate · dry run")
- top-level lines start at column 0
- lines a module indented on purpose keep their indent
- a failed run (exit non-zero, printed to stderr) opens with ✕, and what
follows is indented under it as the fix

Backticked commands still turn pink and lose their backticks. Wrapping
still happens before colouring, so widths stay right. Paths are never
broken, because wrap() only breaks at spaces.

Restyling the presenter, rather than each command, is the review's plan
for screens the design does not draw: "restyle the shared pieces in
place so most follow automatically".

* feat(hooks): installs and removals keep the agent selection true

Step 6f of the TUI redesign handoff: decision D2, "turn the agent back
on". `agents.selected` in config.json drives which agents `failproofai
config` hooks and which harnesses the daemon collects, but only `config`
wrote it, so:

- `policies --install --cli goose`, on a machine whose selection left
Goose out, wired Goose, and the next `failproofai config` silently
  removed those hooks again.
- `policies --uninstall --cli goose` left Goose traced, so its sessions
kept being collected and the next `config` put the hooks back.

Now:

- Installing an agent's hooks, at any scope, adds it to
  agents.selected (INTEGRATION_TYPES order), and to `seen` when
detectInstalledClis() finds it. With no `agents` key nothing changes:
  every agent is traced already.
- Removing an agent's hooks at user scope or `--scope all` takes it
out. With no `agents` key it writes `selected` = every
INTEGRATION_TYPES id but the removed ones, `seen` =
detectInstalledClis(). An empty selection is allowed.
- One line after the command's own output, built with screenKit().ok:
"✓ Goose is traced again, so failproofai config keeps its hooks." /
"✓ Goose is no longer traced, so failproofai config won't add its
hooks back." Several agents read "A, B and C are ...". A quiet
  install prints nothing.

Covered: `policies --install` / `-i` (with --cli or the agent picker),
`policies add <name>`, `policies --uninstall` / `-u`, `policies remove
all`, and the dashboard's per-agent panel (app/actions/
install-hooks-web.ts, server side). `policies add <owner>/<repo>` and
the bare `policies add` picker install packs, not hooks. Untouched:
`failproofai config` (its own installHooks call writes no selection and
prints no note), `failproofai uninstall`, and project- or local-scope
removals.

The logic is one function, keepAgentSelectionTrue, in the new
src/hooks/agent-selection.ts. It is that module's only top-level name,
so the bundle gains nothing a policy or detector local can collide
with; the audit cache fingerprint is unchanged
(engineVersion=10c367f637740d67 detectorVersion=895c2fedece85e93).

Judgment calls:

- Opt-in, not default. installHooks takes `syncAgentSelection` in its
options and removeHooks in its opts; bin's four call sites and both
dashboard actions pass it. Default-on would need the wizard and the
full uninstall to opt out (or a check on the `source` telemetry tag),
would make every bare removeHooks() write config.json (manager.test.ts
pins exactly three settings-file writes for --scope all, and none
when there is nothing to remove), and would let any caller that does
  not isolate FAILPROOFAI_HOME rewrite a real config.json.
- A removal untraces every agent the command acted on, including one
with nothing to remove: "No settings file found. Nothing to remove."
is followed by the note. The command still said "not this agent",
and leaving it traced lets the next `failproofai config` hook it.
This includes the Claude Code default when no agent CLI is on PATH.
- An install traces only agents whose hooks were actually written, so
`policies add <third-party pack policy>`, which wires no hooks,
  traces nothing.
- Adding an agent rewrites `selected` in INTEGRATION_TYPES order. The
wizard saves it detected-first, but every reader re-sorts, so only
the file changes. Ids this build does not know (a newer build's
agents) are kept, after the known ones. `seen` is rewritten only when
an agent is added to it.
- Plural notes say "their hooks" where the singular says "its hooks",
alongside the plural verbs. "failproofai config" in the note goes
through kit.cmd: pink with colour on, the exact text with it off.
- A config.json write that fails does not fail the command, because the
hooks are in place. It prints a ▲ line naming the reason and the
consequence instead of the ✓ note.
- `policies remove <name>` passes the flag too. Its only effect is on
`policies remove all`, which removes hooks; a policy name takes the
policy-only path, which never touches the selection.
- A quiet install still writes the selection; only the note is muted.
No current quiet caller opts in.
- The dashboard panel shows errors only, no success messages, so its
UI is unchanged and the note lands in the server log.
- Left alone: `failproofai update`'s Hermes migration. It rewrites
hooks a profile already has (shell hooks to the native plugin) and
  installs for no new agent.

Consequences worth knowing:

- A project-scope install now traces the agent machine-wide, so the
next `failproofai config` also gives it user-scope hooks.
- On a machine with no config.json, a user-scope removal creates one
(defaults plus the selection), as `harness add-path` and the
telemetry opt-out already can; detectLayout reads that home as
current. Its first `failproofai config` then opens on that list
(every agent but the removed one, undetected ones included) rather
  than on the detected agents.

Tests: __tests__/hooks/agent-selection.test.ts (32),
__tests__/dashboard/install-hooks-web-agent-selection.test.ts (3),
__tests__/e2e/cli/agent-selection.e2e.test.ts (3, through the real
binary). tsc and eslint clean; full vitest 391 files / 8179 passed;
e2e 18 files / 337 passed. A first full run on a loaded machine had 17
timeouts in unrelated files, which all passed re-run alone.

* test(cli): pin the new failure shape of action results

A failed flush prints the kit header, then ✕ and the module's sentence,
then its fix indented two columns, with no literal backticks. Also moves
the printReport CHANGELOG line from Fixes to Features, where a new look
belongs.

* feat(policies): show --releases and the empty picker in the new look

Two pack-lane screens still used the retired title/stack/emptyState/
nextStep helpers:
- `policies show <owner>/<repo> --releases`: the kit header names the
repository (the id is case-sensitive, so it is never uppercased), with
a RELEASES heading and its count. The releases stay a column table,
since the kit's rows are two-column. The "—" note is grey, and the
  install command is pink.
- `policies add` / `remove` with nothing installed: the same three
doors the policies screen's own empty state offers (ours, anyone's,
look first), as indented pink commands.

The facts the tests pin are unchanged: the release count, "no releases",
"— means the release did not record it." and "No policies are installed
yet."

* fix(config): an unattended re-run keeps a saved empty selection

Removing every agent's hooks (policies --uninstall, after 6f) saves an
empty selection. An unattended `failproofai config` then fell back to
all twelve agents, because the fallback for an empty start was meant
for a brand-new machine with nothing detected. It put back every hook
the user had just removed.

A saved answer now stands even when empty. The run hooks nothing,
because an empty list is not something to hand installHooks, and the
AGENTS line reads "▲ Tracing no agents · failproofai config". The
interactive picker still needs at least one agent.

* feat(config): --connect, --disconnect and rename results in the new look

They printed bare lines with no heading. They now go through printReport
like the other action results: the kit header names what ran (Connect,
Disconnect, Rename), and a failure opens with ✕ with its fix indented
under it. The modules' own lines are unchanged, so their tests are too.

* feat(notices): errors, notices and the one-off screens in the new language

Step 7b of the TUI redesign. Every error now prints as the design's
notice: `✕` and what went wrong, then the fix on its own line with the
command in pink. The remaining one-off screens move to the screen
language: the uninstall question (D17), the banners printed before a
command, the publish done screen and `jev status`.

Errors
- One printer for every CliError, bin's and the manager.ts ones it
  carries: `Error: <message>` becomes `✕ <line 1>`, every later line a
fix line indented two, and a `backticked` command painted pink with the
backticks dropped (new screenKit().notice). Colour is decided from
stderr; exit codes are unchanged. Written with process.stderr.write and
drained through exitAfterFlush, because Bun paints console.error red
on a terminal, a colour from outside the palette. "Unexpected error"
stays plain: that path cannot trust the install enough to load the kit.
- Q4, the design's words: "There is no command called 'status'." then
"Did you mean failproofai config --status?", from the new
src/hooks/did-you-mean.ts. A table of words people type comes first
(status, pause, resume, login, setup, install, logout, upgrade, hook,
dashboard and more). Then the nearest subcommand by spelling: a swap of
two letters counts as one edit, and a unique prefix wins, so `back` is
  backfill, not `pack`.
- Q6, a distance cut-off: two letters must match exactly, up to four
letters one edit, longer words two. `install` suggests `policies
--install` (the table is asked before spelling), `-i` no longer
suggests audit, and a word close to nothing gets no guess and points
at `failproofai help`. Flag suggestions are whole commands
(`failproofai --version`), and --hook is never offered.
- Q5: `--cli` in policies --install, --uninstall, add and remove goes
through one helper reading INTEGRATION_TYPES: "Not an agent: claud
(did you mean claude?)" or "Missing agent name after --cli.", then all
  12 ids on two rows.
- Every other bin error reworded per the inventory: full stops, two
spaces around commands, label-shaped first lines rewritten (policies
  show, --custom), `policy add` is `policies add`, the remove lane's
missing --scope gains `all`, the add|remove lane's unknown flag points
  at `failproofai policies <add|remove> --help`, --version's extra
argument now says `failproofai help` like --help's, and a missing
--token value points at FAILPROOFAI_CLOUD_TOKEN, never argv. The
unreachable "Unknown policies subcommand" branch is deleted (ERR-11).
- The --hook usage error is the notice shape written out by hand, with
no colour (see the audit-cache note below).
- manager.ts: fix lines gain backticks so their commands paint. The two
errors it printed itself (`Error: …` for a custom-policy file that
will not load or registers nothing) go through the notice too.

Uninstall (D17)
- ONE question: `Continue? y/N`, from the new screenKit().confirm. It
reads raw keys like the pickers; only y or yes, then Enter, is a yes,
and Enter alone, Esc, Ctrl+C, Ctrl+D or closed input is a no. Above it,
a plan read off what the steps do: "This removes failproofai's hooks
from N agents and the failproofaid service (needs sudo).", the agents,
"Also resets enabled policies, custom policy paths and policy
parameters in policies-config.json." (removeHooks(…, "all") does that),
and what is kept, or what ~/.failproofai holds with --purge.
- Yes is yes to the whole plan, the service included, which is what
--yes always meant. The second "remove the service too?" question is
gone; keeping the daemon while removing hooks is `failproofai policies
--uninstall`. --yes still asks nothing and removes the service, and
without a terminal and without --yes it still refuses with exit 1.
- The outcome is ✓ / ✕ lines under header("Uninstall"). removeHooks
gains an opt-in `silent`, so its own `policies --uninstall` rows no
longer land between the question and the outcome.

Banners before a command (one state line and one fix line each)
- fp-reset: a newer layout is a ✕, since it stops the command. The
stale-home reorganisation is a ✓ with what was rebuilt and what was
kept, a ▲ for undelivered batches and a ✕ for a failed step. The three
daemon self-heals and the version-skew hint are ▲ plus a fix, with
"denies every tool call" kept on one line. The kit is loaded with
import() inside checkLayoutForCli, so handler.ts's hook path (it
imports fp-reset) never loads the renderer.
- The `--token` warning, and the aborted-setup hint in
  onboarding-attempt.ts.

publish and jev status
- publish: header("Publish"), a ✓ per side effect (git commit, bundle,
created repository), "✓ Published <id>@<version> with N policies."
(the repository and tag are named only when they differ from the id
and version), then " Anyone can install it: failproofai policies add
  <repo>". The private-repo variant puts a ▲ in the install line's place,
and the private refusal is a ✕ notice.
- jev status: header("Jev"), a status row, then provider and model,
endpoint, mode, timeout, key, config with its permissions, and what Jev
may clear; any ▲; the ACTIVITY block; the next commands last. There is
no "last test" row (D16: nothing records one) and no key tail (nothing
in this module prints the key). --json is untouched.

Root-required copy
- update: an install that needs root says "✕ failproofaid <v> was not
installed: root privileges are required. The previous daemon is
untouched." and "Run `sudo -v` and then `failproofai update` again",
instead of "re-run failproofai config" plus a unit file to paste.
printReport paints a leading state glyph by role.
- uninstall: a service it could not remove gets "Remove it as root:",
the exact sudo commands, and where to check it.

Audit cache keys unchanged: engineVersion=10c367f637740d67
detectorVersion=895c2fedece85e93. The first build moved engineVersion:
Bun orders modules by where each import() first appears, and a kit
import in the --hook usage-error branch, the first import in bin,
pulled tui.ts ahead of builtin-policies.ts. Its top-level `ctx` then
renamed every policy's `ctx` parameter to `ctx2`. That branch now
imports nothing, and the builtin-policies module is byte-identical to
the base build.

Tests updated on purpose (copy or behaviour this change sets)
- cli-args.e2e: "Unknown command: X" becomes "✕ There is no command
called 'X'.". The "always offers a runnable suggestion" case becomes
"suggests nothing for a word close to no command", because Q6 changes
exactly that. The closest-flag case uses --versoin, since
  --unknownflag now gets no guess.
- help-index: the unknown-topic error's new words. The index check now
looks for those words; checking the old ones would test nothing.
- uninstall-cli: the dry-run and cancel lines, the plan prefix, and the
"who decides the daemon's fate" block rewritten for one question (the
two keep-the-service cases now expect removal on yes, per D17).
- publish-command: "Published X to Y at tag Z." becomes "Published X
with N policies." in seven places. The short form is printed only when
tag and version agree, so the pin still guards that, and the comments
say so. "3 assets attached" is dropped (the uploads are asserted
directly), the install line has its new words, and "PRIVATE" is
  "private".
- jev-cli, jev-cli-status-reviewable, jev-cli-status-stats: "Jev is off"
becomes the status row "○ off: there is no …". jev-cli-cloud: the
  separate-steps check reads one-line steps.
- fp-reset kept every pinned phrase; the test that wants what was
rebuilt AND what was kept is why the rebuilt line stays.

New tests: did-you-mean (table hits, typos, the cut-off, silence); the
kit's notice() and confirm() with colour on and off; the D17 question's
exact wording and colour; the banners' shape and colour; the update
root-required lines; the publish done screen (side effects, a
differing tag, colour on a terminal); the jev status layout and colour;
the --token warning's shape; and the --cli errors and the printer's
shape end to end.
