Subject: [PATCH 1/2] fix: pin the verified CLI releases to 2.1.220 and 0.147.0

claude 2.1.220 and codex-cli 0.147.0 both pass the live suite unchanged.
Every Claude flag the crate passes still resolves, including
--permission-prompt-tool, which left --help but is still accepted. Every
app-server method the Codex path speaks is still in the 0.147.0 protocol.

Copilot stays at 1.0.78. Its four live failures are monthly quota
exhaustion on the account, not a flag or protocol change.
