Tracking issue for the v0.9.3 stable release to npm.

Scope

Focused polish on the altimate review dbt PR reviewer plus TUI startup UX and audit-trail hardening on the signed verdict envelope. See CHANGELOG.md for the full user-facing entry.

Governance notes

1. Release PR flow used instead of direct-to-main because branch protection blocked the direct-push shape (recent releases used direct pushes). PR release: v0.9.3 #1041 approved + admin-merged with 1 pre-existing test-isolation failure bypassed — the failing check (CI/TypeScript) was caused by test/mcp/lifecycle.test.ts:250 (a spyOn(os, "homedir") in beforeEach with no matching afterEach.restore()), unrelated to v0.9.3 and not exercised by the release workflow. Follow-up filed separately for that test-isolation bug.
2. Stale fork-inherited v0.9.3 tag from anomalyco/opencode's own September 2025 release was manually deleted before tagging (user-approved). This was exactly the tag-collision scenario the preflight script was written to catch — flagged, surfaced, and resolved explicitly rather than auto-deleted.

v0.9.3 is live. No follow-up PRs needed for this release.
