Summary

Neither duplicated accessible names nor split flex/grid text runs appear in the
screen reader testing workflow, so a tester would not look for them. Both are
visible in the accessibility tree before they are audible, so the workflow now
points at where to confirm them.

What changed

  • skills/testing-coach/references/screen-reader-testing.md: two additional
    checks in the NVDA list, and an "Extra Verbosity: Duplicated Names and Split
    Text Runs" section (duplicated names via Chrome DevTools; split runs only in
    Safari / iOS VoiceOver).
  • CHANGELOG.md, skills/a11y-core/references/sources.md.

A labelled field that also carries a placeholder announces its name twice, and
a flex or grid row that reads as one phrase takes one swipe per child on WebKit.
Both are visible in the accessibility tree before they are audible: the
duplicated name in Chrome DevTools > Accessibility, the split run only in
Safari or iOS VoiceOver. Add them to the screen-reader testing checklist.
